Impact of cigarette smoking on response to interferon therapy in chronic hepatitis C Egyptian patients

AIM:Smoking may affect adversely the response rate to interferon-α.Our objective was to verify this issue among chronic hepatitis C patients. METHODS:Over the year 1998,138 chronic hepatitis C male Egyptian patients presenting to Cairo Liver Center, were divided on the basis of smoking habit into:group I which comprised 38 smoker patients(>30 cigarettes/d) and group Ⅱ which included 84 non-smoker patients. Irregular and mild smokers(16 patients)were excluded. Non eligible patients for interferon-α therapy were excluded from the study and comprised 3/38(normal ALT)in group I and 22/84 in group Ⅱ(normal ALT,advanced cirrhosis and thrombocytopenia).Group I was randomly allocated into 2 sub-groups:group Ia comprised 18 patients who were subjected to therapeutic phlebotomy while sub-group Ib consisted of 17 patients who had no *** sub-group la,3 patients with normal ALT after repeated phlebotomies were excluded from the ***-α 2b 3 MU/TIW was given for 6 mo to 15 patients in group Ia,17 patients in group Ib and 62 patients in group Ⅱ. Biochemical,virological end-of-treatment and sustained responses were evaluated. RESULTS:At the end of interferon-α treatment,ALT was normalized in 3/15 patients(20%)in group Ia and 2/17 patients(11.8%)in group Ib compared to17/62 patients (27.4%)in group Ⅱ(P=0.1).Whereas 2/15 patients(13.3%) in group *** 2/17 patients(11.8%)in group Ib lost viraemia compared to 13/62 patients(26%)in group Ⅱ (P=0.3).Six months later,ALT was persistently normal in 2/15 patients(13.3%)in group 1a and 1/17 patients (5.9%)in group Ib compared to 9/62 patients(14.5%)in group Ⅱ(P=0.47).Viraemia was eliminated in 1/15 patients (6.7%)in group Ia and 1/17 patients(5.9%)in group Ib compared to 7/62 patients(11.3%) in group Ⅱ,but the results did not mount to statistical significance(P=0.4). Sustainable mitigation of the continuously rising concentration of NO contaminants is among the most urgent issues of this *** electrocatalytic conversion of NO into useful NH_(3)offers an attractive path toward achieving sustainable NO abatement and NH_(3)production ***,its efficiency is challenged by the intense competition from hydrogen evolution reaction and relatively high energy barriers of NO *** is thus highly desirable to explore active electrocatalyst for NO reduction reaction and investigate the mechanisms on relevant ***,we introduce an FeP nanorod array on carbon cloth as a high-efficiency catalyst for NO electroreduction to *** 0.2 M phosphate-buffered solution,this catalyst exhibits a low onset potential of-0.014 ***,it achieves a remarkable Faradaic efficiency of 88.49%and a large NH_(3)yield of 85.62μmol·h^(-1)cm^(-2),with durability for stable NO conversion over 12 h of *** catalytic mechanism on FeP is investigated further by theoretical calculations.

为了实现虚拟制造环境中智能高效的虚拟手,改善目前主流的依赖对用户手抓握动作跟踪的状况,提出一种适用于虚拟制造环境的虚拟手自主行为构造方法.通过在虚拟手结构模型上增加感知环设计,构建虚拟手的感知能力;结合文中提出的虚拟手三阶段感知算法实现虚拟手的自适应行为机制,极大地减轻了操作者的认知和操作负载;通过构造虚拟手手势集和手势状态转换模型,实现了虚拟手在复杂零件表面的连续抓握操作.文中还重点研究了复杂零件表面虚拟手的连续抓握的行为机制和相关算法.实验结果表明,文中的虚拟手具有很好的几何真实感和行为真实感;提出的方法具有良好的可用性、可靠性和实时性.

Molecular mechanisms of the anti-obesity potential effect of Moringa oleifera in the experimental model

