Fast Track Surgery and Its Outcome in Colorectal Surgery in a Tertiary Care Hospital

Background: Fast track surgery is an evidence-based multidisciplinary approach. The underlying principle is to enable patients to recover from surgery and leave the hospital sooner by minimizing the stress responses on the body during surgery. Our aim was to compare the outcome of fast-track protocol and conventional methods in colorectal surgery. Method: It was an observational cross-sectional study carried out at the Department of Surgery in different tertiary level hospitals, Dhaka Bangladesh during the period January 2014 to December 2017. Among this population, 50 patients were placed in the fast-track program (Group A) from January 2014 to December 2015 and 50 patients were in the conventional method (Group B), from January 2016 to December 2017. The fast-track patients were selected after receiving ethical approval from the Bangladesh College of Physician & Surgeons. Data analysis was done using the statistical package for social science (SPSS) for windows version 20. Results: The average age of the patients was 45.24 ± 16.65 years (range: 11 - 70 years) in the fast track group (Group A) and 43.24 ± 17.76 years in the conventional method (Group B). The majority were female between two groups, with 56% in group A and 52% in group B. General and surgical complications occurred in 9 (18%) patients and 11 (22%) patients respectively in group A. On the other hand in group B general and surgical complications occurred in 10 (20%) patients and 13 (26%) patients respectively. The average hospital stay was 9.24 ± 5.99 days in group A and the average hospital stay was 10.10 ± 6.04 days in group B. Conclusion: Making the decision to adopt fast-track surgery will challenge current traditional practice for all members of the multidisciplinary team across the whole local health community.

行星齿轮箱在直升机汽车飞机发动机重型机械和海洋车辆中应用十分广泛行星齿轮箱经常在恶略的工况工作,通常会致其故障的发生当行星齿轮箱中的太阳轮行星轮或齿圈发生故障时,故障源产生的振动传递到壳体的传感器会到传递路径的影响,使得采集到的故障信号十分复,这加大了故障诊断的难度动力学分析是降噪声和减振的必要条件,因本文对行星齿轮箱进行了详细的动力学分析本文研究了行星齿轮箱在发生齿轮轴故障时的信号规律,针对NGW11-8型号行星减机的太阳轮行星轮行星轮轴外圈内圈部件分别进行了程度的破坏,进行故障模拟,利用传感器及信号采集仪对度的振动信号进行采集并分析,探究其规律对比行星齿轮箱在相转常故障信号的时频谱图,提特征信息,验证已有结论对比普通振动信号高频信号以及应力波信号在转故障置的异,探究哪种方式更加适合故障诊断和特征提研究传递路径对于采集到的信号的影响,最优的采集方式和测点布置对采集到的齿轮的振动信号用经验模态分解EMD的方法提齿轮箱齿轮常磨损和断齿的故障特征信息对提的齿轮种状态信号的时域和频域内的特征参数,分别并计算了运行度的啮合频率和特征频率最,提出了一种检测单个颗行星齿轮单齿裂纹的信号分解方法,并进行了实验验证本文提出的方法有助于们了解行星齿轮箱的振动特性,并为开发新的齿轮故障检测信号处理系统提供了新的思路。

Investigation of Subsurface Contamination due to Chromium from Tannery Effluent in Kasur District of Pakistan

The present study is conducted in the scenario of the tannery waste hazards in Kasur district of Pakistan where the tannery industry is considered as major cause of groundwater quality deterioration. Area focused in this research constitutes the surroundings of the effluent carrying drains near tannery units. This study includes soil explorations, groundwater monitoring and wastewater analysis in the research area so as to find out the contamination extent of chromium in subsurface. Initial groundwater monitoring exhibited chromium concentrations as high as 90 mg/L in the tannery area. Even groundwater sampling from monitoring wells installed in the adjacent areas of effluent carrying drains, showed concentrations up to 10.4 mg/L. Wastewater analysis of all the drains in the research area has evidenced potential risk of contaminant seepage into soil and groundwater as level of chromium in wastewater samples has reported to be immensely high and varies from 68 mg/L to 2,152 mg/L. However the 30 soil samples collected from two soil bores did not show any significant results as the maximum values obtained for hexavalent chromium for leaching and retained in soil are 0.02 mg/L and 8.1 mg/kg, respectively. These low concentrations of soil samples suggest that the soil contamination may not be a main environmental issue in the areas adjacent to the effluent carrying drains. The research concludes as possibility of direct interference of the tannery wastewater with groundwater through damaged structures and sewers.

Acid rain has become a major concern due to increasing atmospheric pollution,particularly in *** investigated whether acid rain inhibits the germination of seeds and subsequent emergence of seedling of four tree species from southern China:Cunninghamia lanceolata,Fokienia hodginisi,Pinus massoniana and Phoebe zhennan by simulating acid rain with pH of 2.5,3.5,4.5 and *** hypothesized that the inhibitory effect of acid rain on germination of seeds and emergence of seedling varies between species and the degree of acidity.A solution of 1 N H2SO4 and 1 N HNO 3 in the ratio of 10–1 was prepared and diluted to four pH levels,and seeds were supplied with solutions of these pH values and distilled water as control and tested for germination in a controlled growth *** results revealed that simulated acid rain of pH 2.5 adversely affected the germination capacity of *** and ***;while all acid solutions significantly increased germination of ***;but had no effect on germination of *** *** acid solution(pH of 2.5)adversely affected elongation of radicle and hypocotyl as well as fresh and dry weights of radicle and hypocotyl of tender *** result demonstrated that seedling emergence is more sensitive than seed germination to simulated acid rain,and germination of conifer species are less sensitive than broad leaved species to simulated acid *** a whole,acid rain of pH of 3.5 is the threshold level and acid rain below this value will have a detrimental effect on seed germination and seedling emergence.

