Urgent ERCP for acute cholangitis reduces mortality and hospital stay in elderly and very elderly patients

BACKGROUND： Acute cholangitis in old people is a cause of mortality and prolonged hospital stay. We evaluated the effects of methods and timing of biliary drainage on the outcomes of acute cholangitis in elderly and very elderly patients. METhoDS： We analyzed 331 patients who were older than 75 years and were diagnosed with acute calculous cholangitis. They were admitted to our hospital from 2009 to 2014. Patients＇ demographics, severity grading, methods and timing of biliary drainage, mortality, and hospital stay were retrospectively obtained from medical records. Clinical parameters and outcomes were compared between elderly （75-80 years, n= 156） and very elderly （≥81 years, n=175） patients. We analyzed the effects of methods [none, endoscopic retrograde cholangio- pancreatography （ERCP）, percutaneous transhepatic biliary drainage, or failure] and timing （urgent or early） of biliary drainage on mortality and hospital stay in these patients. RESULTS： Acute cholangitis in older patients manifested as atypical symptoms characterized as infrequent Charcot＇s triad （4.2%） and comorbidity in one-third of the patients. Patients were graded as mild, moderate, and severe cholangitis in 104 （31.4%）, 175 （52.9%）, and 52 （15.7%）, respectively. Urgent bili-ary drainage （≤24 hours） was performed for 80.5% （247/307） of patients. Very elderly patients tended to have more severe grades and were treated with sequential procedures of transient hiliary drainage and stone removal at different sessions. hospital stay was related to methods and timing of biliary drainage. Mortality was very low （1.5%） and not related to patient age but rather to the success or failure of biliary drainage and severity grading of the acute cholangitis. CONCLUSIONS： The methods and timing used for biliary drainage and severity of cholangitis are the major determinants of mortality and hospital stay in elderly and very elderly patients with acute cholangitis. Polyurethane-based Actuators with Various Polyols

This study dealt with the electrostrictive responses of polyurethane （PU） actuators with different microphase separation structure, which was a promising candidate for a material used in polymer actuators. In order to construct PUs with different higher-order structure, PUs with various types of polyol were synthesized： poly（neopentyl glycol adipate）（PNAD）, poly（tetramethylene glycol）（PTMG）, and poly（dimethyl siloxnae）（PDMS）. Synthesized PU was characterized by Fourier transform-infrared （FT-IR） spectroscopy and gel per- meation chromatography （GPC）. Thermal analysis and mechanical properties of PU films were carried out with differential scanning calorimetry （DSC） and UTM （universal testing machine）, respectively. And PU actuator was formed in a type of monomorph, which was made by carbon black electrodes on the both surfaces of PU film by spin coating method. Actuation behavior was mainly influenced on microphase separation struc- ture and mechanical property of PU. In result, PU actuator with PNAD, polyester urethane, had the largest field-induced displacement.

Clinical analysis of risk factors contributing to recurrence of pterygium after excision and graft surgery

AIM: To find the risk factors related to the reproliferation of the pterygial tissue after excision and graft ***: Charts of 130 eyes of 130 patients who had pterygial excision from March 2006 to April 2011 were reviewed. Preoperative pterygium morphology, surgical methods, and adjunctive treatments were statistically analyzed for their relationship with ***: During the follow-up period, recurrence was observed in 20 eyes(15.4%). None of the preoperative morphologic features were affected the rate of the recurrence. however, an age <40y [P =0.085, odds ratio(OR) 3.609, 95% confidence interval(CI) 0.838-15.540]and amniotic membrane graft instead of conjunctival autograft(P =0.002, OR 9.093, 95% CI 2.316-35.698) were statistically significant risk factors for *** analysis revealed that intraoperative mitomycin C(MMC)(P =0.072, OR 0.298, 95% CI 0.080-1.115)decreased the rate of recurrence. CONCLUSION: Younger age is a risk factor for reproliferation of pterygial tissue after excision and amniotic membrane transplantation(AMT) are less effective in preventing recurrence of pterygium after excision based on the comparison between conjunctival autograft and AMT. Intraoperative MMC application and conjunctival autograft reduce recurrence.

