Objective: Oral diadochokinesis (ODK) has been shown to be associated with oral frailty. In this study, we investigate the relationship between physical function, swallowing function, nutritional evaluation, and each syllable of ODK. Methods: The subjects are 26 elderly people living in an area where they can walk. ODK, tongue pressure, modified water swallowing test, EAT-10, grip strength, MNA-SF, skeletal muscle index, height, weight, and BMI were evaluated. For statistical analysis based on ODk values, the subjects were divided into two groups on the basis of the number of times the syllables /pa/, /ta/, and /ka/ were repeated: 1) The first group with >6 repetitions exceeded six; 2) the second group with ≤5 repetitions. Both groups were compared using the Mann-Whitney U test (Model 1). In addition, the syllables /pa/, /ta/, and /ka/ were also divided among the groups and a comparison was performed (Model 2). Results: In Model 1, the tongue pressure value was significantly higher in the good group (p ka/. Conclusion: In community-dwelling elderly people, tongue pressure was positively associated with ODk values for the syllables /pa/ and /ta/, suggesting that tongue pressure may be associated with strength of the lip and tongue tip muscles. Furthermore, it was suggested that the ODk values may even be lower than normal values in the elderly who maintain skeletal muscle mass and thus have adequate tongue pressure;therefore, these indicators may be used as a guideline to assess oral frailty.

Outcomes of locally advanced prostate cancer:a single institution study of 209 patients in Japan

Aim： To investigate the outcomes for Asian populations with locally advanced/clinical stage Ⅲ prostate cancer （PCa） treated with currently prevailing modalities. Methods： We reviewed the record of 209 patients with clinical stage Ⅲ PCa, who were treated at Niigata Cancer Center Hospital between 1992 and 2003. Treatment options included hormone therapy-combined radical prostatectomy （RP＋HT）, hormone therapy-combined external beam irradiation （EBRT＋HT） and primary hormone therapy （PHT）. Results： The 5- and 10-year overall survival rates were 80.3% and 46.1% in all cohorts, respectively. The survival rates were 87.3% and 66.5% in the RP＋HT group, 94.9% and 70.0% in the EBRT＋HT group and 66.1% and 17.2% in the PHT group, respectively. A significant survival advantage was found in the EBRT＋HT group compared with that in the PHT group （P 〈 0.0001）. Also, the RP＋HT group had better survival than the PHT group （P = 0.0107）. The 5- and 10-year disease-specific survival rates for all cases were 92.5% and 80.0%, respectively. They were 93.8% and 71.4% in the RP＋HT group, 96.6% and 93.6% in the EBRT＋HT group and 88.6% and 62.3% in the PHT group, respectively. A survival advantage was found in the EBRT＋HT group compared with the PHT group （P = 0.029）. No significant difference was found in disease-specific survival between the EBRT＋HT and RP＋HT groups or between the RP＋HT and PHT groups. Condusion： Although our findings indicate that radiotherapy plus HT has a survival advantage in this stage of PCa, we recommend therapies that take into account the patients＇ social and medical conditions for Asian men with clinical stage Ⅲ PCa.

Fatigue Alleviation Mechanism of Citric Acid Determined by Gene Expression Analysis in the Mouse Liver

Citric acid has been recognized for its ability to alleviate fatigue;however, the mechanism contributing to this effect has not yet been elucidated. Our previous study suggested that the alleviation of fatigue by citric acid intake might be induced by the acceleration of gluconeogenesis. In this study, we tested this hypothesis and evaluated whether the intake of citric acid accelerates gluconeogenesis in the mouse liver through microarray analysis. Because the microarray of focus includes 200 metabolic genes, it could be used to provide information on genes related to glucose metabolism as well as other pathways such as inflammation. Eight-week-old male C57/BL6J mice were divided into the distilled water and citric acid groups (n = 6), and were administered distilled water or a citric acid solution (20 mg/30g body weight) intragastrically with a probe, respectively. Forty minutes after administration, the mice were sacrificed by decapitation. The blood glucose level of the citric acid group was significantly higher (P = 0.001) than that of the distilled water group. The expression levels of genes related to glucose metabolism (Gck, Pfkl, Pklr, Pck1, G6pc) were not changed by the intake of citric acid. On the other hand, some transcriptional differences were observed in several other genes, including those related to inflammation, blood pressure, the electron transport system, uncoupling proteins, and anti-oxidation. The expression levels of genes related to inflammation were reduced in the citric acid group;e.g., the Il6 expression level was 0.78 times lower in the citric acid group than that in the distilled water group. In conclusion, the results of this study suggest that citric acid intake could accelerate gluconeogenesis and suppresses inflammation. Therefore, the fatigue-alleviation effects of citric acid might be induced by an additive effect between the maintenance of blood glucose levels and the suppression of inflammation.

A numerical analytic process is suggested combining direct boundary element methodwith discrete vortex method and internal flow in cross-flow fan calculated. The process considersnot only function of impeller, but also effect of volute casing. Internal flow field of cross-flow fan ispractically measured, and results compared with that from calculation. It shows that the process isexpected to be used in predicting performances of fluid machinery.

Objective: Aspiration pneumonia is thought to be caused by a decline in respiratory and swallowing function. We aimed to clarify whether the pectoralis major muscle and the rectus abdominis muscles are prognostic factors for aspiration pneumonia and to investigate the relationships between these muscles and nutritional status, swallowing function and sarcopenia, Methods: Medical records of 139 Japanese patients hospitalized for aspiration pneumonia between December 2010 and December 2014 were reviewed retrospectively. The volume and thickness of the pectoralis major muscle, the crosssectional area and thickness of the rectus abdominis muscles, and cross-sectional area of the psoas major muscle were measured using computed tomography. Swallowing function, nutritional status, activities of daily living, and prognosis also were evaluated.