BACKGROUND Surgical site infections are a major cause of morbidity and mortality following orthopedic *** efforts to identify sources of contamination in the operating rooms have implicated mobile *** To investigate microbial colonization on the mobile phones of health care professionals in the orthopedic operating *** We conducted a cross-sectional study involving culture and sensitivity analysis of swabs taken from the mobile phones of orthopedic and anesthesia attendings,residents,technicians and nurses working in the orthopedic operating rooms over a period of two *** and cell phone related factors were recorded using a questionnaire and the factors associated with contamination were *** Ninety-three of 100 mobile phones were *** isolated were Coagulase-negative Staphylococcus(62%),Micrococcus(41%)and Bacillus(26%).The risk of contamination was increased with mobile covers and cracked screens and decreased by cell phone *** Mobile phones belonging to health care workers are frequently contaminated with pathogenic bacteria with the potential of transferring drug resistance to nosocomial *** investigating the relationship to surgical site infections need to be *** concept of"mobile hygiene"involving the change of mobile covers,replacement of cracked screens or even wiping the phone with an alcohol swab could yield the cost-effective balance that contaminated cell phones deserve until they are established as a direct cause of surgical site infections.

针对自然环境中有效磷养分主要分布于土壤表层而容易导致植物根系激烈竞争的问题,选择同一杉木(Cunninghamia lanceolata)无性系幼苗为研究对象,采用水平方向空间狭小而垂直方向空间大的室内盆栽模拟装置,以单株种植为对照,构建双株种植的竞争处理,通过设置3个供磷水平:不供磷处理(0 mg/kg KH_2PO_4)、低磷处理(6 mg/kg KH_2PO_4)和正常供磷处理(12 mg/kg KH_2PO_4),采用破坏性试验方式收获,分别在试验的前期(50 d)、中期(100 d)和后期(150 d)测定不同处理条件下杉木幼苗根系生物量与根系形态的变化,研究邻株杉木根系在垂直方向上对有限磷素资源的竞争策略。结果表明:竞争处理和供磷水平对杉木幼苗根系长度、平均直径等形态指标的影响存在交互作用(P0.05)。竞争处理中杉木根系形态增量均明显高于非竞争处理的单株幼苗,且随着胁迫时间的增加,根系形态增量均呈现显著的上升趋势,其中在胁迫中期和后期的增量明显高于前期,且邻株竞争处理明显提高了杉木的比根长,提升了根系觅磷的能力;随着供磷水平的提高,根表面积和根体积增量大体上呈现先上升后下降的趋势。与非竞争处理相比,竞争条件下杉木地上部生物积累量差异不明显,而根系生物量、根冠比均低于非竞争处理的单株幼苗。

Lumbar Hernia: An Unusual Presentation of Bear Maul

Introduction: Typical lumbar hernias are very rare surgical conditions. Lumbar hernias can be congenital or acquired. About 25% of all lumbar hernias have a traumatic etiology. Case-Report: We here reported a case of a 55-year-old male who was mauled by a bear. The patient developed an atypical lumbar hernia after 6 months of sustaining craniofacial and abdominal trauma. Open hernioplasty, which was a very challenging job, was done in this patient. Conclusion: Post traumatic lumbar hernias have been reported but till today no case of lumbar hernia following an attack by a bear has been reported in literature. Both acute and long-standing post-traumatic lumbar hernias are rare but challenging conditions. The corrective surgical procedure becomes more complex as hernial defect enlarges. Reconstruction is a challenging aspect of lumbar hernia surgery.

Flat breads are very popular especially in those parts of the world where bread constitutes a major source of dietary protein and calories. There are several forms of flat bread, and the variation is mainly in terms of ingredient, technology, and quality. Several modifications in the formulations have been made in the recent past in order to improve the quality and delicacy of these food products. With urbanization and industrialization, the demand for ready to eat and easy to carry products resembling flat bread in appearance, but having desirable qualities of bread offers one exciting possibility to this effect. In India, wheat is one of the daily staples, consumed in the form of different flat breads such as Chapati, Parotha, Phulka, Puri and Tandoori Roti. Different wheat varieties have been used for the production of flat breads. In recent years, many researchers have tried to improve ingredient level, baking properties, organoleptic characteristics, nutritional value and extension of the shelf life of flat bread. They are usually produced from a simple recipe consisting of flour, salt and water in varying proportions, however, sometimes the manufactures also use optional ingredients like yeast fat, skim milk powder and certain additives like emulsifiers, hydrocolloids, enzymes and preservatives for quality improvement and shelf life enhancement.
