AIM: To summarize the effects of laparoscopic ethanol injection and radiofrequency ablation (L-EI-RFA), thora- coscopic (T-EI-RFA) and open-surgery assisted EI-RFA (O-EI-RFA) under general anesthesia for the treatment of hepatocellular carcinoma (HCC). METHODS: Time-lag performance of RFA after ethanol injection (Time-lag PEI-RFA) was performed in all cases. The volume of coagulated necrosis and the applied en- ergy for total and per unit volume coagulated necrosis were examined in the groups treated under general (group G) or local anesthesia (group L). RESULTS: The results showed that the total applied energy and the applied energy per unit volume of whole and marginal, coagulated necrosis were significantly larger in group G than those in the group L, resulting in a larger volume of coagulated necrosis in the group G. The rate of local tumor recurrence within one year was extremely low in group G. CONCLUSION: These results suggest that EI-RFA, un- der general anesthesia, may be effective for the treat- ment of HCC because a larger quantity of ethanol and energy could be applied during treatment under pain- free condition for the patients.

AIM:To evaluate the left ventricular structure and function in isolated metabolic ***:One hundred and fifty six consecutive adults with metabolic syndrome were enrolled in the *** nine had isolated metabolic syndrome(group A) and 97 had metabolic syndrome with hypertension and/or diabetes(group B).There was a control group of 34 healthy *** addition to classic echocardiographic assessment of myocardial structural and functional changes,the Tei index was used to evaluate global left ventricular ***:There were no statistically significant differences between group A and controls in all parameters of left ventricular structural,systolic,and diastolic function except global myocardial performance(Tei index).On the other hand,significant differences were observed between group B and the control group in most of the parameters of left ventricular structural and global ***:The early identification of isolated metabolic syndrome in non-diabetic,non-hypertensive adults may be an indication that aggressive preventive measures should not be postponed until overt obesity,hypertension or diabetes mellitus has developed.

Paleo and recent earthquakes have been recorded in Wadi Hagul area and its environs, which have left behind geologic structures of deformation preserved in exposed sedimentary rocks. To evaluate such deformation and surface break, different techniques and data types are used compromising image processing techniques, Geographic Information Systems (GIS), seismicity data, as well as, field investigation. The field investigation clarified that the study area is enriched with soft-sediment deformation structures encompassing two types of geologic structures;brittle and viscoplastic structures. The analysis of the various types of data elucidate that, the earthquakes of Wadi Hagul are frequently distributed at an average depth ranging from (1 to 35 km) within the top of the Earth’s crust which are mainly controlled by existing Hagul fault zone. The study gives new insight for a better understanding of the seismic activity in the study area which helps in the seismic hazard

The HKC （Hong Kong Convention） adopted by the IMO in 2009 is aimed at ensuring that ships, when being recycled after reaching the end of their operational lives, do not pose any unnecessary risk to human health and safety of the marine environment. The text of the ship recycling Convention was developed with input from the IMO （International Maritime Organization） Member States and relevant NGOs （Non-Governmental Organizations）, and in co-operation with the ILO （International Labor Organization） and the Parties to the Basel Convention. The Convention intends to address all the issues around ship recycling, taking into account that ships sold for scrapping may contain environmentally hazardous substances. It addresses concerns raised about the working and environmental conditions at many of the world ship recycling locations. Regulations in the new Convention cover： the design, construction, operation and preparation of ships so as to facilitate safe and environmentally sound recycling, without compromising the safety and operational efficiency of ships; the operation of ship recycling facilities in a safe and environmentally sound manner; and the establishment of an appropriate enforcement mechanism for ship recycling, incorporating certification and reporting requirements. This paper will present and discuss the evaluation of the Hong Kong Convention. Also it will discuss implementing the Hong Kong convention to improve the recycling business, highlighting the effect of the convention on the maritime industry.

Use of the urinary trypsinogen-2 dipstick test in early diagnosis of pancreatitis after endoscopic retrograde cholangiopancreatography (ERCP)

Background: Acute pancreatitis is one of the most serious complications of ERCP. Early diagnosis of post ERCP pancreatitis helps physicians to provide intensive care and possible medical treatment as early as possible. Trypsinogen-2 in urine is a good diagnostic and prognostic marker of acute pancreatitis. Objectives: To evaluate the diagnostic value of urinary trypsinogen-2 dipstick test for early diagnosis of post ERCP pancreatitis. Methods: A total of 37 patients with obstructive jaundice were tested with the urinary trypsinogen-2 dipstick test and serum levels of amylase and lipase before ERCP and 6 hours after ERCP. Results: Post ERCP pancreatitis was diagnosed in 6 (16%) of 37 patients. The sensitivity, specificity, positive predictive value and negative predictive value of urinary trypsinogen-2 dipstick test at 6 hours after ERCP were 100%, 97%, 86%, 100% respectively. At the cutoff level (130 U/L) for lipase, the positive predictive value and negative predictive value all were (100%), however, the positive predictive value and negative predictive value for amylase levels at cutoff (122 U/L) were 60%, 100% respectively. Serum lipase level was the best test for diagnosing post ERCP pancreatitis followed by the urinary trypsinogen-2 dipstick test. Conclusions: The urinary trypsinogen-2 dipstick test can be used as a rapid and easy test for early diagnosis of post ERCP pancreatitis with high sensitivity and specificity.

