Post traumatic diaphragmatic injuries have long been known. However their varied clinical, expressions lead to difficulties which cause its delay. The occurrence of herniation of hollow viscera in the thoracic cavity followed by its necrosis or perforation, is a delayed complication, a rare entity with a poor prognosis. The discovery of a colonic tumor in a diaphragmatic hernia is an exceptional clinical circumstance. Here we report the case of a patient with a complicated diaphragmatic hernia, whose symptoms are precipitated by the presence of a colon stenosing tumor. The management consisted of an exclusive laparotomy had allowed dealing in one surgical intervention with both the abdominal and thoracic injuries.

Single Pulmonary Nodule Management: Experience of a Moroccan Center about 24 Patients

Introduction: The management of single pulmonary nodule remains difficult, since it is of multiple etiologies. The endemicity of tuberculosis and hydatidosis in our context makes the etiological orientation even more difficult. Materials and Methods: This is a retrospective study, involving 24 patients, all operated on for a single pulmonary nodule in the department of thoracic surgery of CHU Hassan II of Fez over a period of 8 years. Results: There were 15 men and 9 women, with an average age of 51 years. 45.4% of patients had a history of known neoplasia. Discovery was fortuitous in 54% of cases. The radiologic diagnosis based on thoracic computed tomography had objectified an isolated nodule in all the patients with a right localization in 71% of the cases It was a solid pulmonary nodule in 91.5% of cases, and a frosted glass appearance in 8.5%. Bronchial fibroscopy was contributory in one case which was a carcinoid tumor. The approach was a conservative postero-lateral thoracotomy in 22 patients. The gesture was atypical resection in 67%, and lobectomy in 24.5% of cases, in the context of centrilobular nodules. The etiologies were dominated by pulmonary metastases in 29% of cases, pulmonary tuberculosis in 21% of cases. The postoperative course was simple in all patients. Conclusion: The pathological antecedents and the characteristics of the nodule on the imaging are essential elements allowing an etiological orientation. However, the pathological study remains the only method to confirm the diagnosis.

The objective of this study was to determine the polyphenol, flavonoid and tannin content and the antioxidant power of methanolic extracts from the different parts of the Senegalese baobab fruit. Phytochemical screening revealed the presence of saponosides, tannins, alkaloids, sterols, flavonoids, coumarins and total sugars in all extracts from the fruit parts. The total polyphenol content was determined by the folin-ciocalteu method. This method is based on the quantification of the total concentration of hydroxyl groups present in the extract. In an alkaline medium, the reagent of folin-ciocalteu, oxidizes the phenols to ion phenolates and partially reduces its hetero-polyacids, hence the formation of a blue complex. The absorbance is read at 765 nm against a control. The Flavonoids vere determined using aluminum trichloride and sodium hydroxide. Aluminum trichloride forms a yellow complex with flavonoids and sodium hydroxyde forms a pink complex absorbing in the visible range at 510 nm. The alternative colorimetric method based on reactions with vanillin in an acidic medium made it possible to determine the tannin content. Absorbances were measured at 500 nm. The results show that the methanolic extracts of the fibers and shell have very high polyphenol, flavonoid and tannin contents. In fact, the polyphenol contents of the fiber (159.00 ± 0.93 μg EAG/mg extract) and shell (155.39 ± 0.89 μg EAG/mg extract) were much higher than those of the pulp (27.21 ± 0.26 μg EAG/mg extract) and seeds (18.36 ± 0.07 μg EAG/mg extract). A Rare Localization of Tuberculosis: A Case Report and a Review of the Literature

