Outcome of selective CT vs. pan-CT scan in elderly trauma patients: A retrospective cohort study in a level 1 trauma center

There are currently no clear guidelines for use of pan-or selective CT in elderly trauma patients and this subject matter remains *** aim of this study is to compare the outcome of elderly trauma patients in a level 1 trauma centre who required a pan-or selective CT scan on admission.;The Trauma Audit Research Network database was reviewed to identify eligible patients(65 years old and older)over a one-year period,from January 2018 to January ***'demographics,mechanism of injury,injury severity score,length of hospital stay(LOS),mortality and type of CT scans done were recorded.;There were 481 patients with the mean age of 80.8 years were evaluated(48.6%male).Among them 232 cases were multiple injuries while 249 were single system *** 235 patients(48.8%)underwent pan-CT in whom 66.8%were multiple injuries;246(51.1%)did selective CT scan in whom 69.5%were single system *** multiple injury patients,performing a pan-CT scan on presentation was associated with lower LOS compared to those who had a selective CT,in which 76.4%patients spent60 days in pan-CT group compared to 64%in selective CT group(p<0.0001).Performing pan-CT was also associated with lower need to repeat CT(p<0.01).In patients with a single system injury,no differences were found in LOS or the need to repeat CT if either pan-CT or selective CT were requested.;We recommend doing pan-CT scan in all elderly patients with multiple system injuries as it decreases the LOS and the need for another CT during hospital *** difference in LOS or need to repeat another CT if pan-CT or selective CT were requested initially in single system *** age and injury severity score are poor predictors for the need to do pan-CT,the mechanism of injury may be helpful.

Suppression of inflammatory gene expression in T cells by Porphyromonas gingivalis is mediated by targeting MAPK signaling

There is increasing awareness of the effects of Porphyromonas gingivalis on host immune responses. Degradation of cytokines and chemokines by cysteine proteinases has previously been reported. However, the precise mechanisms by which P. gingivalis is able to alter intracellular signaling, and thus proliferation and inflammation, have not been descri bed. We have previously reported s u ppression of activator protei n-1 （AP-1） and degradation of I L-2 by protei nases from P. gingivalis. In the present study, we have analyzed the effects of P. gingivalis on Jurkat T-cell signal transduction and subsequent IL-2 and CXCL8 expression. We found that CXCL8, but not IL-2, gene expression levels were significantly suppressed by viable P. gingivalis. Analysis of intracellular signaling revealed an inhibitory effect of P. gingivalis on c-Jun and c-Fos, but not NFκB（p50and p65）, NFATor STAT5expression. This inhibitory effect was not due to suppression of mitogen-activated protein kinase （MAPK）（p38, erkand JNK） gene expression, but was rather due to prevention of protein kinase C （PKC） and p38 phosphorylation, as demonstrated by western blot analysis. Furthermore, SOCS1 and SOCS3 expression levels decreased following treatment of Jurkat T cells with viable P. gingivalis. The results indicate that P. gingivalis is able to suppress inflammatory gene expression by targeting the activity of MAPK pathways in T cells, which was confirmed by using specific inhibitors of NF-κB, PKC, ERK, p38 and JNK.

As the media is one of the essential tools to cement bonds between peoples, Egypt's State Information Service (SIS) signed cooperation protocols with a number of Chinese media institutions to promote ties between the two countries in this area in 2016. These protocols now include an SIS online portal in Chinese language launched in April this year that will feature information and news to keep the public informed about important issues in Africa and China, as well as the cooperation among them. SIS is Egypt's official media and public relations organ responsible for regulating the affairs of foreign press and media correspondents in Egypt.