Degree of Hyper-Dynamic Circulation Correlates with the Severity of Liver Disease and Predicts Poor Outcome in Spontaneous Bacterial Peritonitis Patients in Intensive Care Unit

Background: Circulatory dysfunction is known in spontaneous bacterial peritonitis (SBP) patients. We aimed to determine whether degree of hyper-dynamic circulation is significantly correlated with severity of liver disease and poor outcome in these patients or not. Methods: 61 patients diagnosed to have SBP were enrolled. In addition to routine laboratory investigation, Child Pugh and APACHE II scores were calculated for all patients. Degree of renal impairment was defined using Acute Kidney Injury Network (AKIN) criteria. Stroke volume (SV) was measured by M mode and 2-dimensional Doppler echocardiography. Cardiac output (CO) and Systemic vascular resistance (SVR) were also calculated. All data were statistically analyzed. Results: SV, CO, SVR were significantly correlated with Child Pugh score, p value 0.001, respectively. In 46 (75.4%) patients who had AKI, degree of kidney injury using AKIN criteria is significantly correlated with SVR, SV, and CO. Inpatient mortality occurred in 10 (16.4%) patients. Non-survivors had lower SVR and higher SV and CO compared to survivors (1805 ± 105 dyn·sec/cm2, 80.3 ± 11.2 ml, and 6.7 ± 1.1 L/min vs 1936 ± 504, 67.6 ± 12.7, and 5.7 ± 0.9, p value 0.029, 0.005, 0.007 respectively). Using the Receiver Operating Characteristics (ROC) curve, the cut-off value of CO that predicts mortality was 5.89 L/min with sensitivity and specificity measuring 80%, 70.6% respectively. The area under the curve (AUC) is 0.767 with 95% confidence interval (CI) is 0.566 to 0.968, p value 0.008. At this value, the odds ratio (OR) is 7.33 with p value 0.012. Conclusion: Degree of hyper-dynamic circulation is significantly correlated with the severity of liver disease and predicts poor outcome in SBP patients.

硅酸钙的加速碳化过程是非水硬性工业固废利用、二氧化碳的捕集与封存以及新型低钙硅酸盐水泥研究的共性关键问题。综述了时间、温度、CO2气体分压及水固比等对碳化速度的影响,统计分析了这些影响因素的最佳范围,并对硅酸钙碳化过程中的化学计量关系、碳化反应及碳化产物的类型进行了总结,提出了硅酸钙类矿物碳化反应模型。

Design and evaluation of PV-wind hybrid system with hydroelectric pumped storage on the National Power System of Egypt

National and international policies encourage increased penetration of solar and wind energy into electrical networks in order to reduce greenhouse gas emission. Solar radiation and wind speed variations complicate the integration of wind and solar generation into power systems and delay the transition of these sources from centralized to distributed energy sources. The increased penetration of nontraditional energy sources into the electric grid stimulates the demand for large capacities in the field of energy storage. A mathematical model, which describes the operation of a proposed hybrid system, including solar PV, wind energy, and a pumped storage hydroelectric power plant is developed in this paper. This hydropower plant utilizes seawater as a lower reservoir, and only a tank has to be built in order to reduce the installation cost of the storing system. The pumped storage power plant used for compensation of the variation of the output energy from the PV and wind power plants by discharging water from the upper reservoir, which is previously pumped in the case of surplus energy from PV and wind turbine power plants. The impact of the proposed system on the grid utility is investigated in accordance with the values of energy exchange(deficits and surpluses of energy) between the considered hybrid system and the grid. The optimum design is determined by the pump and turbine capacities, upper reservoir volume, and the volume of water left in the tank for emergencies. Different scenarios of the optimum operations are presented for analysis. The results obtained from the examined scenarios indicate the ability of such a hybrid energy system to reduce the exchange of energy with the grid. This paper indicates the technical feasibility of seawater pumpedstorage hydropower plant for increasing the Egyptian national grid’s ability to accept high integration of renewable energy sources.

Clinical and Radiological Localization of Language Dysfunctions in Acute Ischemic Stroke

Objective: To study the effect of cerebral ischemic vascular insult on the localization of language areas as regard to automatic speech, initiation, and mutism. Materials and Methods: Seventy ischemic stroke patients were selected within the first 48 h of the onset of stroke with left hemispheric stroke with aphasia. Inclusion criteria: any age above seven years, both sexes, first ever clinical stroke, admitted within the first 2 days of stroke insult. Patients underwent careful history taking, neurological examination, routine laboratory investigations, echo cardiography, and carotid duplex and language assessments. Results: Automatic speech disorders were mainly seen along left insular, left supramarginal and left frontal inferior orbital lesions. Initiation difficulty is seen mainly along as left insular, left temporal superior and left periventricular lesions. Mutism is seen along left external capsule, left internal capsule, left pallidum, left frontal inferior orbital and left inferior trigone lesions. Conclusions: Deep periventricular white matter and insular area in the dominant hemisphere play an important role in many language tasks. Thus aphasia is not a mere cortical function.

Introduction: Reperfusion injury of spinal cord “white cord syndrome” refers to acute neurological deterioration after decompressive spinal surgery without any reported intraoperative events. The postoperative appearance of diffuse hyperintense intramedullary areas in T2-weighted MRI is characteristic without any other pathological or compressive signs. The etiology may be due to sudden expansion of the cord after reperfusion of chronically ischemic areas leading to disruption of blood-spinal cord barrier. The incidence and risk factors of the syndrome are not well documented in the literature. Methods: This is a retrospective case series that included 150 patients with cervical spondylotic myelopathy who had undergone decompression surgery in our institution from 2017 to 2019. Patients’ demographics, clinical, imaging characteristics, intraoperative and postoperative data were collected and analysed. Results: In this study, we documented 7 cases of this syndrome. We found that advanced age, chronic hypertension, pre-existing cord signal in MRI and long history of symptoms may be risk factors for this syndrome. Conclusion: Early diagnosis and prompt treatment of this syndrome with steroids and physical therapy may lead to initial improvement. Awareness of the surgeons about this syndrome should be increased and identifying its risk factors is very important for patients counseling.
