Objective:This study aimed to evaluate the effects of foot reflexology on fatigue and sleep quality in school-aged children receiving ***:A quasi-experimental pretest-posttest design was *** children of ages 6-12 undergoing in-center hemodialysis were *** Pittsburgh Sleep Quality Index(PSQI)and Inventory of Fatigue Symptom(IFS)scales were administered at *** then received 30 min of foot reflexology massage before hemodialysis sessions 3 days per week for 12 *** administration of the sleep and fatigue scales occurred after the intervention ***:Reflexology massage led to significant improvements in sleep quality components,including duration(0%-30%normal sleepers),efficiency(0%-50%>85%),latency(50%-0%>60 min),disturbances,and daytime *** mean PSQI score decreased from 18.2 to 9.7(P<0.05).Fatigue severity substantially decreased,with the mean IFS score improving from 105.7 to 64.1(P<0.05).Conclusion:Foot reflexology is an effective nursing intervention for reducing fatigue and improving sleep quality in children on hemodialysis,warranting integration into routine care.

In this work, the possibility of enhanced activity during the hydrodesulfurization of dibenzothiophene over certain nano-MoS2 catalyst due to the presence of H2S was examined by focusing on the reaction kinetics. With H2S generated in situ, the overall reaction followed the autocatalytic rate law;while in the absence of H2S the kinetics indicated a pseudo-first-order reaction. H2S appears to modify the relative contributions of parallel hydrogenation and desulfurization reactions by drastically increasing the hydrogenation rate. Kinetic models were developed that describe the hydrodesulfurization reaction at various H2S concentrations, and the kinetic parameters and adsorption equilibrium constants associated with this process were estimated by fitting the experimental data. The results suggest that the promotion and/or inhibition of hydrodesulfurization by H2S likely result from the same overall reaction mechanism.

Most of developing countries suffer from decreasing and poor quality of drinking water which led to emergence of many dangerous diseases. In addition, there isn’t any methodology followed to predict and disinfect drinking water by using advanced software program. Maintaining water quality in the water distribution system has become a prominent issue in the study of the water network. Residual chlorine concentration is the indicator to ensure the quality of water in the water network because it eliminates contaminants in the distribution network beginning of the treatment plant down to the consumer. In collaboration with Alexandria (Egypt) Water Company, samples were taken from El-NOZHA water plant station and El-HADARA water distribution network to know the free residual chlorine. In this paper, WaterCAD software has been used to make hydraulic analysis and calibration of residual chlorine in water distribution network to know the ideal chlorine dose that should be added at the water treatment plant and to know the areas of strength and weakness in the concentration of free residual chlorine in the water distribution network. In addition different scenarios have been found to know the free residual chlorine at the weakness areas after injecting chlorine in some junctions and the impact of a fire case or breaking in the water pipe distribution network on the residual chlorine. Results showed ensuring in the water quality in the distribution network by adding chlorine dose in water less than the existing dose which has been added in the El-NOZHA water treatment plant. It is possible to maintain the percentage of free residual chlorine concentration at different locations without relying on adding chlorine only in water treatment plant by injecting low percentage of chlorine dose in the junctions.

Prophylaxis of Postoperative Acute Endopthalmitis Following Cataract Surgery

Background: Postoperative Endophthalmitis is a rare complication after intraocular surgeries. However, it remains one of the most devastating complications following cataract surgery because of its poor prognosis. We describe the effects of antibiotic prophylaxis on the incidence of postoperative endophthalmitis after cataract surgery, particularly phacoemulsification. Aim of the work: To identify risk factors of acute endophthalmitis and describe the effects of antibiotic prophylaxis on the incidence of postoperative endophthalmitis after phacoemulsification surgery. Methods: A prospective randomized cataract surgery study recruited 300 patients;150 patients who received just intracameral vigamox at the end of surgery without topical antibiotics before surgery and 150 patients who received both intracameral vigamoxas well as topical perioperative vigamox which either just half an hour before surgery or three days preceding surgery. We took a conjunctival swab before and after vigamox administration in order to detect the type of conjunctival flora in the Libyan population as well as the effect of the vigamox on the conjunctival flora by using Phoenix-BD machine. In total we collected 166 specimens (83 patients). Results: In all groups no acute post-operative endophthalmitis was reported. Conclusions: The study was based on intracameral Moxifloxacin at the end of surgery with and without topical antibiotics drops administration. The incidence of postcataract endophthalmitis was not reported in our study after using intracameral antibiotics, as it seems to be effective in preventing endophthalmitis after cataract surgery.

