AIM： To study the safety and efficacy of hepatitis A vaccine （HAV） in children with chronic liver disease of various etiologies. METHODS： eleven children with chronic liver disease and thirteen age- and sex-matched controls negative for HAV antibodies were vaccinated against hepatitis A after they gave their informed consent. Children with uncontrolled coagulopathy or signs of hepatic decompensation were excluded. The vaccine （Havrix： 720 elISA units in 0.5 mL, from GlaxoSmithKline Biologicals） was given intramuscularly in the deltoid in 2 doses 6 mo apart. Children were tested for HAV antibodies one and six months after the ist dose and one month after the 2^nd dose. Total serum bilirubin, alanine aminotransferase （ALT）, and aspartate aminotransferase （AST） were determined immediately before and after one month of the 1st dose of the vaccine. RESULTS： Only 7 out of the 11 patients were positive for HAV antibodies after the 1^st dose of the vaccine, as compared to 100% of the controls. One month after the 2nd dose, all patients tested were positive for HAV antibodies. No deterioration in liver functions of patients was noted after vaccination. No adverse events, immediate or late, were reported by the mothers after each dose of the vaccine. CONCLUSION： Hepatitis A vaccine is both safe and effective in this small studied group of children with chronic liver disease. Given the high seroconversion rate, post-vaccination testing for HAV antibodies is not needed.

AIM:To investigate the value of duplex Doppler ultrasonography (US) in the assessment of the hemodynamics of the portal and hepatic veins in a cohort of children with chronic liver disease (CLD) and to detect any relationship between the US changes,etiology and severity (or stage) of CLD. METHODS:We prospectively enrolled 25 children with biopsy-proven CLD. Thirteen had cirrhosis (aged 8.9 ± 2.0 years) and 12 had chronic hepatitis (aged 9.3 ± 2.3 years). Gray scale and color-coded duplex Doppler US were performed for all,as well as 30 healthy age and sex-matched controls. Findings were correlated with clinical,laboratory and histopathological characteristics. RESULTS:Prominent caudate lobe was detected in 100% of cirrhotics,but none of the chronic hepatitis or controls. Thickened lesser omentum and loss of the triphasic waveform of the hepatic vein were present in 69.2% and 53.8% of cirrhotics vs 33.3% and 8.3% of chronic hepatitis respectively. Portal vein flow velocity was significantly lower (P elation with both abnormal hepatic vein waveform and direction of portal blood flow; and a negative correlation with both hepatic and portal vein flow velocities. No correlation with the etiology of CLD could be detected. CONCLUSION:Duplex Doppler added to grayscale US can detect significant morphologic and portal hemodynamic changes that correlate with the severity (stage) of CLD,but not with etiology.

AIM: To assess hepatic fibrosis and factors associated with its progression in children with HCV infection. METHODS: At the Hepatology Unit,Cairo University Children's Hospital,a single liver biopsy was performed to 43 children with HCV infection after an informed consent between 1998-2004. Their mean age at liver biopsy was 8.67 ± 4.3 years. RESULTS: Among the 43 patients' biopsies,12 (27.9%) were having no fibrosis,20 (46.5%) mild fibrosis and 11 (25.6%) moderate to severe fibrosis. The median time for development of fibrosis was estimated to be 5.5 years. Developing fibrosis was significantly associated with shorter duration from first detected ALT elevation to biopsy (12 mo vs 1.2 mo,P = 0.015) and having higher levels of direct serum bilirubin (0.3 mg/dL vs 0.5 mg/dL,P = 0.048). No association was found between fibrosis stage and the presence of co-morbid conditions (P = 0.33). CONCLUSION: Hepatic fibrosis was present in 72.1% of children with HCV infection. The development of fibrosis was associated with higher levels of direct serum bilirubin. There was no significant association between fibrosis and age,duration of infection,risk factors,co-morbid conditions and most biochemical parameters.

