A 53-year old previously healthy male underwent a screening colonoscopy for detection of a potential colorectal neoplasm. The terminal ileum was intubated and a mass was noted. examination of the colon was normal. The biopsy of the ileal mass was consistent with an adenocarcinoma arising from the terminal ileum. His father who had never been previously ill from gastrointestinal disease died of natural causes, but was found to have Crohn's disease postmortem. The patient underwent exploratory laparotomy and a right hemicolectomy with a 30 cm section of terminal ileum in continuity. Findings were consistent with ileal adenocarcinoma in the setting of Crohn's disease. The patient made an uneventful recovery. The pathology was stage 1 adenocarcinoma. This is a unique case in that on a screening colonoscopy, a favorable ileal adenocarcinoma was discovered in the setting of asymptomatic, undiagnosed ileal Crohn's disease in a patient whose father had Crohn's disease diagnosed postmortem.

Pancreatic cancer is the fourth most common cause of cancer-related deaths in the United States, with over 38000 deaths in 2013. The opportunity to detect pancreatic cancer while it is still curable is dependent on our ability to identify and screen high-risk populations before their symptoms arise. Risk factors for developing pancreatic cancer include multiple genetic syndromes as well as modifiable risk factors. Genetic conditions include hereditary breast and ovarian cancer syndrome, Lynch Syndrome, familial adenomatous polyposis, Peutz-Jeghers Syndrome, familial atypical multiple mole melanoma syndrome, hereditary pancreatitis, cystic fibrosis, and ataxia-telangiectasia; having a genetic predisposition can raise the risk of developing pancreatic cancer up to 132-fold over the general population. Modifiable risk factors, which include tobacco exposure, alcohol use, chronic pancreatitis, diet, obesity, diabetes mellitus, as well as certain abdominal surgeries and infections, have also been shown to increase the risk of pancreatic cancer development. Several largevolume centers have initiated such screening protocols, and consensus-based guidelines for screening high-riskgroups have recently been published. The focus of this review will be both the genetic and modifiable risk factors implicated in pancreatic cancer, as well as a review of screening strategies and their diagnostic yields.

Recently, the quasi-two-dimensional electron gas （q2DeG） confined at the interface between LaAlO3 and SrTiO3 has attracted significant attention. In this paper, we briefly review experimental methods that have been used to tune the carrier density and mobility of this q2DeG. These methods can be classified into two categories： growth-related tuning （i.e. substrate, growth temperature, oxygen pressure, post-annealing, LaAlO3 thickness, stoichiometry, and capping layers） and post-growth tuning （i.e. electrostatic field gating, conductive atomic force microscopy and surface adsorbates）. Taken together, these methods enable the broad tuning of the electronic properties of this interface.

Purpose:The purpose of this study was to compare knee biomechanics of the replaced limb to the non-replaced limb of total knee replacement(TKR)patients and healthy controls during walking on level ground and on decline surfaces of 5°,10°,and 15°.Methods:Twenty-five TKR patients and 10 healthy controls performed 5 walking trials on different decline slopes on a force platform and an instrumented ramp *** analyses of variance,2×2(limb×group)and 2×4(limb×decline slope),were used to examine selected biomechanics ***:The replaced limb of TKR patients had lower peak loading-response and push-off knee extension moment than the non-replaced and the matched limb of healthy *** differences were found in loading-response and push-off knee internal abduction moments among replaced,non-replaced,and matched limb of healthy *** knee flexion range of motion,peak loading-response vertical ground reaction force,and peak knee extension moment increased across all slope comparisons between 0°and 15°in both the replaced and non-replaced limb of TKR ***:Downhill walking may not be appropriate to include in early stage rehabilitation exercise protocols for TKR patients.

Retinopathy is a human eye disease that causes changes in retinal blood vessels that leads to bleed,leak fluid and vision *** of retinopathy are blurred vision,changes in color perception,red spots,and eye pain and it cannot be detected with a naked *** this paper,a new methodology based on Convolutional Neural Networks(CNN)is developed and proposed to intelligent retinopathy prediction and give a decision about the presence of retinopathy with automatic diabetic retinopathy screening with accurate *** CNN model is trained by different images of eyes that have retinopathy and those which do not have *** fully connected layers perform the classification process of the images from the dataset with the pooling layers minimize the coherence among the adjacent *** feature loss factor increases the label value to identify the patterns with the kernel-based *** performance of the proposed model is compared with the related methods of DReAM,KNN,GD-CNN and *** results show that the proposed CNN performs better.

Papillary fibroelastomas (PFes) are benign tumors of the endocardium that most frequently affect cardiac valves and typically present with embolic symptoms such as stroke or transient ischemic attack (TIA). Surgical excision is usually recommended for left-sided tumors and is associated with excellent long-term outcomes. The use of a robot-assisted, minimally invasive surgical approach for management of mitral valve disease is growing, and has been associated with shorter hospital stays and improved early quality of life. Three-dimensional (3D) transesophageal echocardiography (Tee) offers several advantages in the assessment of mitral valve disease and cardiac tumors, including the ability to precisely locate the site of attachment of the mass and the spatial relationships to surrounding structures. These factors are particularly important when planning a surgical approach. We report two cases of mitral valve PFes which were successfully removed using a robot-assisted, minimally invasive surgical approach with 3D Tee imaging. This approach to treatment of PFes is an attractive alternative to the traditional approach involving median sternotomy.
