Background Long-term outcomes for Medicare beneficiaries hospitalised with transient ischaemic attack(TIA)and role of ABCD^(2) score in identifying high-risk individuals are not *** We identified 40825 Medicare beneficiaries hospitalised from 2011 to 2014 for a TIA to a Get With The Guidelines(GWTG)-Stroke hospital and classified them using ABCD^(2)s of mortality and rehospitalisation(all-cause,ischaemic stroke,haemorrhagic stroke,myocardial infarction,and gastrointestinal and intracranial haemorrhage)for high-risk versus low-risk groups adjusted for patient and hospital *** Of the 40825 patients,35118(86%)were high risk(ABCD^(2)≥4)and 5707(14%)were low risk(ABCD^(2)=0-3).Overall rate of mortality during 1-year follow-up after hospital discharge for the index TIA was 11.7%,44.3% were rehospitalised for any reason and 3.6%were readmitted due to *** with ABCD^(2) score≥4 had higher mortality at 1 year than not(adjusted HR 1.18,95%CI 1.07 to 1.30).Adjusted risks for ischaemic stroke,all-cause readmission and mortality/all-cause readmission at 1 year were also significantly higher for patients with ABCD^(2) score≥4 vs *** contrast,haemorrhagic stroke,myocardial infarction,gastrointestinal bleeding and intracranial haemorrhage risk were not significantly different by ABCD^(2) *** This study validates the use of ABCD^(2) score for long-term risk assessment after TIA in patients aged 65 years and *** efforts for community-based follow-up care after TIA are needed for ongoing prevention in Medicare beneficiaries who were hospitalised for TIA.

建筑物震后损伤评估是地震灾害损失评估、震后恢复和重建工作的重要内容之一。建筑物震后损伤评估要现场找出结构损伤或破坏位置并对其进行分类统计和分析,它不仅提供了有关任何结构中损坏构件的数量和类别的重要信息,还帮助我们发现导致损坏的可能的结构缺陷。传统的损伤评估技术包括由工程师或专家对每个受损结构进行目视检查,这种检查方式不仅危险、耗时,而且需要大量熟练人力。基于计算机视觉的结构损伤识别技术已经成为结构监测和检测领域中一个极为先进和创新的研究方向,并逐渐受到了土木工程研究人员的关注。计算机视觉技术的主要优点是非接触、远距离、高精度、更大的覆盖范围以及可通过移动设备和无人机进行识别。本论文主要研究更易受地震影响的砌体结构的损伤评估。首先,本论文对卷积神经网络（CNN）理论进行了回顾和讨论,分析了迁移学习,超参数调试,卷积可视化等算法。同时,对砌体结构墙体在地震中可能发生的多类损伤（如对角裂缝、拐角分离、分层和平面外破坏等）进行了分类。其次,本文提出了一种通过在Image Net数据集上预先训练的VGG16模型上应用微调和转移学习而构建的深度CNN模型架构。梯度加权类激活映射（GradCAM）技术用于解决损坏的定位问题。最后,将本论文的方法应用于砌体结构墙体的损伤评估,对具有不同网络参数的多个模型在训练、测试和验证集结果进行了对比分析,并给出了基于GradCAM注意力机制的损伤定位图。结果表明,所提出模型的整体性能在表征和定位损伤方面表现出良好的一致性和鲁棒性。论文的结果证明了该模型在实际应用中的可行性。

Zinc supplementation as an adjunct to standard therapy in childhood nephrotic syndrome-a systematic review

AIM To evaluate the role of zinc as add on treatment to the "recommended treatment" of nephrotic syndrome(NS)in *** All the published literature through the major databases including IVledline/Pubmed,Embase,and Google Scholar were searched till 31 st December *** lists from the articles were reviewed to identify additional pertinent *** papers concerning the role of zinc in childhood NS were reviewed by the authors,and the data were extracted using a standardized data collection *** trials(RCTs) comparing zinc vs placebo was *** of zinc was studied in both steroid sensitive and steroid dependent/frequent relapsing *** primary outcome measure was the risk of relapse in 12 *** secondary outcome measures were mean relapse rate per patient in 12 mo,mean relapse rate per patient in 6 mo,risk of infection associated relapse in 12 mo,cumulative dose of steroids in two groups,mean length of time to next relapse,adverse effects of therapy,and change in serum zinc *** Of 54 citations retrieved,a total of 6 RCTs were *** was used at a dose of 10-20 mg/d,for the duration that varied from 6-12 *** to placebo,zinc reduced the frequency of relapses,induced sustained remission/no relapse,reduced the proportion of infection episodes associated with relapse with a mild adverse event in the form of metallic *** GRADE evidence generated was of "very low-quality".CONCLUSION Zinc may be a useful additive in the treatment of childhood *** evidence generated mostly was of "very low-quality".We need more good quality RCTs in different country setting as well different subgroups of children before any firm recommendation can be made.

