Objective: To elucidate the molecular mechanisms of the potent anti-obesity effect of moringa oleifera Lam.(m. oleifera) ethanolic extract and to clarify the link between these mechanisms and the associated metabolic and vascular risks in the experimental model of visceral ***: m. oleifera ethanolic extract was orally administered at 600 mg/kg body weight in obese female rats daily for 12 weeks. At the end of treatment, body weight was determined, and the atherogenic index, coronary artery index, glucose level, insulin resistance status, liver and kidney functions were assessed. Also, the m RNA of leptin, adiponectin and resistin in visceral adipose tissue was determined by quantitative real ***: The results showed that m. oleifera extract down-regulated m RNA expression of leptin and resistin, while it up-regulated adiponectin gene expression in obese rats relative to untreated obese control counterparts. This amelioration of genes expression was paralleled by a reduction in body weight and improvement of the atherogenic index and coronary artery index, as well as glucose level and insulin resistance value without adverse effects on liver or kidney functions, versus the untreated obese control ***: It is reasonable to assume that the anti-obesity, anti-atherogenic and antidiabetic properties of m. oleifera are mechanistically achieved via working directly on the adipokines of the visceral adipose tissue. Therefore, m. oleifera may be a good therapeutic candidate for the symptoms of metabolic syndrome.

The stability of the pure calcium sulfoaluminate hydrates at room and boiling water temperatures is studied. The formation of these phases during the hydration of C3 A with gypsum at room temperature, also the effects of lime, alite and alkali hydroxide on the reaction are investigated. Based on the results, a mechanism for the delayed ettringite formation is proposed. It is found that a sulfate deficient ettringite is formed as a result of the decomposition of the monosulfate hydrate in water at room temperature. The process is accelerated by unsaturated lime solution and is delayed by portlandite . The ettringite converts to monosulfate hydrate in boiling water and to sulfatepoor monophases in ≥50. 2m sodium hydroxide solution at room temperature. The monosulfate hydrate decomposes further to hydrogarnet and calcium hydroxide by boiling in ≤0. 2m alkali hydroxide . It is suggested that the expansive reaction of the ettringite is connected to the incorporation of excess sulfate in its structure.

Applicability of Sigmoid Colon Graft for Vaginal Replacement (Colovaginoplasty) at Young Age

Objective: - To evaluate the applicability, feasibility, complications, short and long term results of vaginoplasty by using sigmoid colon graft with stapler assistance for resuming colonic continuity in children with absent vagina. Patients and methods: - Records of 15 patients 8 of them had Androgen Insensitivity Syndrome, 4 had congenital vaginal atresia, and 3 had Congenital Adrenal Hyperplasia, underwent sigmoid graft vaginal reconstruction with the aid of circular stapler during the period from 2005 to 2010 were reviewed retrospectively. Results: A neovagina was constructed in those patients who aged from 3 to 9 years (mean 4 years), where sigmoid colon was used in all patients. Hospital stay ranged from 5 to 12 days (mean 7), operative time ranged from 60 to 120 min (mean 75 min). All patients were followed up 6 monthly in the first year and then yearly, complications occurred in 4 patients: introital stenosis in 2, and another 2 cases had minor wound infection. The need for neovaginal dilation was indicated in 5 out of 15 patients. There was no complications related to bowel anastomosis, No mortality in this group and the cosmetic results were excellent in all cases. Conclusion: Sigmoid colovaginoplasty appears to be applicable for remedy of vaginal atresia at any age. It is associated with a less complication rate, the long term results is satisfactory and it seems a better technique, especially in girls who doesn’t expect sexual activity shortly after surgery.

Impact of cigarette smoking on response to interferon therapy in chronic hepatitis C Egyptian patients