Inverse Problem of Flame Surface Properties of Wood using a Repulsive Particle Swarm Optimization Algorithm

The convective heat transfer coefficient and surface emissivity before and after flame occurrence on a wood specimen surface and the flame heat flux were estimated using the repulsive particle swarm optimization algorithm and cone heater test results. The cone heater specified in the ISO 5660 standards was used, and six cone heater heat fluxes were tested. Preservative-treated Douglas fir 21 mm in thickness was used as the wood specimen in the tests. This study confirmed that the surface temperature of the specimen, which was calculated using the convective heat transfer coefficient, surface emissivity and flame heat flux on the wood specimen by a repulsive particle swarm optimization algorithm, was consistent with the measured temperature. Considering the measurement errors in the surface temperature of the specimen, the applicability of the optimization method considered in this study was evaluated.

Optimization of Pyrolysis Properties using TGA and Cone Calorimeter Test

The present paper describes an optimization work to obtain the properties related to a pyrolysis process in the solid material such as density, specific heat, conductivity of virgin and char, heat of pyrolysis and kinetic parameters used for deciding pyrolysis rate. A repulsive particle swarm optimization algorithm is used to obtain the pyrolysis-related properties. In the previous study all properties obtained only using a cone calorimeter but in this paper both the cone calorimeter and thermo gravimetric analysis (TGA) are used for precisely optimizing the pyrolysis properties. In the TGA test a very small mass is heated up and conduction and heat capacity in the specimen is negligible so kinetic parameters can first be optimized. Other pyrolysis-related properties such as virgin/char specific heat and conductivity and char density are also optimized in the cone calorimeter test with the already decided parameters in the TGA test.

The commercial viability of thermoelectric(TE)devices relies heavily on two factors:cost reduction and efficiency *** this study,we first produce p-type Cu_(12)Sb_(4)S_(16-x)(x=0,3,4)using a low-temperature bottom-up approach and demonstrate Cu_(12)Sb_(4)S_(13)to show the best TE performance among the three tested ***,the TE energy conversion efficiency of Cu_(12)Sb_(4)S_(13)is further enhanced by optimizing its electronic band structure through the incorporation of small amounts of *** an optimal Te content of 5 mol%,more than a twofold increase in the TE figure of merit(zT)is *** gain insight into the mechanism of improvement on the transport properties of the mate-rial,we compare the interphase transport mechanism by incorporating nanodomains of different metals(Ag and Cu)into the Cu_(12)Sb_(4)S_(13)*** improved electrical conductivity obtained with Cu_(12)Sb_(4)S_(13)-Te nanocomposites is attributed to a charge flooding of the Cu_(12)Sb_(4)S_(13)*** contrast,excessive down-ward band-bending at the interphases of Ag/Cu metal-semiconductor drastically reduces the electrical ***,a weighted mobility(μw)analysis shows a dominant thermal activation of carri-ers in Cu_(12)Sb_(4)S_(13)-Te *** this material,a strong decrease in lattice thermal conductivity is also found,which is associated with a phonon-carrier scattering *** work shows the impor-tance of proper band-engineering in TE nanocomposites to decouple electrical and thermal transport to enhance TE performance,and the efficacy ofμw for electrical and thermal transport analysis.

Promoter hypermethylation and loss of CD133 gene expression in colorectal cancers

AIM: To understand CD133 promoter hypermethyl-ation and expression in 32 colorectal cancer cell lines. METhoDS: Nucleic acid was isolated from 32 colorectal cancer cell lines and CD133 expression levels were measured by reverse transcription-polymerase chain reaction (RT-PCR) and real-time PCR. Promoter methylation status of the CD133 gene was analyzed with a methylation-specific PCR after sodium-bisulfi te modification and by clonal sequencing analysis. The correlation between expression and promoter methylation of CD133 gene was confirmed with treatment of 5-aza-2’-deoxycytidine. RESULTS: We measured CD133 expression levels in 32 colorectal cancer cell lines. RT-PCR analysis showed undetectable or low levels of CD133 expression in 34.4%of cell lines. To verify the relation between CD133 expression and methylation status of the CD133 gene promoter in colorectal carcinogenesis, CD133 gene promoter hypermethylation was analyzed in 32 cancer cell lines. Promoter hypermethylation was detected in 13 (40.6%) of the cell lines using methylation specificPCR and confirmed by bisulfite sequencing analysis. Treatment of 11 of the cell lines with the demethylation agent 5-aza-2’-deoxycytidine recovered CD133 expression in most of them. CONCLUSION: Transcriptional repression of CD133 is caused by promoter hypermethylation of the CD133 CpG islands in some of colorectal cancer cell lines. The study may contribute to the understanding of the role of CD133 inactivation in the progression of colorectal cancers.
