Objective:the purpose of this study was to investigate the safety and efficacy of alvimopan,a novel peripherally acting mu-opioid receptor antagonist,in patients who undergo simple total abdominal hysterectomy. Study design:Women (n = 519)-were randomized (4:1) to receive alvimopan 12 mg (n = 413) or placebo (n = 106)≥2 hours before the operation then twice daily for 7 days (hospital and home). Adverse events were monitored up to 30 days after the last dose of study drug was administered. Efficacy was assessed for 7 postoperative days. Results:Overall,the most common adverse events were nausea,vomiting,and constipation; tients discontinued use because of adverse events. Alvimopan significantly accelerated the time to first bowel movement (hazard ratio,2.33; P time to first bowel movement was reduced by 22 hours,with more frequent bowel movement and better bowel movement quality found in the treatment cohort. Conclusion:Alvimopan has a safety profile that is similar to that of placebo and provides significantly improved lower gastrointestinal recovery in women who undergo simple total abdominal hysterectomy.

Vulvar carcinoma is rare in the premenopausal age group. the incidence during pregnancy is even more uncommon. to date, no cases of recurrent vulvar carcinoma in pregnancy have been reported. We describe the first case of recurrent vulvar carcinoma during the same pregnancy. A 36- year- old G5P4004 presented with vulvar pain at 18 weeks of pregnancy. She had a 2.2- cm tender mass anterior to the left labium majus, of which a biopsy revealed invasive squamous cell carcinoma. At 23 weeks gestation, she underwent a modified radical vulvectomy and bilateral inguinofemoral lymph node dissection. Eleven weeks later, she complained of a newly tender area to the right of the clitoris. A biopsy at that time demonstrated predominantly severe vulvar intraepithelial neoplasia (VIN III) with a small focus of invasive squamous cell carcinoma. A radical local excision was performed at 9 weeks postpartum. Careful surveillance is warranted as squamous cell carcinoma recurrence can occur quickly in the setting of pregnancy.

Purpose: the aim of this study was to compare mechanical muscle function in the eccentric/concentric phases of vertical bilateral jumping in anterior cruciate ligament-reconstructed(ACLR), elite(ELItE), and adolescent(ADOL) alpine ski racers and ski cross ***: Alpine ski racers/ski crossers(ACLR: n = 12, age = 26.7 § 3.8 years; ELItE: n = 12, age = 23.9 § 3.0 years; ADOL: n = 12, age = 17.8§ 0.7 years; females: n = 6 per group, males: n = 6 per group) performed 5 maximal countermovement.j.mps(CMjs) and 5 squat.j.mps. the ground reaction forces for each limb were analyzed using dual force plate recording to obtain body center of mass(BCM) velocity, displacement,and power. the eccentric deceleration(ECC) and concentric phases were determined from BCM velocity. CMj net concentric and ECC impulses were calculated(body mass normalized) along with the peak and mean BCM power and maximal vertical jump height. CMj lower limb stiffness(LLS) was determined by the slope of the ground reaction forces vs. the BCM displacement curve over the ECC phase. Concentric and ECC asymmetry indices were calculated for each leg, and the left vs. right LLS was compared. Plasmonic grating structures have been shown effective at increasing near-field optical enhancement. A doublewidth plasmonic grating design is introduced, where each period has two alternating metal widths separated by a nanogap. With this new design, analysis has shown that plasmonic resonances couple between each metal section,resulting in even greater optical enhancement compared with single-width gratings. the geometry that gives the greatest optical enhancement has been determined with a computational model. this work demonstrates that the increased enhancement is due to hybridized modes that couple between the two grating segments.

Metal-semiconductor-metal photodetectors on semi-insulating Ga As with interdigital electrodes showed significant enhancement in the spectral response in the near-infrared region as the electrode spacing is reduced. the photocurrent for the device with 5 μm interdigital spacing is five orders of magnitude higher than the dark current, and the room temperature detectivity is on the order of 2.4 × 1012cm Hz1∕2W-1at 5 V bias. Furthermore,the spectral response of this device possesses strong dependence on the polarization of incident light showing potential plasmonic effects with only microscale dimensions. these experimental data were analyzed using optical simulation to confirm the response of the devices.

Objective. End-of-life (EOL) medical care consumes 10-12%of national health care expenditures and 27%of Medicare dollars annually. Studies suggest that hospice services decrease EOL expenditures by 25-40%. the goal of this study was to compare the total cost of hospital-based resources utilized in ovarian cancer patients during their last 60 days of life for those enrolled in hospice versus those not on hospice. Methods. Study eligibility included patients who expired from ovarian cancer from 1999 to 2003. Medical records were reviewed for demographic data as well as treatment, response and recurrence rates, histologic type, grade and stage. Billing records were analyzed for costs of inpatient and outpatients visits, including radiologic, laboratory and pharmacy charges. total cost of hospital resources was compared between patients managed on hospice for >10 days (hospice group) versus the following methods: Mann-Whitney U, Kruskal-Wallis and Student’s t tests. Overall survival was compared using Kaplan-Meier statistics. Results. Of the 84 patients analyzed, 67 (79.8%) were in the non-hospice group and 17 (20.2%) were in the hospice group. Demographic, histologic and staging characteristics as well as platinum sensitivity were similar between the two groups before the last 60 days of life. Mean number of chemotherapy cycles before the study period was also similar (20.4 and 21.0, respectively). However, during the study period, the mean total cost per patient in the non-hospice group was ＄59,319 versus ＄15,164 in the hospice group (P = 0.0001). A significant difference in cost was noted for mean inpatient days (＄6584 vs ＄1629, P = 0.0007), radiology (＄6063 vs ＄2343, P = 0.003), laboratory (＄12,281 vs ＄2026, P = 0.0004) and pharmacy charges (＄13,650 vs ＄4465, P = 0.0017) as well as for treating physician per patient (＄112,707 vs ＄34,677, P = 0.04). Background: About 20%of patients with familial hemiplegic migraine (FHM) deve lop progressive cerebellar signs. Genetic studies have established an associatio n with mutations in the CACNA1A gene. However, the mechanisms underlying cerebel lar involvement are largely unknown. Objective: to use proton MR spectroscopy (1 H-MRS) to investigate metabolic alterations in the cerebellum as well as cortic al regions known to be involved in the propagation of migraine aura. Methods:Fif teen CACNA1A mutation carriers from three FHM families and 17 healthy control su bjects were studied. Eleven patients had clinical signs of cerebellar involvemen t. LCModel fits were used to estimate absolute concentrations of N-acetylaspart ate (NAA), myo-inositol (ml), glutamate (Glu), choline containing compounds, to tal creatine, and lactate in the superior cerebellar vermis (SCV), parietal cort ex, and occipital *** control for atrophy effects, automated image segment ation was performed using SPM99. the brain parenchyma fraction(BPF) was determin ed for all three regions. Results: Compared with controls, the brain parenchyma fraction (BPF), NAA,and Glu were significantly reduced and mI was significantly elevated in the SCV of patients with FHM. In contrast, no metabolite alterations were found in supratentorial regions. BPF and NAA in the SCV significantly corr elated with cerebellar scores, in particular, gait ataxia. Conclusions: the find ings suggest that there is a regionally distinct neuronal impairment in the superio r cerebellar vermis that exceeds macroscopic tissue loss. Correlations with clin ical scores emphasize the functional relevance of localized atrophy (brain paren chyma fraction) and N-acetyl aspartate levels. these measures may be useful to monitor disease progression. the observed reductionin glutamate may in part refl ect impaired glutamatergic neurotransmission.