AIm:Smoking may affect adversely the response rate to interferon-α.Our objective was to verify this issue among chronic hepatitis C patients. mETHODS:Over the year 1998,138 chronic hepatitis C male Egyptian patients presenting to Cairo Liver Center, were divided on the basis of smoking habit into:group I which comprised 38 smoker patients(>30 cigarettes/d) and group Ⅱ which included 84 non-smoker patients. Irregular and mild smokers(16 patients)were excluded. Non eligible patients for interferon-α therapy were excluded from the study and comprised 3/38(normal ALT)in group I and 22/84 in group Ⅱ(normal ALT,advanced cirrhosis and thrombocytopenia).Group I was randomly allocated into 2 sub-groups:group Ia comprised 18 patients who were subjected to therapeutic phlebotomy while sub-group Ib consisted of 17 patients who had no *** sub-group la,3 patients with normal ALT after repeated phlebotomies were excluded from the ***-α 2b 3 mU/TIW was given for 6 mo to 15 patients in group Ia,17 patients in group Ib and 62 patients in group Ⅱ. Biochemical,virological end-of-treatment and sustained responses were evaluated. RESULTS:At the end of interferon-α treatment,ALT was normalized in 3/15 patients(20%)in group Ia and 2/17 patients(11.8%)in group Ib compared to17/62 patients (27.4%)in group Ⅱ(P=0.1).Whereas 2/15 patients(13.3%) in group *** 2/17 patients(11.8%)in group Ib lost viraemia compared to 13/62 patients(26%)in group Ⅱ (P=0.3).Six months later,ALT was persistently normal in 2/15 patients(13.3%)in group 1a and 1/17 patients (5.9%)in group Ib compared to 9/62 patients(14.5%)in group Ⅱ(P=0.47).Viraemia was eliminated in 1/15 patients (6.7%)in group Ia and 1/17 patients(5.9%)in group Ib compared to 7/62 patients(11.3%) in group Ⅱ,but the results did not mount to statistical significance(P=0.4). AIm： To observe the imbalance between T helper cell Th1 and Th2 cytokines in several chronic hepatitis disease at different stages of disease progression. mETHODS： We measured the cytokine levels of Thl （IL-2 and IL-2R）, Th2 （IL-10） and the pro-inflammatory cytokines （IL-6 and IL-6R and TNF and TNF-RI and II） by the ELISA technique in the sera of 33 hepatocellular carcinoma （HCC） patients and 20 chronic liver disease （CLD） patients. In addition, 20 asymptomatic hepatitis C virus carriers and 20 healthy subjects negative for hepatitis C virus（HCV） markers served as controls. RESULTS： Anti-HCV antibodies were found to be positive in 94% of HCC cases and 75% of CLD cases. On the other hand, HCV viremia was detected using RT- PCR in 67% of HCC cases and 65% of CLD cases. HBsAg was positive in 9% of HCC cases and 30% of CLD cases. Also bilharziaI-Ab was positive in 55% of HCC cases, 65% of CLD cases and in 70% of asymptomatic carriers （ASC）. HCC patients had significantly higher values of IL- 2R, TNF-RII （P〈0.001）, and TNF-RI （P〉0.05）, but lower TNFα （P〈0.001） and IL-6 （P = 0.032） in comparison to ASC. But, in comparison to non-cancer controls, HCC patients had higher values of IL-2R, IL-6R, TNF-RI and TNF-RII, but lower TNF-α （P〈0.001）. CLD patients had higher IL-2R, TNF-RI, and TNF-RII （P〈0.001） than ASC. But, in comparison to non-cancer controls, CLD patients had higher values of IL-2R, TNF-RI and TNF-RII, but lower TNF-α （P〈0.001）. IL-10 was higher （though not significantly） in HCC and CLD patients than in symptomatic carriers and non-cancer controls. CONCLUSION： Liver disease progression from CLD to HCC due to HCV genotype-4 infection is associated with an imbalance between Th1 and Th2 cytokines. IL-2R, TNF-RI, and TNF-RII could be used as potential markers.

Formation and Stabilization of Vaterite Calcium Carbonate by Using Natural Polysaccharide