The hepatic tuberculosis is rare. The interest of this Knowledge consists in the diagnostic problems particularly in its primitive and macronodular forms. It touches all ages with a peak of frequency during 17 - 50 years old, and a female prevalence. The clinical picture is polymorphous but it is still dominated by the prolonged fever and the alteration of the general state. Case report: 30-year-old female was admitted to our hospital with painless swelling on the right side of chest and feeling of pressure in the right upper quadrant of abdomen. She has no history of fever, weight loss, and night sweating or poor appetite. On clinical examination, the patient was in good general condition. The inflammatory tumefaction of the right chest wall was marked. The palpation of the abdomen revealed a sensitive hepatomegaly. A CT scan revealed a large cystic liver. The laboratory investigations were normal. This appearance was suggestive of a hydatid cyst of the right liver and a surgical management was decided. But the histological study revealed granulomatous inflammation consisting of tuberculoid type granuloma with caseous necrosis. However, abscess of the liver due to MT was highly suspected and anti-tuberculosis therapy was started, the patient responded well. Conclusion: The analysis of this observation and the data of the literature make it possible to retain the rarity;the polymorph expression and not very suggestive of this location.

Few cases of Liposarcoma of the spermatic cord have been reported in the literature. This rare tumor represents 7% of malignant tumors of the spermatic cord. We report a case of an unusual liposarcoma of the spermatic cord, with respect to its sclerosing variant, occurring in a healthy looking 42 years old man. A wide local excision of the tumor was performed. Adjuvant radiotherapy may sometimes be effective on local recurrence. Even though this tumor evolves slowly, a prolonged follow up is required because of the risk of late recurrence.

Hepatic hydatid cyst is a rare parasitic affection which constitutes a major health problem in countries endemic to hydatidosis. Rupture of hydatid cyst into gallbladder is an exceptional complication first described in 1952 by Atlas and Kamenear. We report the case of a hepatic hydatid cyst rupture into gallbladder in a 31-year-old patient with no history of disease.

Choledochal cyst is defined as a cystic dilatation of the distal common bile duct protruding into the duo- denum. It is considered as the rarest congenital cyst of the biliary tract by 1.4%. We report a 46 years old woman who presented with recurrent jaundice associated with abdominal pain of 07 years duration. The liver function tests showed cholestasis. The abdominal scan and Biliary MRI revealed a dilatation of intrahepatic, pancreatic ducts and a dilatation of the common bile duct with a stenosis in its the lower part. The diagnosis of a common bile duct cholangiocarinoma was mentioned. The gastroscopy revealed a stenosed duodenal bulb not allowing us to perform an echo endoscopy and ERCP. An intraoperative cholangiography illustrating a cystic dilatation of the papillary region in which exist a separate protrusion of the choledochal and wirsung ducts. Therefore, we didn’t accomplish the cephalic duodeno-pancreatectomy and we decided to realize a partial resection of the papilla. The histolological examination proved the absence of any tumoral lesion and the presence of biliary mucosa layered the internal surface of the cyst. The patient is still asymptomatic after one year of the surgery.

进行这项工作的目的是利用微生物的生物测定法MetPLATEM^TM，评价径流、被金属污染的土壤、Marrakech区两个采矿点的地下水中的金属毒性。这种生物测定方法主要是根据金属污染引起的Esther ichia大肠菌突变株的β半乳糖苷酶的特异性抑制。两个矿区所有采样站的河水样全部都是毒性的．而且呈现出的酶抑制超过87％．矿区C中SWA4和SWB1站除外。这些河水的Cu和Zn的浓度高。说明MetPLATE表现出了剧毒性。B和C矿河水的pH值在2．1和6．2之间变化，可能是金属的活化作用造成的，这就提出了这些矿区酸性矿山排水的问题。生物测定的MetPLATE方法还应用于酸性水污染的尾矿和土壤的测定。结果表明．由于可溶性Zn和Cu的浓度较高导致这些土壤和尾矿的毒性高。地下水毒性测试使用的MetPLATE方法说明。除了B矿GW3外、（潮湿季节抑制作用95.3％，干燥季节抑制作用82．9％），其余的呈现出的金属的毒性都低，抑制作用为2．7—45．5％。这种毒性高的原因主要是铜（189μg Cul^-1）和锌（505μgZnl^-1）的浓度高于常见的浓度。这些结果说明这两个含金属矿附近的不同生态系统的潜在风险。观察的总的趋势是MetPLATE测定的金属毒性随着研究的母岩中总金属浓度和移动的金属浓度的增加而增加。因此。MeIPLATE生物测定是评估水样和土样金属毒性的可靠而且快速的生物测定方法。
