First-line eradication for Helicobacter pylori-positive gastritis by esomeprazole-based triple therapy is influenced by CYP2C19 genotype

AIM: To evaluate the effect of first line esomeprazole(EPZ)-based triple therapy on Helicobacter pylori(H. pylori) ***: A total of 80 Japanese patients with gastritis who were diagnosed as positive for H. pylori infection by endoscopic biopsy-based or ^(13)C-urea breath tests were included in this study. The average age of the patients was 57.2 years(male/female, 42/38). These patients were treated by first-line eradication therapy with EPZ 40 mg/d, amoxicillin 1500 mg/d, and clarithromycin 400 mg/d for 7 d. All drugs were given twice per day. Correlations between H. pylori eradication, CYP2C19 genotype, and serum pepsinogen(PG) level were analyzed. This study was registered with the UMIN Clinical Trials Registry(UMIN000009642).RESULTS: The H. pylori eradication rates by EPZbased triple therapy evaluated by intention-to-treat and per protocol were 67.5% and 68.4%, respectively, which were similar to triple therapies with other first-generation proton pump inhibitors(PPIs). The eradication rates in three different CYP2C19 genotypes, described as extensive metabolizer(EM), intermediate metabolizer, and poor metabolizer, were 52.2%, 72.1%, and 84.6%, respectively. The H. pylori eradication rate was significantly lower in EM than non-EM(P < 0.05). The serum PG?Ⅰ?level and PG?Ⅰ/Ⅱ ratio were significantly increased after eradication of H. pylori(P < 0.01), suggesting that gastric atrophy was improved by H. pylori eradication. Thus, first-line eradication by EPZbased triple therapy for patients with H. pylori-positive gastritis was influenced by CYP2C19 genotype, and the eradication rate was on the same level with other firstgeneration PPIs in the Japanese ***: The results from this study suggest that there is no advantage to EPZ-based triple therapy on H. pylori eradication compared to other firstgeneration PPIs.

The relationship between tyrosine kinase inhibitor therapy and overall survival in patients with non-small cell lung cancer carrying EGFR mutations

For patients with epidermal growth factor receptor (EGFR) mutation-positive lung cancer, the relationship between the dose or duration of treatment with tyrosine kinase inhibitor (TKI) and overall survival remains unclear. Here, we analyzed clinical data of 39 patients who were diagnosed with EGFR mutation-positive non-small cell lung cancer and treated with TKI, but subsequently died. Several parameters were measured in this study: overall survival; first, second, and overall TKI therapy durations; first TKI intensity (actual dose/normal dose); and TKI rate (overall TKI therapy duration/overall survival). The response rate to TKI therapy was 50% , and the median survival was 553 days. After TKI therapy failed, 38.5% patients were re-challenged with TKI. We observed a moderate relationship [r = 0.534, 95% confidential interval (CI) = 0.263 to 0.727, P < 0.001] between overall TKI therapy duration and overall survival. However, we found no relationship between overall survival and first TKI intensity (r = 0.073, 95% CI = -0.380 to 0.247, P = 0.657) or TKI rate (r = 0.0345, 95% CI = -0.284 to 0.346, P = 0.835). Nonsmall cell lung cancer patients with mutation-positive tumors remained on TKI therapy for, on average, 33% of the overall survival time. These findings suggest that patients with EGFR mutation-positive tumors should not stick to using TKIs.

The Relationships between Dyeing Methods and Dyeability in Hair Colouring by Utilising Enzymatic Oxidation of (+)-Catechin

The enzymatic oxidation of (+)-catechin (Cat) with copper-containing oxidase, tyrosinase, was applied to hair colouring in the study. The Cat is oxidised to be o-benzoquinone form, the catechinone, which works as the hair dyestuff. Four kinds of hair colouring techniques using Cat were examined and each of their dyeability was compared in order to improve it. The first dyeing method is dyeing hair by the redissolved catechinone, which is preliminarily obtained by the oxidation of Cat (Redissolution Dyeing Method). The second one is dyeing hair in the dye solution just after the dye being formed by the enzymatic reaction (Sequential Dyeing Method). The third one is dyeing hair in solution containing Cat and the dyeing process is accompanied by the oxidation of Cat (Simultaneous Oxidation Dyeing Method). The fourth one is two-step dyeing that the hair immersed in Cat solution at the first step and then it is oxidised enzymatically to colour at the second step (Post-Oxidation Dyeing Method). The hair is dyed pale yellow by Redissolution Method, deeper yellowish brown by Sequential and Simultaneous Oxidation Dyeing Method and slightly reddish brown by Post-Oxidation Method with Cat. The order of their dyeability is the Simultaneous Oxidation ≈ Post-Oxidation > Sequential > Redissolution Dyeing Method at 30°C. The dyeability of Redissolution Method increases with an increase in dyeing temperature (T) between 20°C - 60°C. In Simultaneous Oxidation Method, the T increment also provides higher dyeability, and the colour of dyed hair becomes duller over 50°C. The Post-Oxidation Method shows higher dyeability at the lower oxidation temperature (20 °C - 40°C).