Oral Vinorelbine as Switch Maintenance Therapy versus Best Supportive Care in Patients with Advanced Adenocarcinoma Non-Small Cell Lung Cancer EGFR Wild Type

Background: This study was performed to evaluate the efficacy and safety of switch maintenance therapy with oral vinorelbine in advanced non-small cell lung cancer (NSCLC) with adenocarcinoma limited to epidermal growth factor receptor (EGFR) wild type. Materials and Methods: In this single randomized trial, patients with advanced stage (IIIB and IV) NSCLC with adenocarcinoma EGFR wild-type status, treated with 6 cycles of platinum based chemotherapy. Patients did not show progression after first-line chemotherapy were randomly assigned to receive switch maintenance with vinorelbine (80 mg/m2, day 1, 8) (group I) or the best supportive care until disease progression (group II). Results: The median progression free survival (PFS) was 9.7 months for group I versus 5.7 months for group II with statistically significant difference between both groups [HR = 1.15;95% CI 1.19 to 1.49;P value = 0.002], while the median overall survival (OS) was 13.2 months for group I versus 11.9 months for group II with no statistically significant differences between both groups [HR = 1.24;95% CI 1.05 to 1.46;P value = 0. 3]. The patients who received oral vinorelbine had tolerable toxicity profile. Conclusion: Switch maintenance therapy with oral vinorelbine, though improve PFS, did not improve OS in patients with NSCLC with adenocarcinoma EGFR wild type.

AIM: To evaluate the efficacy of intraoperative endoscopic retrograde cholangio-pancreatography(ERCP) combined with laparoscopic cholecystectomy(LC) forpatients with gall bladder stones(GS) and common bile duct stones(CBDS).METHODS: Patients treated for GS with CBDS were included. LC and intraoperative transcystic cholangiogram(TCC) were performed in most of the cases. Intraoperative ERCP was done for cases with proven ***: Eighty patients who had GS with CBDS were included. LC was successful in all cases. Intraoperative TCC revealed passed CBD stones in 4 cases so intraoperative ERCP was performed only in 76 patients. Intraoperative ERCP showed dilated CBD with stones in 64 cases(84.2%) where removal of stones were successful; passed stones in 6 cases(7.9%); short lower end stricture with small stones present in two cases(2.6%) which were treated by removal of stones with stent insertion; long stricture lower 1/3 CBD in one case(1.3%) which was treated by open hepaticojejunostomy; and one case(1.3%) was proved to be ampullary carcinoma and whipple's operation was scheduled. CONCLUSION: The hepatobiliary surgeon should be trained on ERCP as the third hand to expand his field of therapeutic options.

Structural Studies on Ag2O-P2O5Glasses

Silver phosphate glasses of general formula xAg2O·(100 - x)P2O5 have been investigated over compositional range from x = 40 to 62.5 mol%. The local structure around phosphorus atom has been studied via 31P nuclear magnetic resonance. The distribution of [PO4]Qn species as a function of composition has been shown to slightly deviate from the simple binary alkali phosphate model. An anomalous behavior has been recorded and interpreted in terms of mixed ring-chain effect in metaphosphate composition. The splitting of NMR spectra into sub resonances is assigned to different binding sites characterizing Q1 ring and Q1 chain structure. Higher Ag2O concentration (≥50 mol%) leads to formation of phosphate groups with specific resonance peaks which are mainly related to pyro and orthophosphate species. The rate of change of the chemical shift of the 31P NMR depends on the bond type, which in turn reflects the extent of double bonding between phosphorus and oxygen atoms. Increasing concentration of Q0 with increasing Ag2O content leads to decreasing quantities of bridging and double bonds. As a consequence, specific symmetric resonance peak of higher intensity and chemical shift (Q0) is a feature of silver rich glasses (orthophosphate). The latter species is therefore proposed to compose of separated membered rings,?which cause deshielding of phosphate units.?XRD and EDP studies have shown that, amorphous phosphate network is the dominant structure of glasses containing ≤ 55 mol% Ag2O. Some ordered and well crystallized phases are formed at higher Ag2O concentration. Increasing non-bridging oxygen atoms is shown to have the main effect on crystallization behavior. Orthophosphate composition is the most crystalline one among the other compositions (ultra, meta- and pyrophosphate). Presence of orthophosphate species which typically contains highest concentration from isolated Q0 units is the main reason for building up crystalline Ag3PO4phosphate phase.

Pathogenic Bacteria Associated with Different Public Environmental Sites in Mecca City

Background: The hygiene of environmental surfaces from shopping, ATM machines, telephones and computers and miscellaneous sites play role in spreading fecal and total coliform bacteria as well as pathogenic bacteria. Objectives: This study addresses the contaminated common sites by pathogenic or potentially pathogenic bacteria in Mecca, SA. Materials and Methods: A total 648 swab samples were collected and analyzed for presence or absence of pathogenic bacteria. Results: Of the total samples 422 were negative bacterial count (71%) and 226 (29%) were positive. All collected samples (100%) of glass windows in the fish markets were bacterial counted;most dominated was Bacillus spp. (n = 97) and the highest population of species was Enterococcus faecalis (n = 40) and E. coli (n = 16). Conclusion: Some public sites were very contaminated with different types of fecal coliform group of bacteria such as shopping cart handles, inner surfaces and child seats in supermarkets, and the glass windows in the fish market. Acinetobacter haemolyticus and other hemolytic bacteria were isolated from more than site.
