Outcome of Patients with Placenta Accreta at El Shatby Maternity University Hospital

Objective: The aim of this study was to determine the incidence, risk factors, and outcomes of management of patients with placenta accreta. Background Placenta accreta occurs when the placental implantation is abnormal. The marked increase in incidence has been attributed to the increasing prevalence of cesarean delivery in recent years. The most common theory is defective decidualization. The most important risk factor for placenta accreta is placenta previa after a prior cesarean delivery. The first clinical manifestation of placenta accreta is usually profuse, life-threatening hemorrhage. The recommended management of suspected placenta accreta is planned preterm cesarean hysterectomy with the placenta left in situ. Patients and methods: It’s a study of all cases of placenta accreta at El-Shatby Maternity University Hospital starting from 1/4/2016 till 1/10/2016. Selection of the cases will only be dependent upon their pregnancy gestational age above 28 weeks of gestation. Results: The incidence of placenta accreta was 1/75 cesarean deliveries. The ultrasonography and doppler had a false negative rate of 54.6% and a sensitivity of 45.2% in diagnosis of placenta accreta. The rate of blood transfusion was 79.6%. Uterine preserving procedures performed in 66%. Cesarean hysterectomy performed in 34%. Intensive care unit admission occurred in 27.3%. The mean gestational age at delivery was 33.8 ± 4.6 weeks’ gestation. 31.8% admitted to the neonatal intensive care unit. Conclusion: The incidence of placenta accreta increased due to the increasing rate of cesarean deliveries, prenatal diagnosis of placenta accreta is paramount, as most women are asymptomatic. Prenatal diagnosis allows time for a multidisciplinary team to make delivery plans, which will help decrease surgical complications.

Evaluation of Age Related Changes in Cervical Spine in Saudi Arabian Adult Population: Using CT Scan Images

Purpose: Cervical spine is considered to be the most moving part of vertebral column. It is located in a very important part of the body i.e. neck having some very important structures related to it. Any age related changes occurring in such a part of vertebral column may lead to many disorders and disabilities. Complete familiarity of this entire region anatomically as well as radiologically is therefore of utmost importance for the diagnosis and management of such disorders. Aim of this work is to evaluate the frequency of age related degeneration in cervical spine in Saudi adult asymptomatic subjects using CT scan images. Methods: In this study, 105 cases of symptomless adults ranging between 18 - 90 years of age were included. The cases were classified into 3 groups;adult group (18 - 35 years old), middle age group (36 - 55 years old) and old age group (56 - 90 years old). Their CT scans were performed in the department of radiology, King Khalid Hospital, Al Kharj and studied for any age related changes. Results: The first age group category included 35 cases;20 had normal and 15 degenerative changes. The second category included 35 cases;3 had normal and 32 degenerative changes. The third category included 35 cases. Single had normal and 34 degenerative changes. Conclusion: Asymptomatic degenerative changes are common in the cervical spine after 30 years of age in Saudi adult asymptomatic subjects.

Estimation of Standard Splenic Volume in Saudi Arabian Adult Population: Using 3D Reconstruction of Abdominal CT Scan Images

Purpose: Spleen is enlarged in a variety of clinical conditions including infectious, infiltrative, immunologic and malignant states. Evaluation of splenic size is important in every physical examination of the abdomen by a physician. Determination of its size by palpation can be extremely inaccurate because spleen is never palpable till it is enlarged 2 to 3 times its own size. The accurate diagnosis of splenic enlargement is a matter of considerable importance as it is a useful guide for arriving at a diagnosis of the disease. It is therefore of utmost importance to resort to a mechanism that will give us an accurate estimation of the size of spleen. Aim of this work was to determine the normal range of spleen dimensions in average adult Saudi Arabian population and compare it with the published data. Methods: CT scans of 34 adult patients (male and female) aged between 20 - 70 years, having no splenic disorders, were collected from department of radio-diagnosis King Khalid Hospital Al-Kharj, KSA. Splenic volume was measured by two methods—by volumetric software and the prolate ellipsoid formula. Results: The average splenic volume of all subjects was 161.42 ± 54.91 cm3 with a range of 106 - 319 cm3. The average splenic volume of males was 196.95 ± 48.70 cm3 and that of female was 196.95 ± 26.97 cm3. Conclusions: These results provide normative data for evaluating patients with splenic enlargement.