森林是能源、木材和非木材林产品、药用和芳香植物、水文功能、生物多样性保护的主要载体,也是国家税收的基本来源之一。研究表明,森林可以极大促进经济增长,并在集约化管理下为当地社区创造就业机会。本文旨在预测森林部门对国民经济的贡献。本文研究的林业经济主要是指木材(timber)、非木材林产品(NTFP)、药用和芳香植物(MAP)以及保护区产生的收益。ARIMA模型可用于预测林业部门的经济贡献。研究发现,在过去15年中,木材/薪材销售的总收入(5019万美元)高于保护区(3758万美元)和非木材林产品的收入(690万美元)。该模型预计,未来十年木材和薪材销售的平均收入将为350万美元/年。同样,在未来十年中,NTFPs/MAPs和保护区的平均收入将分别为约50万美元/年和620万美元/年。因此,实践可持续的森林管理,建立林业苗圃,实施农林业实践以及明晰林业权属,建立以森林为基础的产业,可以改善林业部门的经济状况。

Study of Bacterial Diversity of Mangroves Rhizosphere

Microbial diversity has been an important facet of scientific research, since microbes promise a plethora of biomolecules which are otherwise not found in nature. Microbes are subjected to high level of competition for survival in the environment, and hence develop mechanisms of defense. The biomolecules produced by these microbes as part of their defense or survival mechanism, are of importance for human and animal drugs and many other industrial and environmental applications. The marine counterparts of these terrestrial microbes have yet higher potential, since the marine environment has higher biotic and abiotic stresses, leading to new molecule discovery. In the current study, a bacterial diversity study of the culturable bacteria of the mangrove rhizosphere of Avicennia marina has been undertaken, to understand the flora diversity. Mangroves are unique ecosystems which are under a combination of marine and terrestrial influence. Mangroves are seaward, inland and also found in creek areas. This diversity in their habitat, leads them to produce variable root exudates, which support the growth of different types of organisms. This study has revealed that certain species are dominant in these ecosystems irrespective of the biotic and abiotic stresses, whereas certain species appear only at neutral pH. The study will help select organisms for further biomolecule discovery programs, based on their environment of isolation and other growth parameters.

Isolation of Dental Caries Bacteria from Dental Plaque and Effect of Tooth Pastes on Acidogenic Bacteria

Destruction of calcified tissue was caused by acids which are by product of carbohydrate metabolism of acidogenic bacteria consequent to dental caries. The purpose of this study was to assess the existence of acidogens potentially causing the dental caries and comparatively evaluation of efficacy of different toothpastes. The dental plaques of fifty persons belong to three age groups (1 - 20, 21 - 40, 41 - 60 year and above) were examined to identify microorganisms by the culture method. Thirty nine bacteria were isolated by spread plate method on BSMY I minimal media. Thirteen out of thirty nine, acidogens colonized in the dental plaques. Seven potentially acidogens CD17, CD26, CD27, CD28, CD29, CD34 and CD35 were treated with five different toothpastes. Inhibition effect of Triclosan and Fluoride containing tooth pastes were found more efficient. The results of the present study revealed that bacteria that commonly cause dental caries colonized in dental plaques of children and alcoholic person. Therefore, dental plaques must be considered a specific reservoir of colonization and subsequent dental caries. To reduce the dental problem triclosan and fluoride containing product should be recommended.

Managing Knowledge in Shared Spaces

Information has always been an integral component of business strategy. However, with the rise of information technology (IT) and globalization of businesses, information has taken a far more important role in explaining business performance. IT can be used to enhance research activities of the firm;and its product knowledge can be used to improve business productivity. In fact, scholars and researchers have indicated that knowledge can be a source of sustainable advantages for the firm. However, not much research has been done that explains what kind of knowledge is important for sustainable advantages. In this paper, we present three kinds of knowledge: core, complementary, and peripheral. We also develop an idea of shared space along three dimensions: physically shared space, technically shared space, and cognitively shared space. The main theme of this paper is to show the relevance and importance of these different kinds of knowledge along the shared spaces that create sustainable advantages for the firm.

Determination of cilostazol and its active metabolite 3,4-dehydro cilostazol from small plasma volume by UPLC-MS/MS

A simple,rapid and sensitive ultra performance liquid chromatography-tandem mass spectrometry（UPLC-MS/MS） method has been developed for the simultaneous determination of cilostazol and its pharmacologically active metabolite 3,4-dehydro cilostazol in human plasma using deuterated analogs as internal standards（ISs）.Plasma samples were prepared using solid phase extraction and chromatographic separation was performed on UPLC BEH C18（50 mm × 2.1 mm.1.7 μm） *** method was established over a concentration range of 0.5-1000 ng/mL for cilostazol and 0.5-500 ng/mL for 3.4-dehydro ***- and inter-batch precision（%CV） and accuracy for the analytes were found within 0.93-1.88 and 98.8-101.7% for cilostazol and 0.91-2.79 and 98.0-102.7% for the metabolite *** assay recovery was within 95-97% for both the analytes and internal *** method was successfully applied to support a bioequivalence study of 100 mg cilostazol in30 healthy subjects.