Applying reticular synthesis to the design of Cu-based MOFs with mechanically interlocked linkers

Percutaneous Endoscopic Lumbar Discectomy versus Conventional Open Discectomy: A Prospective Randomized Comparative Trial

Introduction: The majority of series considered the conventional open discectomy as the gold standard for the treatment of lumbar disc prolapse. Despite of the popularity of the lumbar endoscopic discectomy nowadays, many neurosurgeons still prefer conventional open discectomy. Purpose: Our study has been designed to compare between percutaneous endoscopic lumbar discectomy and conventional open discectomy;regarding surgical results, complications, clinical and functional outcomes. Patients and Methods: This study is a clinical prospective randomized controlled trial conducted upon 30 patients suffering from prolapsed lumbar disc, from December 2016 to May 2018. Those patients were divided randomly into 2 groups, 15 patients each. One group treated by percutaneous endoscopic interlaminar lumbar discectomy (PELD) and the other group treated by conventional open discectomy (COD). Operative time, wound size, Intraoperative blood loss, Intraoperative complications, postoperative hospital stay, Postoperative complication and the results of visual analogue score (VAS) and modified MacNab’s criteria were assessed. Results: This study included 30 patients (18 males and 12 females). The mean age was about 35 years. Although, there was postoperative improvement of the VAS and MacNab’s criteria in the two groups, there was no statistically significant difference between the preoperative and postoperative VAS of low back pain and radicular pain for the two groups in the follow up period. Conclusion: Both techniques give good results for patients;each technique has some advantages over the other.

Fixture Design to Supplement Machining and Fuel Cell Education

A vital part of enhancing the ability of students to learn about advanced fabrication techniques is identifying the barriers to the student’s entry and excelling in the manufacturing field. In the Manufacturing and Mechanical Technology fields, there exists an intimidating experience gap or barrier between professionals and novice students. The students’ participation in the design and fabrication of a fixture for manufacturing a mechanical part will aid in eliminating this barrier by making fuel cell’s bipolar plates production accessible to inexperienced technology students. The process of manufacturing a fixture for the production of hydrogen fuel cell and hydrogen purifier plates required a careful planning and step-by-step methodological implementation. Through the use of our designed fixture, it is now possible to teach students how to use a CNC milling machine with relative ease while also allowing for precision part machining. The setup of the fixture allows students to observe the necessary measures to properly machine a part while also teaching them the benefits of fixtures in the manufacturing industry. In summary, the purpose of this paper is to provide the students with applied learning experience through involving them in the design and fabrication of a multi-disciplinary mechanical fixture and the utilization of practice oriented teaching resources in a full scale learning environment.

Dear Editor,We are describing a rare case presenting with page phenomenon,9 years post radical cystectomy for muscle invasive bladder *** kidney was developed secondary to a late recurrent urothelial tumour in the left renal *** case is a fifty-seven-year-old gentleman presented with poorly controlled hypertension and left dull aching loin *** blood pressure(BP)was 180/80 mmHg at diagnosis and was down to 150/80 mmHg using angiotensin converting enzyme(ACE)inhibitor and beta blocker *** underwent a radical cystectomy(RC)and ileal conduit urinary diversion 9 years *** of his cystectomy specimen was localized urothelial carcinoma with negative resection margin and negative lymph nodes(T2N0M0).Examination showed a palpable mobile mass in the left hypochondrium.

Generalized Order Statistics from Generalized Exponential Distributions in Explicit Forms

The generalized order statistics which introduced by [1] are studied in the present paper. The Gompertz distribution is widely used to describe the distribution of adult deaths, and some related models used in the economic applications [2]. Previous works concentrated on formulating approximate relationships to characterize it [3-5]. The main aim of this paper is to obtain the distribution of single, two, and all generalized order statistics from Gompertz distribution with some special cases. In addition the conditional distribution of two generalized order statistics from the same distribution is obtained. The Gompertz distribution has a continuous probability density function with location parameter a and shape parameter b, , where x restricted by the interval . The nth moment generated function of the Gompertz distributed random variable X is given on the form: where, is the generalized integro-exponential function [6]. In this paper we shall obtain joint distribution, distribution of product of two generalized order statistics from the Gompertz distribution, and then derive some useful formulas of these distributions as special cases.
