The article describes the pantoprazole healing effect on the gastroduodenal mucosa in patients with NSAID gastropathy. Two groups of patients were compared, depending on the treatment they have got group, which was taking 75 mg of enteric aspirin per day and the group, where pantoprazole has been added to aspirin in the usual dose. Mucosa assessment was studied using fibrogastroduodenoscopy and Lanza score. Also, the relationship between aggression factors that cause NSAID gastropathy and mucosal protection agents has been studied. It was proved that the pantoprazole influence reduces the ulcerative-erosive lesions amount. Stomach erosive lesions percentage decreased from 58.18% to 42.42%, stomach ulcers from 14.55% to 6.06%, duodenum erosive lesions decreased from 34.55% to 24.24%, duodenum ulcers deceased from 9.09% to 3.03%. A positive correlation between LTB4 and Lanza scale was checked after pantoprazole treatment, indicating an impact on the LTB4 reduction in ulcers healing in patients with NSAID gastropathy.

AIM To evaluate incidence, risk factors and treatment outcome of BK polyomavirus nephropathy(BKVN) in a cohort of renal transplant recipients in the Auckland region without a formal BK polyomavirus(BKV) surveillance *** A cohort of 226 patients who received their renal transplants from 2006 to 2012 was retrospectively *** Seventy-six recipients(33.6%) had a BK viral load(BKVL) test and 9 patients(3.9%) developed BKVN. Cold ischaemia time(HR = 1.18, 95%CI: 1.04-1.35) was found to be a risk factor for BKVN. Four recipients with BKVN had complete resolution of their BKV infection; 1 recipient had BKVL less than 625 copies/mL; 3 recipients had BKVL more than 1000 copies/mL and 1 had graft failure from BKVN. BKVN has a negative impact on graft function [median estimated glomerular filtration rate(eG FR) 22.5(IQR 18.5-53.0) mL /min per 1.73 m^2, P = 0.015), but no statistically significant difference(P = 0.374) in renal allograft function was found among negative BK viraemia group [median e GFR 60.0(IQR 48.5-74.2) mL /min per 1.73 m^2), positive BK viraemia without BKVN group [median eG FR 55.0(IQR 47.0-76.0) mL /min per 1.73 m^2] and unknown BKV status group [median eG FR 54.0(IQR 43.8-71.0) mL /min per 1.73 m^2]. The incidence and treatment outcomes of BKVN were similar to some centres with BKV surveillance *** Recipients with BVKN have poorer graft function. Although active surveillance for BKV has been shown to be effective in reducing incidence of BKVN, it should be tailored specifically to that transplant centre based on its epidemiology and outcomes of BKVN, particularly in centres with limited resources.

Rebamipide and Pantoprazole Combination in NSAIDs-Gastropathy Treatment

The objective of this study was to investigate the pantoprazole and rebamipide efficiency on NSAIDs （nonsteroidal anti-inflammatory drugs）-gastropathy restoring and healing in patients with coronary heart disease, stable angina, who have been taking aspirin for a long time period. The study included three groups of patients according to the treatment they have got： ASA （acetylsalicylic acid） in the first group; ASA and pantoprazole in the second; ASA, pantoprazole and rebamipide in the third one. To obtain the results, endoscopic method and proinflammatory cytokines LTB4 and protective prostaglandin e2 determination in the blood were used. The research demonstrated no ulcer effects in the group of patients who were treated by rebamipide, and significantly fewer gastroduodenal erosions, in comparison to the group, where treatment contained ASA and pantoprazole. The LTB4 （leukotriene B4） level decreased in pantoprazole and rebamipide treatment groups, but the PGe2 （prostaglandin e2） level increased only after rebamipide therapy. Therefore, rebamipide should be included to the therapy for the better NSAIDs-gastropathy treatment, in reason of its good reparative and gastroprotective properties.

Age-related ecto-Nicotinamide Adenine Dinucleotide Oxidase Disulfide Thiol exchangers 3 (eNOX3) or age-related NADH oxidases (arNOX) are expressed at the cell surface as five members of the TM-9 superfamily, initially membrane anchored, all functionally similar, with the N-termini exposed at the cell’s exterior. eCTO-NOXes are cell surface proteins with both time-keeping CoQH2 [NAD(P)H] oxidase and protein disulfidethiol interchange activities. They are designated as eCTO-NOX proteins because of their localization on the outer surface of the plasma membrane and to distinguish them from the phox-NOXes of host defense. A ca. 30 kDa N-terminal fragment is cleaved and accumulates in body fluids (serum, saliva, urine, perspiration). arNOXes appear around age 30 and increase steadily thereafter. Reduced quinones, i.e., reduced coenzyme Q, of the plasma membrane are natural substrates. NAD(P)H is oxidized as an artificial substrate. In one phase of the arNOX cycle electrons are transferred to oxygen to generate superoxide. Substrates for the shed forms of arNOX appear to be proteins of body fluids. Circulating lipoproteins and skin matrix proteins emerge as potentially important health-related targets. Through oxidation of collagen, elastin and other proteins of the skin matrix, arNOXes are major contributors to skin aging through tyrosine and thiol oxidation and subsequent cross linking. The main destructive action of arNOX, however, may be to directly oxidize circulating lipoproteins. arNOX in the blood is structured as an integral component of the LDL particle through site-specific binding. As such, arNOXes are implicated as major risk factors for cardiovascular disease due to specific oxidation of LDLs. The superoxide produced and its conversion to hydrogen peroxide would be one part of the potentially destructive properties by contribution to lipid oxidation.