Objective: To elucidate the molecular mechanisms of the potent anti-obesity effect of Moringa oleifera Lam.(M. oleifera) ethanolic extract and to clarify the link between these mechanisms and the associated metabolic and vascular risks in the experimental model of visceral ***: M. oleifera ethanolic extract was orally administered at 600 mg/kg body weight in obese female rats daily for 12 weeks. At the end of treatment, body weight was determined, and the atherogenic index, coronary artery index, glucose level, insulin resistance status, liver and kidney functions were assessed. Also, the m RNA of leptin, adiponectin and resistin in visceral adipose tissue was determined by quantitative real ***: The results showed that M. oleifera extract down-regulated m RNA expression of leptin and resistin, while it up-regulated adiponectin gene expression in obese rats relative to untreated obese control counterparts. This amelioration of genes expression was paralleled by a reduction in body weight and improvement of the atherogenic index and coronary artery index, as well as glucose level and insulin resistance value without adverse effects on liver or kidney functions, versus the untreated obese control ***: It is reasonable to assume that the anti-obesity, anti-atherogenic and antidiabetic properties of M. oleifera are mechanistically achieved via working directly on the adipokines of the visceral adipose tissue. Therefore, M. oleifera may be a good therapeutic candidate for the symptoms of metabolic syndrome.

Microstructure, Corrosion, and Fatigue Properties of Alumina-Titania Nanostructured Coatings

Air Plasma spray process was used to deposit a conventional and nanostructured Al2O3-13 wt% TiO2 coatings on a stainless steel substrates. Morphology of the powder particles, microstructure and phase composition of the coatings were characterized by XRD and SEM. Potentiodynamic polarization tests and Electrochemical Impedance Spectro- scopy (EIS) were used to analyze the corrosion of the coated substrate in 3.5% NaCl solutions to determine the opti-mum conditions for corrosion protection. The fatigue strength and hardness of the coatings were investigated. The experimental data indicated that the nanostructured coated samples exhibited higher hardness and fatigue strength compared to the conventional coated samples. On the other hand, the conventional coatings showed a better localized corrosion resistance than the nanostructured coatings.

Anti-CD-20 Therapy in Refractory Adult Still’s Disease

Adult Still’s disease is a relatively rare form of rheumatoid arthritis with systemic inflammatory features. The prevalence is around 1.5 cases per 100,000 - 1000,000. In the current case we display a 30-year-old male patient with refractory adult still’s disease who suffered recurrent attacks of fever 39.5°C, arthritis in proximal interphalangeal joints (PIPs), wrists, tempromandibular joints (TMJs), knees and ankles, stitching chest pain, dyspnea, erythematous rash over the trunk, sore throat, weight loss (15 Kilograms in 4 months). The patients’ disease remained uncontrolled despite of synthetic disease modifying anti-rheumatic drugs and repeated intramuscular corticosteroid injections. Laboratory workup revealed erythrocyte sedimentation rate (ESR) of 95, C-reactive protein (CRP) of 100 mg/L, hemoglobin 10.5 gm%, leukocytosis 12,000/microlitre, mild elevation of liver function tests and dyslipidemia. Serology revealed negative rheumatoid factor, anti-nuclear antibody titre of 1:80, elevated serum ferritin 4000 micrograms/litre. The patient was started on rituximab (375 mg/m2), prednisolone 20 mg/day and selective Cox-2 inhibitor. Follow up for over three months following the completion of his pulse therapy, revealed no relapse of fever or fatigue, with morning stiffness of 5 - 10 minutes, VAS of 3, DAS28 of 3.8, HAQDI of 0.62, ESR 23, CRP 4.9, Hb 12.5 gm%, leucocytic count 9000/microlitre, the dose of prednisolone was successfully reduced to a dose of 5 mg/day orally. Conclusion: Anti-CD20 therapy successfully controlled systemic and articular refractory disease with sustained efficacy over a follow up period of up to 24 weeks.

Increasing global water shortages are accelerating the pace of membrane manufacturing,which generates many environmentally harmful *** challenges need a radical rethink of developing innovative membranes that can address freshwater production without generating environmentally harmful *** work utilized the synthesized ultra-long hydroxyapatite(UHA)by the solvothermal method using the green solvent oleic acid in preparing UHA-based forward osmosis *** membranes were developed using different loading ratios of graphene oxide(GO)by vacuum-assisted filtration *** prepared GO/UHA membranes were identified using X-ray diffraction,scanning electron microscope,Fourier-transform infrared spectroscopy,and X-ray photoelectron *** contact angle and pore size distribution were determined for the obtained GO/UHA *** obtained hierarchical porous structure in the prepared membranes with interconnected channels results in a stable water flux with reverse salt *** best water flux rate of 42±2 L·m^(–2)·h^(–1)was achieved using the 50 mg GO/UHA membrane,which is 3.3 times higher than the pristine membrane,and a reverse salt flux of 67 g·m^(-2)·h^(–1).The obtained results showed a promising capability of a new generation of sustainable inorganic-based membranes that can be utilized in freshwater generation by energy-efficient techniques such as forward osmosis.
