Oesophageal adenocarcinoma:In the era of extended lymphadenectomy,is the value of neoadjuvant therapy being attenuated?

BACKGROUND Neoadjuvant chemotherapy(NACT)and oesophagectomy is the standard of care for resectable oesophageal *** outcomes following resection have been improving over time while NACT remain largely ***,a recent meta-analysis of randomized control trials did not demonstrate a survival benefit in adding NACT,raising the possibility that improved surgical techniques may be reducing the perceived effectiveness of *** To compare the effect of addition of NACT to a standardized surgery and lymphadenectomy on overall and disease-free survival in patients undergoing curative oesophagectomy for oesophageal *** Patient data were analysed from a prospectively maintained surgical survival ***,surgical,and survival outcomes were compared between groups according to treatment and nodal *** The data of 243 consecutive patients were identified.79 patients were given NACT and 162 had surgery *** NACT group were younger,and there was less frequent stage I *** survival was similar between NACT and surgery only groups(5YS:48.7%vs 42.5%;P=0.113),as was diseasefree survival(5YS:40.6%vs 39.9%;P=0.635).There were≥30 nodes removed in 46 patients,and<30 in 197 patients,but were otherwise *** was improved survival in patients with≥30 nodes removed than those with<30 nodes(5YS:64.4%vs 40.7%;P=0.015),and a better disease-free survival that neared significance(5YS:54.9%vs 36.6%;P=0.078).CONCLUSION NACT did not appear to affect overall or disease-free ***,an overall survival benefit was observed in patients with≥30 lymph nodes removed,and a benefit in disease-free survival which was not significant.

Background: Recent projections expect that Vietnam will be affected most severely by climate change with higher temperatures, more precipitation and rising sea levels. Especially increased temperatures will worsen the situations in cities, amplifying the urban heat island effect. Green infrastructures, i.e. urban trees are a common rtool to improve the urban micro-climate for humans. Vital and well growing trees provide greatest benefits such as evaporative cooling, shading, air filtering and carbon storage. However, urban tree growth is often negatively affected by urban growing conditions such as high soil sealing with compacted tree pits providing small growing spaces with limited water, nutrient and oxygen supply, further warm temperatures and high pollution emissions. This study analyzed the growth of urban and rural African mahogany(Khaya senegalensis(Desr.) A. Juss.) trees in the city of Hanoi, Vietnam and the effects of the surrounding climate conditions on tree ***: The results showed that rural African mahogany trees grew better than trees situated in the city center, which is contrary to other results on tree growth of temperate and subtropical cities worldwide. Moreover tree growth was similar regardless of the time of growth. Other results regarding stem growth of African mahogany located in different areas of Hanoi(east, west, north, city center) revealed a better growth in the northern and western outskirts of the city compared to the growth of trees in the city ***: African mahogany trees in the urban centers of Hanoi showed a decreased growth compared to rural trees,which was likely induced by a low ground-water level and high pollution rates. In view of climate change and global warming, the decreased tree growth in the city center may also affect tree service provision such as shading and cooling. The histopathological spectrum of myocardial inflammation in relation to circumstance of death:a retrospective cohort study in clinical and forensic autopsies

Interpreting a myocardial inflammation as causal,contributory or as of no significance at all in the cause of death can be challenging,especially in cases where other pathologic and/or medico-legal findings are also *** further evaluate the significance of myocardial inflammation as a cause of death we performed a retrospective cohort study of forensic and clinical autopsy *** revised the spectrum of histological inflammatory parameters in the myocardium of 79 adult autopsy cases and related these to the reported cause of *** slides were reviewed for the distribution and intensity of inflammatory cell infiltrations,the predominant inflammatory cell type,and the presence of inflammation-associated myocyte injury,fibrosis,edema and ***,the cases were divided over three groups,based on the reported cause of *** 1(n=27)consisted of all individuals with an obvious unnatural cause of *** 2(n=29)included all individuals in which myocarditis was interpreted to be one out of more possible causes of *** 3(n=23)consisted of all individuals in which myocarditis was reported to be the only significant finding at autopsy,and no other cause of death was *** application of our histological parameters showed that only a diffuse increase of inflammatory cells could discriminate between an incidental presence of inflammation(Group 1)or a potentially significant one(Groups 2 and 3).No other histological parameter showed significant differences between the *** results suggest that generally used histological parameters are often insufficient to differentiate an incidental myocarditis from a(potentially)significant one.

