Gallbladder Disorders Detected by Ultrasound

Patients wh. h.ve disease of th. biliary tract commonly present with.acute righ. upper quadrant pain, nausea or vomiting, mid-epigastric pain, and/or jaundice. Etiologies include inflammation with.or with.ut infection, noninflammatory disorders, and benign or malignant neoplasms of th. gallbladder or bile ducts. Ultrasound (US) is now accepted as th. initial imaging modality of ch.ice for th. work-up of suspected biliary tract disease. A retrospective study was carried out at King Abdulaziz University h.spital to discuss th. protocol of ultrasound scanning in demonstrating incidence and complication of Gall-bladder (GB) path.logies. Known cases of GB path.logies (100 patients) were surveyed by ultrasound using spatial digital iU22 Ph.lips Convex probe 3.5 Mh.. All patients were evaluated with.ultrasonogph. following th. international scanning guidelines and protocols. Th. age of th. patients is between (9 - 90) years, 68 Patients (68%) were females and 32 patients (32%) were males. Range of age group of accumulation for gallstone presence was (35 - 50) years in females and above 50 years in males. Incidence of gallbladder path.logies are 59% (female 46% and 13% male). Incidence of gallstone is 37%, and ratio of incidence is between male to female 1:3. Oth.r path.logies of gallbladder were found to be acute ch.lecystities 12%, ch.onic ch.locystities 5%, sludge 2%, carcinoma of th. Gall-bladder 1%, Gall-bladder polyps 1% and Emph.sematous ch.leycystities 1%. Ultransonograph. is a single imaging modality sufficient for evaluation of patient with.suspected gallbladder path.logies (gallstone) wh.ch.can provide information about th. presence of gallstone and more over about site and cause of biliary tract obstruction. Ultrasound is h.gh.y sensitive and specific means for diagnosis of th. gallbladder disorders.

Th. Diagnostic Value of Color Doppler Ultrasound and Grey Scale Sonograph. in Predicting th. Malignancy of Th.roid Nodules

Retrospective study was done at KAUh.to determine th. sonograph.c ch.racteristics in predicting th. malignancy of th.roid nodules. Th. sample was 120 patients consisting of 70 benign nodules and 50 malignant nodules. Out of 120 patients, 106 (88.3%) female and 14 (11.7%) male patients. Out of 106 female patients, 64 (91.45) h.d benign and 42 h.d malignant th.roid nodules. Out of 14 (11.7) male patients, 6 h.d benign and 8 h.d malignant th.roid nodules. Th. youngest patient in our study was 31 years and th. oldest patient was 76 years. Th. common range of patients age was ≥ 60 years and consisting of twenty (28.6%) benign th.roid nodules and fourteen (28%) malignant th.roid nodules. Th. majority of patients’ age group with.malignant th.roid nodules were between 50 - 60 years and consisting of twenty (40%) malignant nodules. In our study, according to th. result of gray scale and color Doppler ultrasonograph., h.po-ech.genicity, solid ech. structure, micro-calcification and intra-nodular vascularity were th. significant ch.racteristics of malignancy in nodules with.sensitivity (72%, 74%, 48% and 88% respectively), specificity (66%, 63%, 94% and 69% respectively), PPV (60%, 59%, 86% and 67% respectively) and NPV (77%, 77%, 72% and 89% respectively).

Real Time Ultrasound Elastograph. for th. Differentiation of Benign and Malignant Th.roid Nodules

Th. purpose of th.s study was to evaluate th. predictive value of real-time ultrasound elastograph. (RUE) in differentiating benign from malignant th.roid nodules, with.fine needle aspiration cytology analysis as th. reference standard. A total of 78 consecutive patients with.th.roid nodules wh. were selected for fine needle aspiration biopsies were examined in th.s prospective study. 78 nodules in th.se patients were examined by conventional ultrasound, and ultrasound elastograph.. Th. final diagnosis was obtained from cytological findings. Tissue stiffness on ultrasound elastograph. was scored from 1 (low stiffness over th. entire nodule) to 5 (h.gh.stiffness over th. entire nodule and surrounding tissue). On real-time ultrasound elastograph., 47 of 62 benign nodules (76%) h.d a score of 1 or 2, wh.reas 15 of 16 malignant nodules h.d a score of 3 to 5, with.sensitivity of 93.7%, specificity of 90%, a positive predictive value of 71%, and a negative predictive value of 98%. Real-time ultrasound elastograph. is a promising imaging tech.ique th.t is useful in th. differential diagnosis of th.roid cancer. Utilization of ultrasound elastograph. could reduce th. rate of th.roid biopsies because of its h.gh.elasticity being h.gh.y associated with.benign cytology.

Th. acute abdomen is one of th. most frequent causes for presentation to th. emergency department. Imaging plays an important role for an accurate diagnosis, wh.ch.in turn diminish.s morbidity and mortality. Th. aim of th.s study was to demonstrate optimum CT aspects and emph.size on th. important features of CT for th.se patients presenting with.an acute abdominal pain at th. Emergency Department both.in general and in a number of selected conditions (appendicitis, small-bowel obstruction, acute pancreatitis, and diverticulitis). Th. reported data by th.s study are based on th. auth.r working experience, wh.ch.forms a continuous protocol adjustment process. Th. present study provides evidence th.t CT would result in definite diagnosis of patients with.abdominal pain in terms of th. detection of some urgent conditions.
