目的探究经颅磁刺激联合醒脑开窍针对脑卒中患者神经功能及认知功能的影响。方法选取76例脑卒中患者,随机分为治疗组与对照组各38例。两组均接受常规对症处理,在此基础上,治疗组予以经颅磁刺激联合醒脑开窍针治疗,而对照组仅予经颅磁刺激治疗,两组均治疗6周。分别于治疗前后进行神经递质指标[P物质(SP)、神经肽Y(NPY)、5-羟色胺(5-HT)]水平检测,并记录神经功能、认知功能、日常生活能力以及中医证候积分,同时于疗程结束后评价临床疗效并观察不良反应。结果治疗后,治疗组的总有效率高于对照组(P0.05)。两组各项中医证候积分、NIHSS评分以及SP、NPY水平均明显降低,且治疗组较对照组更低(P<0.01)。两组MoCA、BI评分以及5-HT水平均明显升高,且治疗组较对照组更高(P<0.01)。结论经颅磁刺激联合醒脑开窍针能够显著改善脑卒中患者的神经功能和认知功能,并对神经递质含量具有良好调节作用,疗效显著且安全性高。

Fault-slip taking place in underground mines occasionally causes severe damage to mine openings as a result of strong ground motion induced by seismic waves arising from fault-slip. It is indicated from previous studies that intense seismic waves could be generated with the shock unloading of fault surface asperities during fault-slip. This study investigates the shock unloading with numerical simulation. A three-dimensional （3D） numerical model with idealized asperities is constructed with the help of discrete element code 3DEC. The idealization is conducted to particularly focus on simulating the shock unloading that previous numerical models, which replicate asperity degradation and crack development during the shear behavior of a joint surface in previous studies, fail to capture and simulate. With the numerical model, static and dynamic analyses are carried out to simulate unloading of asperities in the course of fault-slip. The results obtained from the dynamic analysis show that gradual stress release takes place around the center of the asperity tip at a rate of 45 MPa/ms for the base case, while an instantaneous stress release greater than 80 MPa occurs near the periphery of the asperity tip when the contact between the upper and lower asperities is lost. The instantaneous stress release becomes more intense in the vicinity of the asperity tip, causing tensile stress more than 20 MPa. It is deduced that the tensile stress could further increase if the numerical model is discretized more densely and analysis is carried out under stress conditions at a great depth. A model parametric study shows that in-situ stress state has a significant influence on the magnitude of the generated tensile stress. The results imply that the rapid stress release generating extremely high tensile stress on the asperity tip can cause intense seismic waves when it occurs at a great depth.

AIM:To assess the appropriateness of the indication and route of administration of proton-pump-inhibitors (PPIs) and their associated cost impact. METHODS:Data collection was performed prospec-tively during a 6-mo period on 340 patients who re-ceived omeprazole intravenously during their hospital stay in non-intensive care floors. Updated guidelines were used to assess the appropriateness of the indication and route of administration. RESULTS:Complete data collection was available for 286 patients which were used to assess intravenous (IV) PPIs utilization. Around 88% of patients were receiving PPIs for claimed stress ulcer prophylaxis (SUP) indication; of which,only 17% met the guideline criteria for SUP indication,14% met the criteria for non-steroidal-anti-inflammatory drugs-induced ulcer prophylaxis,while the remaining 69% were identifi ed as having an unjustified indication for PPI use. Theinitiation of IV PPIs was appropriate in 55% of pa-tients. Half of these patients were candidates for switching to the oral dosage form during their hos-pitalization,while only 36.7% of these patients were actually switched. The inappropriate initiation of PPIs via the IV route was more likely to take place on the medical floor than the surgical floor (53% vs 36%,P = 0.003). The cost analysis associated with the appro-priateness of the indication for PPI use as well as the route of administration of PPI revealed a possible saving of up to $17 732.5 and $14 571,respectively. CONCLUSION:This study highlights the over-utili-zation of IV PPIs in non-intensive care unit patients. Restriction of IV PPI use for justified indications and route of administration is recommended.

This paper aims to improve the accuracy and applicability of gas diffusion mathematical models from coal particles. Firstly, a new constitutive model for gas diffusion from coal particles with tri-disperse pore structure is constructed by considering the difference in characteristics between soft coal and hard *** analytical solution is then derived, that is, the quantitative relationship between gas diffusion rate(Qt/Q_∞) and diffusion time(t), The pore structure parameters of soft coal and hard coal from Juji coal mine are determined. Gas diffusion rules are numerically calculated and investigated by physical simulation methods. Lastly, the applicability of this model is verified. The results show that the homogeneous model only applies to the gas diffusion process of hard coal during the initial 10 min. The calculation results from this model and the physical experimental results of soft coal and hard coal are nearly identical during the initial 30 min.

Physio-biochemical and nutrient constituents of peanut plants under bentazone herbicide for broad-leaved weed control and water regimes in dry land areas

The abundance of broad-leaved weeds in peanut fields represents the handicap in weed management programs, since limited specific herbicides can be recommended to control them. Moreover, the physio-biochemical constituents and nutritional status in peanut plants as affected by available herbicides, i.e., bentazone under water stress conditions are not well known. Therefore, field trials were conducted during the growing seasons in 2016 and 2017 to investigate the interactional impact of irrigation levels(I_(50), I_(75) and I_(100), representing irrigation by 50%, 75% and 100% of crop evapotranspiration, respectively) and weed control practices(bentazone, bentazone+hoeing once, hoeing twice and weedy check as control) on dominant broad-leaved weeds as well as peanut physiological and agronomic traits. Result indicated that the efficiency of weed control for each weeded treatment under I_(50) significantly equaled with its counterpart under I_(75) or I_(100). Bentazone+hoeing once diminished weed biomass by 89.3% and enhanced chlorophyll content of peanut plants by 51.2%. Bentazone relatively caused a reduction in carotenoides. Hoeing twice and bentazone+hoeing once under I_(100) in both growing seasons as well as hoeing twice under I_(75) in 2017 were the superior combinations for boosting pod yield of peanut plants. Treatment of bentazone+hoeing once and I_(75) recorded the lowest reduction in N utilization percentage and the highest increase in potassium utilization percentage of peanut plants. Eliminating weeds enhanced water use efficiency by 37.8%, 49.6% and 34.7% under I_(50), I_(75) and I_(100), respectively. In conclusion, peanut seems to be tolerant to bentazone at moderate water supply, thus it can be safely used in controlling the associated broad-leaved weeds.

Coal seam gas content is frequently measured in quantity during underground coal mining operation and coalbed methane(CBM)exploration as a significant basic *** to the calculation error of lost gas and residual gas in the direct method,the efficiency and accuracy of the current methods are not inadequate to the large area multi-point measurement of coal seam gas *** paper firstly deduces a simplified theoretical dynamic model for calculating lost gas based on gas dynamic diffusion ***,the effects of various factors on gas dynamic diffusion from coal particle are experimentally *** sampling procedure of representative coal particle is ***,a new estimation method of residual gas content based on excess adsorption and competitive adsorption theory is *** results showed that the maximum error of calculating the losing gas content by using the new simplified model is only 4%.Considering the influence of particle size on gas diffusion law,the particle size of the collected coal sample is below 0.25 mm,which improves the measurement speed and reflects the safety representativeness of the *** determination time of gas content reduced from 36 to 3 h/***,the absolute error is 0.15–0.50 m^3/t,and the relative error is within 5%.A new engineering method for determining the coal seam gas content is developed according to the above research.