Seasonal changes on two different spatial scales： response of aquatic invertebrates to water body and microhabitat

Knowledge about the spatial and temporal scales of both habitat use and the functional significance of different adaptations is essential for an understanding of the population dynamics of invertebrate assemblages. This fundamental knowledge is not only interesting from an academic point of view, but is sorely lacking and needed in the field of restoration ecology. Many species are threatened due to degradation. Knowing what environmental conditions are needed dtLring the life cycle of these species is important in the design of restoration measures which aim to lift existing bottlenecks for threatened species. To assess the relative importance of water type and microhabitat in structuring the invertebrate assemblage during different seasons, invertebrates were sampled in three water bodies differing in trophic level and acidity. Different parts within a water body （microhabitats） were sampled separately and each water body was sampled in all four seasons. Results show that water body is an important factor structuring the invertebrate assemblage early in the season, whereas microhabitat became more important later in the season. Structural complexity of microhabitats was related to the type of locomotion employed by invertebrates. Seasonal differences could be related to population dynamics （reproduction, mortality）. Moreover, fluctuations in resource availability were expected to differ between the water bodies, with highest fluctuations in the eutrophic water body and with fluctuations becoming less predictable later in the season. This was confirmed by the data： species synchronization to pulses in food availability was strongest in the eutrophic water body. Moreover, synchronization was strongest in summer, while in autumn waters were invaded by dispersive species. Based on these results a synthesis is presented on the functioning of the different waters during the different seasons.

Background:Anthropogenic disturbance can negatively affect an animal's energy budget by evoking movement *** research focuses mainly on immediate displacement as a disturbance effect,since this can be easily observed in the field. However, effects on movement over longer timescales are poorly examined and it is largely unknown if and to what extent they reflect immediate ***-term responses could for example be larger than immediate responses if birds, after disturbance, return to the original location and thereby travel twice the immediate disturbed distance. Methods:We combined GPS tracking data with observational data to quantify the effects of anthropogenic (air force and walkers) and non-anthropogenic disturbances on distances travelled by roosting Eurasian Oystercatchers (Haematopus ostralegus) during the non-breeding season. We compared immediate displacement after a disturbance with distance travelled during the entire high tide period (longer-term response), while accounting for environmental ***,we calculated energy expenditure due to disturbance based on observed disturbance ***:Disturbance resulted in an immediate displacement response of ~200 m (median).Air force disturbances tended to yield larger immediate responses than walker and,especially,than non-anthropogenic ***-term responses and immediate responses were approximately similar,suggesting that,over longer timescales,spatial disturbance effects in the study area remain confined to immediate ***,disturbances were infrequent (0.17 disturbances per bird per hour) and most disturbances were of natural origin (62%).Consequently, anthropogenic disturbance of roosting oystercatchers in the study area on average costs 0.08% of the daily energy expenditure. Tree species identity and interaction determine vertical forest structure in young planted forests measured by terrestrial laser scanning

Scintigraphy in laryngopharyngeal and gastroesophageal reflux disease:A definitive diagnostic test?

AIM: To investigate the utility of scintigraphic studies in predicting response to laparoscopic fundoplication(LF) for chronic laryngopharyngeal reflux ***: Patients with upper aero-digestive symptoms that remained undiagnosed after a period of 2 mo were studied with conventional p H and manometric studies. Patients mainly complained of cough, sore throat, dysphonia and globus. These patients were imaged after ingestion of 99m-technetium diethylene triamine pentaacetic acid. Studies were quantified with time activity curves over the pharynx, upper and lower oesophagus and background. Late studies of the lungs were obtained for aspiration. Patients underwent LF with post-operative review at 3 mo after ***: Thirty four patients(20 F, 14 M) with an average age of 57 years and average duration of symptoms of 4.8 years were studied. Twenty four hour p H and manometry studies were abnormal in all patients. On scintigraphy, 27/34 patients demonstrated pharyngeal contamination and a rising or flat pharyngeal curve. Lung aspiration was evident in 50% of patients. There was evidence of pulmonary aspiration in 17 of 34 patients in the delayed study(50%). Pharyngeal contamination was found in 27 patients. All patients with aspiration showed pharyngeal contamination. In the 17 patients with aspiration, graphical time activity curve showed rising activity in the pharynx in 9 patients and a flat curve in 8 patients. In those 17 patients without pulmonary aspiration, 29%(5 patients) had either a rising or flat pharyngeal graph. A rising or flat curve predicted aspiration with a positive predictive value of 77% and a negative predictive valueof 100%. Over 90% of patients reported a satisfactory symptomatic response to LF with an acceptable sideeffect profile. CONCLUSION: Scintigraphic reflux studies offer a good screening tool for pharyngeal contamination and aspiration in patients with gastroesophageal reflux disease.