Humans coexist with extreme events all the time, only when the intensity of the event becomes greater than a certain level there is a resulting disaster. Small earthquakes occur all of the time with no adverse effects. Only large earthquakes cause disasters. Statistical analysis reveals that larger events occur less frequently than small events. In a year, we would have many values for the events;the annual maximum was the greatest of those values. Within an annual series, only the largest value per year is allowed, even if an additional significant peak occurs. As the magnitude of a hazardous increases, the frequency of occurrence (how often a given magnitude is equaled or exceeded) decreases. Thus, major disasters result from a small number of large events that rarely occur. A plot of recurrence intervals versus associated magnitudes produces a group of points that also approximates a straight line on semi-logarithmic paper. Therefore, past records of earthquakes at the Gulf of Aqaba, Northern Red Sea for months from May, 1999 to Feb, 2016 are used to predict future conditions concerning the annual frequency, the return period, the percentage probability for each event, and the probability of a certain-magnitude earthquake occurring in the region during any period.

Critical care of obese patients during and after spine surgery

Obesity is one of the most prevalent health problems facing the United States today, with a recent JAMA articlepublished in 2014 estimating the prevalence of one third of all adults in the United States being obese. Also, due to technological advancements, the incidence of spine surgeries is growing. Considering these overall increases in both obesity and the performance of spinal surgeries, it can be inferred that more spinal surgery candidates will be obese. Due to this, certain factors must be taken into consideration when dealing with spine surgeries in the obese. Obesity is closely correlated with additional medical comorbidities, including hypertension, coronary artery disease, congestive heart failure, and diabetes mellitus. The pre-operative evaluation may be more difficult, as a more extensive medical evaluation may be needed. Also, adequate radiographic images can be difficult to obtain due to patient size and equipment limitations. Administering anesthesia becomes more difficult, as does proper patient positioning. Postoperatively, the obese patient is at greater risk for reintubation, difficulty with pain control, wound infection and deep vein thrombosis. However, despite these concerns, appropriate clinical outcomes can still be achieved in the obese spine surgical candidate. Obesity, therefore, is not a contraindication to spine surgery, and appropriate patient selection remains the key to obtaining favorable clinical outcomes.

Impact of Cystic versus Cylindrical Types of Bronchiectasis on Pulmonary and Cardiac Functions

Background: The combination of the clinical features, HRCT score and echocardiographic evidence of pulmonary hypertension help assess the prognosis in bronchiectasis. Aim: To test whether pulmonary and cardiac functions associated with bronchiectasis would differ according to its type (cystic versus cylindrical) utilizing HRCT score, PFTs and echocardiography. Patients and Methods: A cross-sectional study of patients with bronchiectasis was conducted at Chest and Internal Medicine Departments at Assiut University Hospital, Egypt. The diagnosis of bronchiectasis type was based on HRCT findings. PFTs, HRCT score and echocardiography were assessed in all cases. Results: We studied 56 patients with bronchiectasis;31 were cystic (group A) and 25 were cylindrical (group B). Forced vital capacity (FVC%) and Dlco% were significantly lower in group A (P as compared with group B;whereas FEF 25%-75% and FEV1 (0.04) were lower in group B. Global HRCT score, RVD and SPAP were significantly higher in group A (P = 0.002) and correlated with FEV1% (r = ?0.51), and with SPAP (r = 0.16). Conclusions: Airway obstruction and small airway dysfunction were more significantly seen in cylindrical bronchiectasis. Patients with cystic bronchiectasis had significantly, higher global HRCT scores, RVD and SPAP. HRCT scores correlate with FEV 1% and SPAP and could be a predictor of future PH. Routine echocardiographic assessment of patients with bronchiectasis, particularly in those with cystic disease is highly recommended.

