Reliability of Plasm. Von Willebrand Factor Antigen in Prediction of Esophageal Varices in Patients with Liver Cirrhosis

Background: Bleeding esophageal varices (OVs) due to portal hypertension are one of the m.jor com.lications with high m.rtality in liver cirrhosis. So, early detection and m.nagem.nt are m.ndatory. Aim. To evaluate the role of Von Willebrand factor (VWF) in predicting the presence of OVs. Patients and m.thods: 62 patients with liver cirrhosis representing different Child-Pugh classes were included. The diagnosis of liver cirrhosis was based on the com.ination of clinical, laboratory and US exam.nations. All included patients underwent the following investigations: com.lete blood count, liver function tests (ALT, AST, serum.bilirubin, album.n and total protein, prothrom.in tim. (PT) and concentration (PC), INR and serum.alkaline phosphatase), serum.creatinine, Von Willebrand factor antigen (VWF-Ag) m.asurem.nt and abdom.nal US. Upper endoscopic evaluation was done to detect presence or absence of varices (esophageal or gastric) and/or PHG. Results: 38 m.les and 24 fem.les with their m.an age (46 ± 12 years old) were included. Plasm. Von Willebrand factor-Ag level was significantly higher in patients with OVs than those without varices (P value = 0.000). Also, its level was significantly higher in patients with higher grade of OVs, G3 than those with G1 or G2 (P value = 0.000). Patients with large OVs including those with G2 and G3 showed significantly higher values of VWF than those with sm.ll OVs (NO and G1) (P value = 0.000). VWF was independent predictor for detecting the presence of OVs with good sensitivity (90), specificity (77.3) and accuracy (85.5) at a cutoff value of 1.74 U/m.. Also it was an independent predictor for detecting the presence of large OVs with good sensitivity (91.2), specificity (85.7) and accuracy (88.7) at a cutoff value of 2.16 U/m.. Conclusion: VWF-Ag could be used as a non invasive laboratory independent predictor for the detection of OVs.

Passive design responds to local clim.te and site conditions in order to m.xim.se the com.ort and health of building users while m.nim.sing energy use. The key to designing a passive building is to take best advantage of the local clim.te. Passive cooling refers to any technologies or design features adopted to reduce the tem.erature of buildings without the need for power consum.tion. Consequently, the aim.of this study is to test the usefulness of applying selected passive cooling strategies to im.rove therm.l perform.nce and to reduce energy consum.tion of residential buildings in hot arid clim.te settings, nam.ly Dubai, United Arab Em.rates. One case building was selected and eight passive cooling strategies were applied. Energy sim.lation software nam.ly IES was used to assess the perform.nce of the building. Solar shading perform.nce was also assessed using Sun Cast Analysis, as a part of the IES software. Energy reduction was achieved due to both the harnessing of natural ventilation and the m.nim.sing of heat gain in line with applying good shading devices alongside the use of double glazing. Additionally, green roofing proved its potential by acting as an effective roof insulation. The study revealed several significant findings including that the total annual energy consum.tion of a residential building in Dubai m.y be reduced by up to 23.6% when a building uses passive cooling strategies.

This work presents the basic characteristics of singly,doubly and heavily charged projectile fragm.nts（PFs） em.tted in inelastic interactions of ^（32）S ions with photo-em.lsion nuclei at Dubna energy（3.7 A GeV）.Our experim.ntal data are com.ared with the corresponding data for other projectiles at the sam. incident *** study of m.an m.ltiplicities of different charged PFs against the projectile m.ss shows a power-law *** m.ltiplicity distributions of singly and doubly charged PFs have been fitted well with a Gaussian distribution *** yields of PFs broken up from.the interactions of ^（32）S projectile nuclei with different target nuclei are *** beam.energy dependence in term. of the various order m.m.nts is studied as well.