In this work,the magnetic,dielectric properties and electric modulus of Ce^(3+) substituted cobaltmagnesium(Co_(0.7)Mg_(0.3)Ce_(x)Fe_(2-x)O_4)(labeled as CMCF) ferrite nanoparticles were investigated in *** magnetization decreases from 50.05 to 34.87 emu/g for further substituting Ce^(3+) ***,coercivity increases from 738.22 Gs for the CMCFO sample to 912.10 Gs for the CMCF2 sample,then decreases monotonically to 762.1 Gs for the CMCF5 *** cerium content and particle size play important roles in controlling the magnetization and coercivity of the CMCF *** CMCF nanoferrites are suitable for microwave applications since their high-frequency response ranges from 7.72 to 11.07 *** CMCF nanoferrites' dielectric parameter dispersion exhibits normal *** pristine Co-Mg nanoferrite only has ε' value of 28.25,but the nanoferrite MCMF2 has ε' value of365.03,with an enhancing ratio of 1192%.The conduction mechanism of the MCMF nanoferrites was determined by fitting the σ_(ac) results via the Jonscher power *** 653 K,large polaron tunnelling is thought to be responsible for this conduction process,which is followed by electron barrier hopping at higher ***-Cole diagrams at different temperatures,assuring the contributions of the grains and their boundaries at lower temperatures(653 K) and only the grains at higher *** on our results,the CMCF nanoferrites hold magnetic and semiconducting nature,which can be used in magnetic devices and dielectrics in lower-frequencies or conductors in higher-frequencies.

Nonlinear fractional differential equations provide suitable models to describe real-world phenomena and many fractional derivatives are varying with time and *** present study considers the advanced and broad spectrum of the nonlinear(NL)variable-order fractional differential equation(VO-FDE)in sense of VO Caputo fractional derivative(CFD)to describe the physical *** VO-FDE transforms into an ordinary differential equation(ODE)and then solving by the modified(G/G)-expansion *** ac-curacy,the space-time VO fractional Korteweg-de Vries(KdV)equation is solved by the proposed method and obtained some new types of periodic wave,singular,and Kink exact *** newly obtained solutions confirmed that the proposed method is well-ordered and capable implement to find a class of NL-VO *** VO non-integer performance of the solutions is studied broadly by using 2D and 3D graphical *** results revealed that the NL VO-FDEs are highly active,functional and convenient in explaining the problems in scientific physics.

Heart disease remains a leading cause of morbidity and mortality worldwide,highlighting the need for improved diagnostic *** diagnostics face limitations such as reliance on single-modality data and vulnerability to apparatus faults,which can reduce accuracy,especially with poor-quality ***,these methods often require significant time and expertise,making them less accessible in resource-limited *** technologies like artificial intelligence and machine learning offer promising solutions by integrating multi-modality data and enhancing diagnostic precision,ultimately improving patient outcomes and reducing healthcare *** study introduces Heart-Net,a multi-modal deep learning framework designed to enhance heart disease diagnosis by integrating data from Cardiac Magnetic Resonance Imaging(MRI)and electrocardiogram(ECG).Heart-Net uses a 3D U-Net for MRI analysis and a Temporal Convolutional Graph Neural Network(TCGN)for ECG feature extraction,combining these through an attention mechanism to emphasize relevant *** is performed using Optimized *** approach improves early detection,reduces diagnostic errors,and supports personalized risk assessments and continuous health *** proposed approach results show that Heart-Net significantly outperforms traditional single-modality models,achieving accuracies of 92.56%forHeartnetDataset Ⅰ(HNET-DSⅠ),93.45%forHeartnetDataset Ⅱ(HNET-DSⅡ),and 91.89%for Heartnet Dataset Ⅲ(HNET-DSⅢ),mitigating the impact of apparatus faults and image quality *** findings underscore the potential of Heart-Net to revolutionize heart disease diagnostics and improve clinical outcomes.

Life Time Attributable Cancer Risk Estimated Using Scanner Reported Dose Length Product during Chest Computed Tomography Imaging in Young Children

This study aims to estimate the lifetime attributable cancer risk (LAR) for pediatric chest computed tomography (CT) examinations in five age groups using recently published age and region-specific conversion coefficients multiplying the widely available scanner registered dose length products (DLP) displayed on the CT console and hence calculating the Effective Dose (ED). The ED is then multiplied by the International Commission on Radiological Protection (ICRP) published risk factor for LAR. The obtained LAR values are compared with the international literature. Factors that may affect the LAR value are reported and discussed. The study included one hundred twenty five chest CT examinations for both males and females aged from less than one year to fifteen years. The patients reported data are from one single medical institution and using two CT scanners from June 2022 to December 2023. The results of this study may serve as benchmark for institutional radiation dose reference levels and risk estimation.
