Prospective controlled study on the effects of polyethylene glycol in capsule endoscopy

A pancreatic paraganglioma is a rare neoplasm that is difficult to distinguish from a pancreatic neuroendocrine tumour. Here we present a case of pancreatic paraganglioma that was surgically resected following preoperative diagnosis of a pancreatic neuroendocrine tumour. Careful evaluation of the endoscopic ultrasonography findings revealed abundant draining ves-sels,which could have led to a correct preoperative diagnosis of pancreatic paraganglioma.

Contrast-enhanced harmonic endoscopic ultrasonography for assessment of lymph node metastases in pancreatobiliary carcinoma

AIM: To assess the usefulness of contrast-enhanced harmonic endoscopic ultrasonography(CH-EUS) for lymph node metastasis in pancreatobiliary ***: All patients suspected of pancreatobiliary carcinoma with visible lymph nodes after standard EUS between June, 2009 and January, 2012 were *** the primary analysis, patients with successful EUSfine needle aspiration(FNA) were included. The lymph nodes were assessed by several standard EUS variables(short and long axis lengths, shape, edge characteristic and echogenicity), color Doppler EUS variable [central intranodal blood vessel(CIV) presence] and CH-EUS variable(heterogeneous/homogeneous enhancement patterns). The diagnostic accuracy relative to EUSFNA was calculated. In the second analysis, N-stage diagnostic accuracy of CH-EUS was compared with EUS-FNA in patients who underwent surgical ***: One hundred and nine patients(143 lymph nodes) fulfilled the criteria. The short axis cutoff ≥ 13 mm predicted malignancy with a sensitivity and specificity of 72% and 85%, respectively. These values were 72% and 63% for the long axis cut-off ≥ 20 mm, 62% and 75% for the round shape variable, 81% and 30% for the sharp edge variable, 66% and 61% for the hypoechogenicity variable, 70% and 72% for the CIV-absent variable, and 83% and 91% for the heterogeneous CH-EUS-enhancement variable, respectively. CH-EUS was more accurate than standard and color Doppler EUS, except the short axis cut-off. Notably, three patients excluded because of EUS-FNA failure were correctly N-staged by ***: CH-EUS complements standard and color Doppler EUS and EUS-FNA for assessment of lymph node metastases.

Traditional Food Culture (Local Cuisines, Japanese Sake) That Has Been Nurtured by the Rich Nature of the Prefecture, Japan

In chiba Prefecture, Japan, during the Edo period (1603-1867), the development of waterway traffic by ships and the management of ports, highways and post towns around the ports progressed with the prosperity of the Edo (present-day Tokyo), which became heavily populated and the center of politics. We estimated that the demand of Japanese sake, which is luxury grocery item, was high. The freshwater layer that is abundant in mineral water to a depth of approximately 10 m is formed in coastal sand dunes. The fresh water layer is hard water, in which the concentrations of minerals such as calcium and magnesium are high. When the fresh water layer is used as the preparation water, the working rice malt and yeast in the sake brewing process become active. Japanese sake trends to be dry with a full-bodied taste. In addition, the main ingredients of local cuisines are fish and shellfish;many local cuisines are seasoned using soy sauce, miso and salt, and these local cuisines pair well with the type of Japanese sake described above. The local cuisines have been nurtured in harmony with the region’s rich nature and heritage. In the future, we need to conserve the rich natural environment of the tidal flat, coast, seaweed beds, and marine, which have been producing the main local cuisine in chiba Prefecture, and the water source area (a successive environment on the plateau from the coast, which was previously called the coastal dune area) of the preparation water for making Japanese sake. We also need to proactively develop local production for local consumption activities. Thus, we hypothesize that if the Japanese food life is secured and the traditional food culture is continued, the region will become revitalized by the development of the exchanges in the region.

Ecosystem services of coastal sand dunes saw from the aspect of Sake breweries in chiba Prefecture, Japan: A comparison of coastal and inland areas

In this report, we reviewed the differences in the ecosystem services of coastal and inland areas surrounding 39 Sake breweries in chiba Prefecture by investigating environmental variables (e.g., location, altitude, soil, and hardness of preparation water). The Sake breweries were located in three distinct environments: the coastal vicinity, the river plains region, and the plateau/ mountainous region. The hardness of the preparation water and the soil types in the coastal vicinity were compared with those of the river plains and the plateau/mountainous region. Strong hard and hard water sources were observed in 70% or more of the breweries in the coastal vicinity, and sand dune regosol, coarse particle brown lowland soil, and coarse particle grey soil were more prevalent along the coast than inland. Most of the Sake brewery wells in the coastal vicinity were approximately 5 - 10 munderground, and there were no great differences in the number of Sake breweries in each well depth class in the river plains and the plateau/mountains region. We analysed environmental factors (distance from the sea, soil type, water hardness and preparation water collection depth) using a principal component analysis. This analysis revealed the existence of three main environments: the coastal vicinity, the river plains and the plateau/mountainous region. We conclude that the decrease in altitude between the inland Sake breweries and those along the coast is accompanied by a corresponding increase in the hardness of preparation water (from soft water to strong hard or hard water) and shifts in soil composition from gley soil, grey lowland soil, brown forest soil, and andosol to sand dune regosol, coarse particle brown lowland soil, and coarse particle grey soil.

Feasible Method to Assess the Performance of a Lung Cancer CT Screening CAD System in Clinical Practice: Dependence on Nodule Size and Density

Detection of small pulmonary nodules is the goal of lung cancer screening. Computer-aided detection (CAD) systems are recommended to use in lung cancer computed tomography (CT) screening to increase the accuracy of nodule detection. Size and density of lung nodules are primary factors in determining the risk of malignancy. Therefore, purpose of this study is to apply computer-simulated virtual nodules based on the point spread function (PSF) measured in same scanner (maintaining spatial resolution condition) to assess the CAD system performance dependence on nodule size and density. Virtual nodules with density differences between lung background and nodule density (ΔCT) values (200, 300 and 400 HU) and different sizes (4 to 8 mm) were generated and fused on clinical images. CAD detection was performed and free-response receiver operating characteristic (FROC) curves were obtained. Results show that both density and size of virtual nodules can affect detection efficiency. Detailed results are possible to use for quantitative analysis of a CAD system performance. This study suggests that PSF-based virtual nodules could be effectively used to assess the lung cancer CT screening CAD system performance dependence on nodule size and density.

A Case of Male Anorgasmia with Cerebral Infarction

Objective: The deficit of male orgasm is an unusual clinical condition. This is a case report on secondary male anorgasmia with cerebral infarction. Material and Method: The patient was a 55-year-old man who had suffered from male anorgasmia, acquired type. The patient had a normal erectile response, but ejaculated without any sensation of orgasm. The patient underwent an evaluation by a psychiatrist and an urologist trained in sexual medicine. Result: The results of physical, psychological, and laboratory examination were normal. Brain MRI revealed cerebral infarction at bilateral parietal lobes and frontal lobes. Conclusion: There is a possibility that these brain regions are related to male orgasmic dysfunction.
