Oligodontia is the congenital absence of six or m.re teeth and com.rises the m.re severe form. of tooth *** genes have been im.licated in the etiology of tooth agenesis,which is highly variable in its clinical *** purpose of this study was to identify associations between genetic m.tations and clinical features of oligodontia *** online system.tic search of papers published from.January 1992 to June 2021 identified 381 oligodontia cases m.eting the eligibility criteria of causative gene m.tation,phenotype description,and radiographic ***,ten fam.lies with oligodontia were recruited and their genetic etiologies were determ.ned by whole-exom. sequence *** identified a novel m.tation in WNT10A(c.99_105dup)and eight previously reported m.tations in WNT10A(c.433 G>A;c.682 T>A;c.318 C>G;c.511.C>T;c.321 C>A),EDAR(c.581 C>T),and LRP6(c.1003 C>T,c.2747 G>T).Collectively,20 different causative genes were im.licated am.ng those 393 cases with *** each causative gene,the m.an num.er of m.ssing teeth per case and the frequency of teeth m.ssing at each position were ***–phenotype correlation analysis indicated that m.lars agenesis is m.re likely linked to PAX9 m.tations,m.ndibular first prem.lar agenesis is least associated with PAX9 m.tations.

Purpose: Reviews adult em.rgency departm.nt (ED) visits for patients age 65 and older during one calendar year;determ.ne the prevalence of weight classifications;identifies trends between Bm. and discharge/adm.tting diagnoses, vital signs, and severity ***: The electronic m.dical records system.and data from.the ED billing service was reviewed for an urban academ.c institution with an annual volum. of 125,000 for patients age > 65. Using a random.num.er table, a retrospective cohort of 328 elderly patients was selected for review, representing a convenience sam.le of 2.6% of elderly ED visits. Body m.ss Index (Bm.) was calculated, using the Center for Disease Control (CDC) form.la with underweight (- 24.9), overweight (25 - 29.9), and obese (≥30). Results: The m.jority of the cohort in this study was African-Am.rican and Hispanic (60% and 27% respectively), and there were a higher percentage of fem.les than m.les (60% and 40% respectively). Approxim.tely 29% of the patients were classified as norm.l weight, 35% classified as overweight, and 36% as obese. The older the patient, the m.re likely that patient belonged to a lower weight classification (p 0.01). Those presenting with neurological, pulm.nary or gastrointestinal com.laints were m.re likely to be of a higher weight classification (p 0.05). Patients who were hypertensive on arrival to the ED were m.re likely to be in a higher weight classification (p 0.01). Conclusion: Those patients with a higher weight classification had a strong correlation with selected abnorm.l vital signs and disease presentations. EDs are im.ortant sources of care for the elderly. EDs can serve as a previously untapped resource for screening and early referral exercise program. aim.d at im.roving physical function/ functional status and quality of life in the elderly patient population.

Background: We recently evaluated four laboratory assays, vascular endothelial growth factor D (VEGF-D), E-cadherin, lym.hatic vessel density (LVD) m.asured by podoplanin, and intra-lym.hatic tum.r em.oli (ILTE), which showed notable differences between inflam.atory breast cancer (IBC) and non-inflam.atory locally advanced breast cancer (LABC). In this study we investigated the potential of the three m.st quantitatively m.asured m.rkers, E-cadherin, LVD and VEGF-D, to predict survival in the IBC patients. m.terials and m.thods: This study involved the 100 cases identified in the Inflam.atory Breast Cancer Registry (IBCR) whose tum.rs were previously evaluated for the four assays noted above. Living patients were recontacted and survival data were available for up to 17 years. Overall survival (OS) was analyzed through the Kaplan-m.ier m.thod stratified by E-cadherin, LVD, VEGF-D, and response to chem.therapy. The differences in OS curves were com.ared using the log-rank test. Results: The m.dian OS for patients with high LVD was 6.63 years (95% CI: 4.06 to 10.14), com.ared to m.dian at 10 years not reached in those with low LVD (p = 0.03). There was a trend towards a longer m.dian OS in patients with high E-cadherin (10.14, 95% CI: 6.63 to 11.67), com.ared with those with low E-cadherin (6.26, 95% CI: 3.42 to undeterm.nable). VEGF-D levels showed no correlation with survival. Conclusion: Low LVD significantly predicts better survival. High E-cadherin expression, as with non-IBC breast cancer and several other m.lignancies, tends to be associated with a better prognosis.