Prevalence and Risk of Anemia in Type-2 Diabetic Patients

Aim: To determine the prevalence and risk of anemia in type-2 diabetes mellitus. In addition, the incidence and risk of anemia in these patients were assessed according to gender and glycemic control status. Methods: The study group comprised 200 patients with type-2 diabetes. Patients were divided into groups according to glycemic control and gender. Glycated hemoglobin (HbA1c), blood glucose level, hemoglobin concentration, mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH) and mean corpuscular hemoglobin concentration (MCHC) of subjects were evaluated. The presence of anemia was defined by a hemoglobin level 6.5%, fasting blood glucose >126 mg/dl and random blood glucose >200 mg/dl. Result: 63% of diabetic patients had anemia. There was higher incidence and risk of anemia in females (36%) as compared to males (27%) (p 7.5% (49.5%) compared to those with controlled diabetes HbA1C < 7.5% (13.5%) (p < 0.05). Conclusion: This study indicated that poor glycemic control and gender difference are associated with the incidence of anemia in type II diabetes. Our findings suggest the need of screening for anemia in diabetes out-patient clinics. Routine hematological tests along with blood glucose level should be mandatory in order to make therapeutic decisions for the treatment of anemia in type II diabetes mellitus.

Comparison of Wheat Planting Methods and Residue Incorporation Under Saline-Sodic Soil

The present research was conducted to monitor the wheat productivity along with residue incorporation under saline-sodic soils by examining different planting methods at Zaidi Farm, Kakar Gill, Sheikhupura District, Punjab Province in 2007-2008. Randomized Complete Block Design （RCBD） with three replications was used having treatments： control （broadcast）, zero till wheat plantation, wheat plantation using happy seeder and wheat plantation on raised beds. It was observed that tillering was pretty higher （141 and 139 m2） under raised bed as well as happy seeder plantation as compared to zero tilled wheat and broadcast technique. A significant relation was detected among maximum straw and grain yield （4,898 and 1,752 kg-ha1） in raised bed followed by happy seeder planting method. The lowest grain yield was recorded in the broadcast method. Maximum net revenue earned by raised bed planting method （39,908 PKR） followed by happy seeder methodology （37,533 PKR）. The overall study suggests that raised bed and happy seeder wheat plantation are the superior planting methods.

Radon Dosimetry in Erbil City Hospitals

The aim of the study was to compare radon concentrations and effective dose equivalent in state hospitals inside the Erbil city having different periods of building. Effective doses were estimated in different floors of the five selected hospitals by using the （CR-39） solid state nuclear track detectors with the guide of 1CRP Publication 66. The hanged dosimeters were left for about 60 days and after etching the radon concentration, annual effective dose and effective dose equivalents were calculated. Annual effective doses were estimated as 〈 1 mSv, so the results reveal the present concentrations to be below the standard values in the studied hospitals. Therefore, no healthy risks concerning the radon activities are existing in the hospital staff and patients in these hospitals.

Role of Ultrasound in Evaluation of Developmental Dysplasia of the Hip in Infants

Background/Aim: The present study was directed to investigate the accuracy of ultrasound (USG) in early detection of developmental dysplasia of the hip (DDH) to prevent permanent complications. Material & Methods: A total of 276 infants was investigated;their ages range from 1 - 6 months. Patients referred from the clinic based on clinical assessment (mainly Barlo & Ortolani maneuvers). Then the patient’s hips were examined by expert radiologists in USG scan. After that, dynamic (coronal and transverse) & static ultrasound views evaluated by the same radiologist using Graf’s criteria. Infants with abnormal ultrasound findings suggestive of DDH were followed up after 4 - 6 weeks by the same investigator to monitor the progress. At the same time, the patient referred to a specialist for conservative management from the first positive findings. Results: Among the 276 referred patients, infants with normal stable hips (Graf Type I and Type II a+) were 221 (80%), 155 of them were female and 66 male infants. Infants with unstable hips based on Graf’s criteria (Type II a-, II b, II c and Type III a) were 55 (20%). Out of 55 with abnormal findings, the affected hips on both sides, right and left were: 5 cases (1.8%), 7 (2.5%) and 43 hips (15.5%) consecutively. Out of 55 infants affected;3 referred to tertiary centers for further surgical management while 52 managed conservatively. Conclusion: Delayed treatment rates of DDH and complications could be reduced by the usage of the USG after the first month of life. In addition to physical assessment, USG for high-risk infants will provide more accurate diagnosis and minimize complications.
