Ezetimbe as potential treatment for cholesterol gallstones: The need for clinical trials

Obesity is associated with insulin resistance, nonalcoholic fatty liver disease (NAFLD) and gallstones. high fat diets (unsaturated fats) rich in cholesterol have been demonstrated to produce not only gallstones but also NAFLD and insulin resistance. Interestingly, a high incidence of gallstones is being reported in association with insulin resistance and NAFLD. Laparoscopic cholecystectomy is the best definitive therapy for symptomatic gallbladder disease. Ezetimibe is a drug that inhibits the absorption of both dietary and biliary cholesterol in the small intestine. Importantly, ezetimibe showed potential benefit not only in treating and preventing gallstones but also in insulin resistance and NAFLD. Further studies are required before the use of ezetimibe for the treatment of gallstones can be advocated.

Introduction: Malnutrition is a pathological state resulting from the relative deficiency or excess of one or more essential nutrients, whether manifested clinically or detected only by biochemical, anthropometric or physiological analyses. The overall objective was to assess the quality of management of acute malnutrition in children aged 0 - 24 months at the Boulbinet health center. Methodology: This was a prospective descriptive study lasting six (06) months from May 5 to October 5, 2018. The study included all children aged 0 to 24 months. Results: Acute malnutrition in children aged 0 - 24 months accounted for 2.11% of cases. The sex ratio was 1.41 in favor of males. The mean age of our patients was 5 months 7 days, with extremes of 1 month and 6 months. The majority came from Ra toma (40.24%). Exclusive breastfeeding was most common (54.02%). The main clinical signs were: pallor 49.42%, diarrhea 46.67, oral lesions37.96%. sAM represented 89.66% and MAM 10.34%. Most associated pathologies: anemia 49.42% and oral candidiasis 37.93%. In terms of outcome, we recorded 56.32% cures, 20.69% deaths, 18.39% dropouts and 4.60% cures. Conclusion: Improving the quality of care for malnourished children aged 0 - 24 months requires raising awareness among mothers and the general public of the consequences of malnutrition.

Obstructive sleep apnea (OsA) is a complex disorder that consists of upper airway obstruction, chronic intermittent hypoxia and sleep fragmentation. OsA is well known to be associated with hypoxia, insulin resistance and glucose intolerance, and these factors can occur in the presence or absence of obesity and metabolic syndrome. Although it is well established that insulin resistance, glucose intolerance and obesity occur frequently with non-alcoholic fatty liver disease (NAFLD), it is now becoming apparent that hypoxia might also be important in the development of NAFLD, and it is recognized that there is increased risk of NAFLD with OsA. This review discusses the association between OsA, NAFLD and cardiovascular disease, and describes the potential role of hypoxia in the development of NAFLD with OsA.

While the peripheral nervous system has the capacity to regenerate following a nerve injury,it is often at a slow rate and results in unsatisfactory recovery,leaving patients with reduced *** regeneration associated genes have been identified over the years,which may shed some insight into how we can manipulate this intrinsic regenerative ability to enhance repair following peripheral nerve *** lab has identified the membrane bound protease beta-site amyloid precursor protein-cleaving enzyme 1（BACE1）,or beta secretase,as a potential negative regulator of peripheral nerve *** beta secretase activity levels are abolished via a null mutation in mice,peripheral regeneration is enhanced following a sciatic nerve crush ***,when activity levels are greatly increased by overexpressing beta secretase in mice,nerve regeneration and functional recovery are impaired after a sciatic nerve crush *** addition to our work,many substrates of beta secretase have been found to be involved in regulating neurite outgrowth and some have even been identified as regeneration associated *** this review,we set out to discuss BACE1 and its substrates with respect to axonal regeneration and speculate on the possibility of utilizing BACE1 inhibitors to enhance regeneration following acute nerve injury and potential uses in peripheral neuropathies.

Biochemical Liver Functions and Molecular Identification of Fasciola hepatica from Experimentally Infected Rat Model