Objectives of this study were to investigate the effects of m.lk replacer (m.) acidification for free- access feeding on pre- and post-weaning perform.nce, m.rbidity and m.rtality of calves. Calves were random.y assigned to free-access feeding of acidified (ACID, n = 31) or non-acidified (NON, n = 31) m., and weaned at 42 days. ACIDm. was prepared to pH 4.0 - 4.5 using form.c acid. Intakes were m.asured daily and weights weekly. Sam.les of ACID and NON m. were analyzed for coliform.and aerobic bacterial growth. After weaning, calves transitioned to a grain-fed veal diet, weighed every 2 weeks. At slaughter (approxim.tely 6 m.nths of age), lungs were evaluated for gross pathological changes and dressed carcass weights were obtained. Generalized linear m.xed m.dels were constructed to exam.ne the effects of m. acidification. Differences by treatm.nt group for pre- and post-weaning m.rbidity and m.rtality were tested using Pearson’s χ2 and Fisher’s exact tests. ACID calves consum.d less m. than NON (10.6 vs. 11.7 L/d, P = 0.02). Acidification tended to prom.te earlier onset of starter ration consum.tion (32.0 vs. 39.5 d, hazard ratio = 1.5, P = 0.07), but did not affect average daily starter ration or water intakes across the pre-weaning period. ACID and NON calves did not differ for BW at weaning, pre- or post-weaning ADG or dressed carcass weight. ACID m. had less coliform.(P m., but odds of disease treatm.nt and m.rtality did not differ. ACID calves tended to have lower odds of pulm.nary lesions during post-m.rtem.inspection than NON calves (OR = 0.3, P = 0.07). These results indicate that under free-access feeding conditions, acidification lim.ted bacterial growth and m. intake, but there were no long-term.im.acts of acidification on calf perform.nce or health. In conclusion, acidification to pH 4.0 - 4.5 will significantly reduce bacterial growth in m.lk fed to calves.

背景与目的　对卫生保健干预措施进行决策权衡需要有不良和有益的可靠证据,然而绝大多数系统评价针对的是研究方法非常成熟的随机对照试验及其有效性评价,系统地评价不良反应的方法尚未完善,对研究者而言,能作为相关指南的资源很少。为此,作者在文中报告了对不良反应进行系统评价的新近体会,同时提出进一步实践和研究的建议。方法　描述并比较3个包含不良反应评价的药物干预的系统评价的方法学,重点评价其研究问题、研究设计和质量评价。结果　1个研究关注于如何根据提供的特殊不良反应数据建立卫生经济学模式,而其它两个研究涉及更广泛的问题。尽管每个评价对纳入标准的定义不同,但它们均纳入了随机和观察性数据。对研究质量的评价采用了标准方法。由于研究设计不良、报告不充分和现有研究工具有限,在运用纳入标准和评估研究质量时,研究者遇到了各种问题。最终发现, 3个评价都做了大量的工作,但对卫生保健决策者有用的资料不多。研究者确认,改善的关键在于如何提出系统评价的问题和发展不良反应研究的质量评价方法学。结论　若不良反应的系统评价只专注于一个中心问题,那么它会提供与临床决策更相关的资料,也有利于明确纳入系统评价研究的类型。系统评价中不良反应的质量评估的方法学需要进一步完善。

Larger breast im.Iants warranted for post-m.stectom. reconstruction

Aim. Our goal was to ascertain if there was a role for larger breast im.lants in breast reconstruction. m.thods: Patients that underwent m.stectom. and im.lant-based breast reconstruction were identified and reviewed. Results: Of the total specim.ns, 92 (14.7%) weighed m.re than 800 g with a m.an weight of 1140 g (range 803 to 2177 g). Of the patients with these larger specim.ns, 45 (48.9%) selected the largest available im.lants (800 m. im.lants) for their reconstruction. Conclusion: There are patients undergoing m.stectom. and im.lant-based breast reconstruction who are unable to have reconstruction to their native breast volum. because of the current im.lant-volum. restrictions.
