Almitrine for COVID-19 critically ill patients – a vascular therapy for a pulmonary vascular disease: Three case reports

BACKGROUND Several reports with clinical,histological and imaging data have observed the involvement of lung vascular function to explain the severe hypoxemia in coronavirus disease 2019(COVID-19)*** has been hypothesized that an increased pulmonary blood flow associated with an impairment of hypoxic pulmonary vasoconstriction is responsible for an intrapulmonary ***-19 may lead to refractory hypoxemia(PaO_(2)/FiO_(2) ratio below 100 mmHg)despite mechanical ventilation and prone *** hypothesized that the use of a pulmonary vasoconstrictor may help decrease the shunt and thus enhance *** SUMMARY We report our experience with three patients with refractory hypoxemia treated with almitrine to enhance *** dose almitrine(Vectarion®;Servier,Suresnes,France)was started at an infusion rate of 4μg×kg/min on a central *** PaO_(2)/FiO_(2) ratio and total respiratory system compliance during almitrine infusion were *** the three patients,the PaO_(2)/FiO_(2) ratio time-course showed a dramatic increase whereas total respiratory system compliance was *** three patients were discharged from the intensive care *** intensive care unit length of stay for patient 1,patient 2 and patient 3 was 30 d,32 d and 31 d,*** from mechanical ventilation was performed 13 d,18 d and 15 d after almitrine infusion for patient 1,2 and 3,*** found no deleterious effects on the right ventricular function,which was similar to previous studies on almitrine *** Almitrine may be effective and safe to enhance oxygenation in coronavirus disease 2019 *** controlled studies are required.

In 1993,the World Bank released a global report on the efficacy of health promotion,introducing the disability-adjusted life years(DALY)as a novel *** DALY,a composite metric incorporating temporal and qualitative data,is grounded in preferences regarding disability *** review delineates the algorithm used to calculate the value of the proposed DALY synthetic indicator and elucidates key methodological challenges associated with its *** contrast to the quality-adjusted life years approach,derived from multi-attribute utility theory,the DALY stands as an independent synthetic indicator that adopts the assumptions of the Time Trade Off utility technique to define Disability *** to rely on no mathematical or economic theory,DALY users appear to have exempted themselves from verifying whether this indicator meets the classical properties required of all indicators,notably content validity,reliability,specificity,and *** DALY concept emerged primarily to facilitate comparisons of the health impacts of various diseases globally within the framework of the Global Burden of Disease initiative,leading to numerous publications in international *** widespread adoption,the DALY synthetic indicator has prompted significant methodological concerns since its inception,manifesting in inconsistent and non-reproducible *** the substantial diffusion of the DALY indicator and its critical role in health impact assessments,a reassessment is *** reconsideration is imperative for enhancing the robustness and reliability of public health decisionmaking processes.

Apoptosis in skeletal muscle and its relevance to atrophy

Apoptosis is necessary for maintaining the integrity of proliferative tissues, such as epithelial cells of the gastrointestinal system. The role of apoptosis in post mitotic tissues, such as skeletal muscle, is less well defined. Apoptosis during muscle atrophy occurs in both myonuclei and other muscle cell types. Apoptosis of myonuclei likely contributes to the loss of muscle mass, but the mechanisms underlying this process are largely unknown. Caspase-dependent as well as -independent pathways have been implicated and the mode by which atrophy is induced likely determines the apoptotic mechanisms that are utilized. It remains to be determined whether a decrease in apoptosis will alleviate atrophy and distinct research strategies may be required for different causes of skeletal muscle loss.

2-Dimensional FEM modeling of macrosegregation in the directional solidification with mesh adaptation

In order to improve the prediction accuracy of macrosegregation channel, an algorithm for dynamic remeshing is proposed. The basic idea is to generate fine elements near the liquidus isotherm. The norm of the gradient of solid fraction is used for piloting the remeshing in the mushy zone; whereas, the objective mesh size in the liquid is considered as a function of the distance to the liquidus isotherm. The efficiency of mesh adaptation is demonstrated by prediction of macrosegregation channel in a case of unidirectional solidification.

Solids behavior in dilute zone of a CFB riser under turbulent conditions

The behavior of the solid phase in the upper zone of a circulating fluidized bed riser was studied using a phase Doppler anemometer. Glass particles of mean diameter 107μm and superficial gas velocities UE covering the turbulent and the beginning of the fast fluidization regime were investigated. Three static bed heights were tested. Ascending and descending particles were found co-existing under all oper ating conditions tested, and at all measurement locations. Superficial gas velocity proved/happened to have a larger effect on descending particles at the wall and on ascending particles in the central region. Transversal particle velocities in both directions （toward the center and toward the wall） behaved rela- tively equivalently, with only slight difference observed at the wall. However, observation of the number of particles moving in either transversal direction showed a change in bed structure when increasing Ug. Furthermore, a balance was constantly observed between the core zone and the annulus zone where the mutual mass transfer between these two zones occurred continuously. Transition from a slow to a fast particle motion was accompanied by a transition to high levels of velocity fluctuations, and was found corresponding to the appearance of significant solid particle flow rate.

Displacement manifold method for type synthesis of lower-mobility parallel mechanisms

Type synthesis of lower-mobility parallel mechanisms is a hot and frontier topic in international academic and industrial field. Based on the Lie group theory, a displacement manifold synthesis method is proposed. For all the nine kinds of lower-mobility parallel mechanisms, the mechanism displacement manifold, limb displacement manifold and the geometrical conditions which guarantee that the intersection of the limb displacement manifold is the desired mechanism displacement manifold are enumerated. Various limb kinematic chains can be obtained using the product closure of displacement subgroup. Parallel mechanisms can be constructed with these limbs while obeying the geometrical conditions. Hence, all the nine kinds of lower-mobility parallel mechanisms can be synthesized using this method. Since displacement manifold deals with finite motion, the result mechanism of synthesis have full-cycle mobility. Novel architectures of lower-mobility parallel mechanisms can be obtained using this method.

Parentage analysis in Gabonese colonies of soil-feeding termites belonging to the Cubitermes sp. affinis subarquatus complex of species （Termitidae＂ Termitinae）

Cubitermes spp. are widely distributed soil-feeding termite species in sub- Saharan Africa which play a fundamental role in soil structure and fertility. A complex of at least four cryptic species （i.e., Cubitermes sp. affunis subarquatus complex of species） has been recently described using molecular markers. In order to investigate the breeding system of these species, five microsatellite markers were used to carry out parentage and re- latedness analyses in 15 Gabonese colonies. Monogamy was confirmed as the predominant reproductive organization in Cubitermes spp. （76% of the colonies）. Within 30% of these monogamous colonies, a high relatedness between reproductives was shown, suggesting that mating between related individuals occurs. However, Cubitermes colonies can deviate from monogamy. Indeed, parental contributions by at least two related reproductives of the same sex were revealed in four colonies and polyandry was demonstrated in two of them. Infiltration of reproductives in the colony is the most plausible explanation for such cases of polygamy in Cubitermes spp.