The current study was performed to evaluate the liver function status as well as molecular characterization of the recovered worms in rats experimentally infected with F. hepatica. sixteen male Wister rats aged 30 days were randomly allocated into two groups (n = 8). The first group was infected orally with 15 viable encysted metacercaria of F. hepatica per animal. The other group was kept non-infected (control group). At zero time (before infection), the 2sup>ndsup>, 4sup>thsup>, 6sup>thsup>, 8sup>thsup>, 10sup>thsup>, 12sup>thsup> and 14sup>thsup> weeks post-infection (WPI), blood and serum samples were collected via puncture of retro-orbital plexus of veins from each rat. serum enzyme level (AsT and ALT) and total protein were measured, and the serum protein profile was carried out using agarose gel electrophoresis. During the period of the experiment, serum ALT and AsT activities and serum total globulins significantly increased while serum total proteins and albumin markedly decreased in the infected group. On the 14sup>thsup> WPI, the data of the electropherogram showed that globulin fractions (α1-, β- and γ-globulin) levels were significantly increased while α2-globulin was markedly decreased in the infected group. The molecular analysis confirmed the amplification of the ITs1, ITs2 and NDI genes of F. hepatica recovered from the infected liver of rats with amplicon sizes of 630, 510 and 560 bp, respectively. sequencing of the amplified ITs gene resulted in the determination of 3 strains (PP108836, PP108837, and PP108838). Also, analysis of the ITs2 gene resulted in obtaining 3 isolates under the accession numbers (PP109065, PP109066, and PP109067). In conclusion, fasciolosis in the rat model is suitable for routine experimental infections and caused a pronounced liver dysfunction with discharging of the Fasciola eggs in the faeces and the development of adult stages in the bile ducts. Introduction: Neonatal peritonitis is a rare but serious condition requiring early diagnosis and management. The lack of antenatal diagnosis, the precariousness of neonatal intensive care, and the delay in diagnosis and treatment are the factors of poor prognosis. The objective of this study was to study the epidemiological, diagnostic and therapeutic aspects of neonatal peritonitis in order to improve its management in the pediatric surgery department of the ChU-Donka. Materials and Methods: This is a retrospective descriptive study of the records of patients treated for neonatal peritonitis from January 1, 2017 to December 31, 2022. Results: Results: We compiled 17 records of patients operated on for neonatal peritonitis, representing a frequency of 3.85% and an incidence of 2.83 cases/year. The mean age of our patients was 8.29 days, with extremes of D0 and D25. Males were more dominant, with a frequency of 71% and a sex ratio of 2.4. The average consultation time was 72 h, with extremes of 12 h and 144 h. Abdominal bloating was the main reason for consultation (100%). Fever and vomiting were frequent, at 70.59% and 82.35% respectively. PsA revealed pneumoperitoneum in 12 patients (70.59%). The colon was the most frequent site of perforation at 29.41%. ECUN was the main etiology with 35.29%. Ostomy was the main surgical procedure in 58.82% of cases, followed by suture excision in 29.41%. Postoperative follow-up was simple in 3 patients (17.65%) and complicated in 14 (82.35%). sepsis was the main cause of death (47.59%). The mortality rate was 76.47%. Conclusion: Mortality from neonatal peritonitis remains high in developing countries. Early diagnosis, early surgical management, and the presence of well-equipped neonatal intensive care units with qualified staff are essential for an improvement in the prognosis of neonatal peritonitis.

The paper presents the investigation of the effect of alkaline treatment of sodium hydroxide(NaOh) on physical and dynamic mechanical analysis(DMA) viscoelastic properties of kenaf fibre filled natural rubber(NR)/thermoplastic polyurethane(TPU) *** treated kenaf fiber,NR and TPU were weighed and proportioned according to the required compositions and were blended using hot mixed Brabender *** polymer composites were then fabricated using the hot press to form a sample *** sample was cut and prepared and water absorption,density,thickness swelling and DMA tests were *** far as physical properties are concerned,composites with the highest NR amount of shows the best results,which indicates good fiber bonding *** polymer composites with the highest amount of TPU shows the highest damping properties at high temperature.

Egyptian heavy vacuum gas oil hydrotreating over Co-Mo/CNT and Co-Mo/γ-Al2O3 catalysts

The catalytic activity of CoMos /CNT towards the Egyptian heavy vacuum gas oil hydrotreating was studied. The delivered CNT was functionalized with 6 mol /L hNO_3. The CNT were loaded with 12% MoO_3( by weight) and 0.7 Co /Mo atomic ratio with impregnation methods. The γ-Al_2O_3 catalyst was also prepared by impregnation method to compare both catalysts *** analysis tools such XRD,Raman spectroscopy,TEM,and BET were used to characterize the catalysts. The autoclave reactor was used to operate the hydrotreating experiments. The hydrotreating reactions were tested at various operating conditions of temperature 325-375 ℃,pressure 2-6 MPa,time 2-6 h,and catalyst /oil ratio( by weight) of 1 ∶75,1 ∶33 and 1 ∶10. The results revealed that the CoMos /CNT was highly efficient for the hydrotreating more than the CoMos /γ-Al_2O_3. Also, the hydrodesulfurization( hDs) increased with increasing catalyst /oil ratio. Additionally,results showed that the optimum condition was temperature 350℃,pressure 4 MPa,catalyst /oil ratio of 1 ∶75 for 2 h. Furthermore,even at low CoMos /CNT catalyst /oil ratio of 1 ∶75,an acceptable hDs of 77.1% was achieved.
