Actinomycosis is an uncommon chronic infectious disease. Common sites of involvement include the cervicofacial, thoracic and abdominopelvic regions. In abdominopelvic actinomycosis, the ileocecal region, including the appendix, is the most commonly involved site. In some reports, limited appendiceal actinomycosis has revealed a thickened appendiceal wall with peri-appendiceal inflammation as acute appendicitis or perforated appendicitis. We experienced pathologically confirmed intraluminal limited appendiceal actinomycosis without peri-appendiceal infiltration. Here, we report the computed tomography and ultrasound findings.

AIM： To determine the changes of quantitative hepatitis B e antigen （HBeAg） that predicts early detection of non-response or breakthrough to long-term lamivudine （LAM） therapy. METhoDS： Among HBeAg positive chronic hepatitis B patients who failed to achieve HBeAg seroconversion within 12 too, we retrospectively analyzed 220 patients who had received LAM more than 24 too. RESULTS： The mean duration of LAM therapy was 36 （range, 24-72） mo. HBeAg seroconversion after the first 12 mo of LAM therapy was achieved in 53 （24.1%） patients. Viral breakthrough was observed in 105 （47.7%） patients. To find out whether the changing patterns of HBeAg levels can predict the outcome of LAM therapy, we analyzed the reduction rates of HBeAg levels during LAM therapy. Using the decrease more than 90% of pretreatment HBeAg levels, the sensitivity and specificity of response were 96.2% and 70.1%, respectively. Patients were divided into 3 groups according to the reduction patterns of the decrease of quantitative HBeAg： decrescendo, decrescendo-crescendo, no change or fluctuating groups. The optimal time to predict non-response or breakthrough was the first 9 mo of therapy. At 9 mo of therapy, 49 （92.5%） of 53 patients who had achieved HBeAg seroconversion were included in the decrescendo group. On the contrary, in the no change or fluctuating group, only four （7.5%） had achieved HBeAg seroconversion. Among patients who did not show the continuous decrease of HBeAg levels at 9 too, 95.2% （negative predictive value） failed to achieve HBeAg seroconversion. CONCLUSION： Almost all patients who failed to show a continuous decrease of HBeAg levels at 9 mo of LAM therapy were non-response or breakthrough. Therefore, monitoring changes of HBeAg levels during LAM therapy in HBeAg positive chronic hepatitis B may be valuable for identifying patients who are at high risk of non-response or breakthrough.

Office environments have recently adopted ubiquitous computing for collaboration and mobile communication to promote real-time enterprises. Ubiquitous offices, introduced by Weiser and adopted as emerging computational technology to support office works, have already affected the practice of companies and organizations. Within this context, this study deals with a work service model of the ubiquitous office environments by understanding human behaviors and works in their workspace. We propose a ubiquitous office model considering the correlation between ubiquitous computing technologies and work services in the office. Two attributes are emphasized, collaboration and mobility, as identifiers for categorizing the work types. The types of work services have variations in the amount of communication and the proportion of working outside of the office. The proposed work service model of the ubiquitous office includes territorial and non-territorial services to enable workers in and out of the office to interact with each other effectively. The findings in this paper would be a theoretical basis for embodying an intelligent office which supports office works efficiently.

4-1BB(CD137)is a strong enhancer of the proliferation of CD8^(+)T *** these cells require increased production of energy and biomass to support their proliferation,we hypothesized that 4-1BB signaling activated glucose and fatty acid *** found that treatment with agonistic anti-4-1BB mAb promoted the proliferation of CD8^(+)T cells in vitro,increasing their size and *** with a glycolysis inhibitor and a fatty acid oxidation inhibitor revealed that CD8^(+)T cell proliferation required both glucose and fatty acid ***-4-1BB treatment increased glucose transporter 1 expression and activated the liver kinase B1(LKB1)-AMP-activated protein kinase(AMPK)-acetyl-CoA carboxylase(ACC)signaling pathway,which may be responsible for activating the metabolism of glucose and fatty *** also examined whether blocking glucose or fatty acid metabolism affected cell cycle progression and the anti-apoptotic effect of 4-1BB *** increase of anti-apoptotic factors and cyclins in response to anti-4-1BB treatment was completely prevented by treating CD8^(+)T cells with the fatty acid oxidation inhibitor,etomoxir,but not with the glycolysis inhibitor,*** conclude that anti-4-1BB treatment activates glucose and fatty acid metabolism thus supporting the increased demand for energy and biomass,and that fatty acid metabolism plays a crucial role in enhancing the cell cycle progression of anti-CD3-activated CD8^(+)T cells in vitro and the anti-apoptotic effects of 4-1BB signaling on these cells.

Hepatocellular carcinoma （HCC） is a so highly invasive tumor that metastasizes hematogenously and lymphogenously to distant site. Frequent sites are lung, regional lymph node, bone, and adrenal gland. But metastasis to the gastrointestinal （GI） tract is rare, and most common site is stomach. Metastasis to the small intestine is extremely rare. Moreover, metastatic HCC of the small bowel causing intussusception has not been reported until now. Here, we report a case of metastasis of HCC to the small bowel manifested by intussusception.

Dear Editor,Angiosarcoma is a rare malignancy,which accounts for approximately 1%-2%of soft-tissue sarcomas[1].It can occur at any site within the human *** research-ers suggested that site distribution may be different between Eastern Asian and Western countries[1].Sev-eral factors,such as age,stage at diagnosis,performance status,site of the disease,histological grade,tumor size,and visceral involvement,were suggested as prognos-tic factors in angiosarcoma[2-5].however,there is still controversy regarding to which prognostic factors are significantly *** there are no randomized clinical trials for angiosarcoma,its treatments are guided based on guidelines of soft-tissue sarcoma or the results of several retrospective *** surgery is the preferred treatment for angiosarcoma[2-4].Some stud-ies suggested that receiving adjuvant chemotherapy(CT)is associated with better outcome[5],while other studies have shown that adjuvant CT has no significant effect[6,7].Chemoradiotherapy has been reported to be effective in angiosarcoma especially for cutaneous angiosarcoma[8].however,evidence for the efficacy and safety of pal-liative cytotoxic CT is still ***,here,we present our analysis based on the clinical features,tumor characteristics,treatment and outcomes of angiosarcoma patients in an attempt to investigate the significant prog-nostic factors and efficient therapeutics in real clinical practice.