This paper aimed to study the effects of natural polysaccharide (gum arabic, GA) on the formation of vaterite calcium carbonate and its stability in aqueous solution. Gum arabic is a macromolecule that has a high molecular weight and is amphoteric in character. A 0, 2.5, 5 or 10 ml of GA solution (30%) was added to the calcium chloride solution with various initial concentrations to prepare the vaterite calcium carbonate. The calcium carbonate particles were characterized using FTIR, XRD, SEm, TEm and DSC-TG as well as calculation of phase contents. The results of XRD and FTIR analysis were showing that the presence of GA during the precipitation changes the behavior of calcium carbonate to form vaterite until at lower dose used. In addition, the molar content of vaterite increased at the expense of content of calcite with increasing the GA dose. There are slightly improvements in the vaterite content by increasing the initial concentration. The formed vaterite calcium carbonates composed of aggregates as broccoli-like or spherical shape and with particle size 1.6 - 2.5 μm in diameter. TEm images showed that these aggregates composed of the sub spherical unit with diameter 15 - 30 nm. The thermal behavior of prepared vaterite was conformed the mineral composition of these phases. The prepared calcium carbonate, which is formed in the presence of different doses of gum, has a stability in the aqueous solutions at different temperatures, as the analyzes indicated that a very small percentage of the vaterite were converted to calcite. The gum arabic prevented or depressed the transformation of vaterite to calcite through capped the surface of particles, i.e. it is more effective in stabilizing the vaterite phase with increasing in initial concentration and time.

Hepatocellular carcinoma in Egypt: A single center study over a decade

AIm： To identify the trend, possible risk factors and any pattern change of hepatocellular carcinoma （HCC） in Egypt over a decade. mETHODS： All HCC patients attending Cairo Liver Center between January 1993 and December 2002, were enrolled in the study. Diagnosis of HCC was based on histopathological examination and/or detection of hepatic focal lesions by two imaging techniques plus α-fetoprotein level above 200 ng/mL. The duration of the study was divided into two periods of 5 years each; period Ⅰ （1993-1997） and period Ⅱ （1998-2002）. Trend, demographic features of patients （age, gender, and residence）, risk factors （HBsAg, HCV-Ab, schistosomiasis and others） and pattern of the focal lesions were compared between the two periods. Logistic regression model was fitted to calculate the adjusted odds ratios for the potential risk factors. The population attributable risk percentage was calculated to estimate the proportion of HCC attributed to hepatitis B and C viral infections. RESULTS： Over a decade, 1 328 HCC patients out of 22 450 chronic liver disease （CLD） patients were diagnosed with an overall proportion of 5.9%. The annual proportion of HCC showed a significant rising trend from 4.0% in 1993 to 7.2% in 2002 （P = 0.000）. A significant increase in male proportion from 82.5% to 87.6% （P = 0.009）; m/F from 5：1 to 7：1 and a slight increase of the predominant age group （40-59 years） from 62.6% to 66.8% （P = 0.387）in periods Ⅰ and Ⅱ respectively, reflecting a shift to younger age group. In the bivariate analysis, HCC was significantly higher in rural residents, patients with history of schistosomiasis and/or blood transfusion. Yet, after adjustment, these variables did not have a significant risk for development of HCC. There was a significant decline of HBsAg from 38.6% to 20.5% （P = 0.000）, and a slight increase of HCV-Ab from 85.6% to 87.9% in periods I and II respectively. Value of duplex doppler ultrasonography in non-invasive assessment of children with chronic liver disease

AIm:To investigate the value of duplex Doppler ultrasonography (US) in the assessment of the hemodynamics of the portal and hepatic veins in a cohort of children with chronic liver disease (CLD) and to detect any relationship between the US changes,etiology and severity (or stage) of CLD. mETHODS:We prospectively enrolled 25 children with biopsy-proven CLD. Thirteen had cirrhosis (aged 8.9 ± 2.0 years) and 12 had chronic hepatitis (aged 9.3 ± 2.3 years). Gray scale and color-coded duplex Doppler US were performed for all,as well as 30 healthy age and sex-matched controls. Findings were correlated with clinical,laboratory and histopathological characteristics. RESULTS:Prominent caudate lobe was detected in 100% of cirrhotics,but none of the chronic hepatitis or controls. Thickened lesser omentum and loss of the triphasic waveform of the hepatic vein were present in 69.2% and 53.8% of cirrhotics vs 33.3% and 8.3% of chronic hepatitis respectively. Portal vein flow velocity was significantly lower (P mpared to controls. Child-Pugh's staging showed a positive correlation with both abnormal hepatic vein waveform and direction of portal blood flow; and a negative correlation with both hepatic and portal vein flow velocities. No correlation with the etiology of CLD could be detected. CONCLUSION:Duplex Doppler added to grayscale US can detect significant morphologic and portal hemodynamic changes that correlate with the severity (stage) of CLD,but not with etiology.
