Ecthyma Gangrenosum about a Case at the Centre Hospitalier Roi Baudoin de Guédiawaye in Senegal

Background and Aim: Ecthyma gangrenosum (EG) is a rare necrotic skin infection caused by Pseudomonas aeruginosa occurring on healthy skin mainly, but not exclusively, in immunocompromised patients. We report the case of EG of unusual presentation in a 12-month-old infant. Case Presentation: This was a 12-month-old male infant who was referred from a peripheral structure for the treatment of facial swelling associated with fever. In his antenatal and intra nasal history, there was a notion of hospitalization for 15 days for early neonatal infection and a delay in psychomotor development. The admission examination showed the fair general condition, hyperthermia at 39°C, tachycardia at 136 beats/minute, and polypnea at 48 cycles/minute. The weight-for-height index with a z-score of 3DS was also found. The dermatological examination at the entrance found an extensive brownish inflammatory tense infiltrated plaque taking the mandibular, submental, retro-auricular occipital regions rapidly evolving towards necrosis, a significant swelling predominant on the upper right eyelid preventing the opening of the associated eyes to mouth ulceration. Biological abnormalities were predominantly neutrophilic leukocytosis, elevated CRP, and elevated muscle enzymes. Microbiological examination of skin samples had isolated Pseudomonas aeruginosa. He had received antibiotic therapy, and a necrosectomy under general anesthesia. The evolution was favorable and he was discharged from the hospital on D26 of hospitalization. Conclusion: This observation is rare due to its location, its early onset, and non-neutropenic background.

The Importance of Uterine Tumors in the Day-to-Day Practice of the General Surgery Department of Ignace Deen National Hospital

Introduction: Uterine tumors are all abnormal cell proliferations developed at the expense of one or more tissue types, which may be located in any uterine segment and have anatomopathological characteristics of benignity or malignancy. The aim of this study was to report on the management of uterine tumors in the general surgery department of Ignace Deen Hospital in Conakry. Methodology: This was a retrospective study lasting five (5) years, from January 1, 2011 to December 31, 2015: All complete records of patients with the diagnosis of a uterine tumor managed in the department were included. Our results are presented in tables and figures. Results: 3200 patients underwent surgery. Among them, 82 cases concerned uterine tumors, i.e. 2% of the department’s overall activity. The average age of our patients was 38.5 years, with extremes of 18 and 59 years. The age group most affected was 41 - 50, with a rate of 39.02%. Housewives and married civil servants were the socio-professional strata most affected, with a predominance of married women. We estimated an increasing proportion of patients admitted to the department during the study period, proving that our study site plays a significant role in the management of uterine tumors. Conclusion: The management of uterine tumors is a major public health problem. Information, communication and education of all socio-professional groups seem necessary.

Vulvectomy: Indications and Results in the General Surgery Department of the Ignace Deen Chu Hospital in Conakry

Introduction: The aim of this study was to report the indications and evaluate the results of vulvectomy in the general surgery department of Ignace Deen Hospital, CHU Conakry. Materials and Methods: This was a retrospective descriptive study, conducted over a period of five (05) years from January 1, 2018 to December 31, 2022, in the general surgery department of Ignace Deen Hospital, CHU Conakry. We included in our study all patient records in whom vulvectomy was performed. Results: We recorded 15 cases of vulvectomy out of 453 perineal surgeries, i.e. 3.31%, with a mean age of 43.56 years and extremes of 35 and 69 years. Vulvar cancer was the most common diagnosis (46.67%), followed by Buschke-Lowenstein (33.33%) and anal canal cancer extending to the vulva (20%). Six patients had undergone biopsy (40%). Vulvectomy with lymph node dissection was performed in only 9 patients (60%), and all surgical specimens were sent to anatomical pathology (100%). Conclusion: Vulvectomy is a surgical technique most often indicated for the treatment of vulvar cancer.

Epidemiological, Clinical and Therapeutic Aspects of Arterial Hypertension at the General Medicine Department of Nzerekore Regional Hospital

Introduction: High blood pressure is a public health problem in Guinea as well as everywhere in Africa. The objective of our study was to assess the epidemiological, clinical and therapeutic aspects of High blood pressure in the general medicine department of the Nzérékoré regional hospital. Methodology:This was a prospective study of a descriptive type, lasting 06 months, from July 1 to December 31, 2014. Results: We recorded 156 cases of High blood pressure among a total of 570 patients, for a hospital frequency of 27%. The 56 - 65 age group was the most affected, at 29.4%, with a predominance of men (sex ratio: 1.5). Alcoholism was the most common cardiovascular risk factor with a frequency of 51.2%, smoking and physical inactivity were found in 35.8% and 30.7% respectively;psychosocial stress was found in 24.3% of patients. The clinic was dominated by headache (100%) and dizziness (57.6%). All our patients were subjected to a healthy diet. Fixed dual therapy was the most widely used treatment regimen, i.e. 91 patients (58.3%) and included a diuretic in 37.1% of cases, a calcium channel blocker in 25% of cases and an ACE inhibitor in 24.3%. Conclusion: This study revealed a high frequency of hypertension in the general medicine department of the regional hospital of N’zérékoré.

Sustainable Management of Biodegradable Waste in Guinea: Effect of Compost on Eggplant and Chilli Crop Yields Pepper

The fight against insalubrity in large urban and peri-urban agglomerations is a major challenge in developing countries. This problem is compounded by that of sustainable waste management mechanisms. Indeed, the current waste collection system in Guinea has proved inadequate, as moving garbage from point “A” to point “B” is tantamount to “moving the problem”. The aim of this experimental work is to demonstrate the cost-effectiveness and benefits of sustainable waste management. As part of this drive to valorize biodegradable waste, the Waste Management Research Center has undertaken a series of activities ranging from composting organic waste to testing compost on certain crop varieties. An experimental field of 8024 m2 was laid out and treated with 1500 Kg of fine compost in doses ranging from 2.5 to 5 T/ha. Two crop varieties, eggplant and chili, were tested. Compost application increased production yields: 15 to 21 tonnes of eggplant and 10.4 to 11.1 tonnes of chili per hectare. Growth rates compared with usual yields varied from 50% to 64% and from 11% to 17% for eggplant and chili, respectively. This study resulted in an optimum compost dose of 2.5 T/ha for this phase.

Prevalence and Contributing Factors of Orthostatic Hypotension in the Cardiology Department of the CHU Ignace Deen in Conakry

Introduction: Orthostatic Hypotension (OH) is a frequent situation during consultation in hypertensive patients. The prevalence in the general population increases with age and it is recommended to systematically search for it in patients with multiple comorbidities. The objective of this study was to determine the prevalence of orthostatic hypotension;describe their socio- demographic profile, the various clinical and paraclinical aspects and the predisposing factors of orthostatic hypotension in treated hypertensives, controlled or not. Method: This was a descriptive cross-sectional study, from January 2 to June 30, 2022 in the cardiology department of the Ignace Deen National Hospital. Included in this study were all patients who presented with orthostatic hypotension under antihypertensive treatment, regardless of age and sex, and who agreed to participate in the study. The non-inclusion criteria were hypertensive patients without OH and those who had not agreed to participate in the study. Each patient had blood pressure and heart rate measured in the supine position at room temperature and with an empty bladder. Then the measurement is taken again three (3) minutes after the switch to orthostatism. We retained the diagnosis of OH if the SBP drops by at least 20 mmHg and/or the PAD by at least 10 mmHg three (3) min after the transition to orthostatism. Our data obtained were analyzed in the Epi-info 7.4.0 software. Results: During our study period, we investigated 385 presented with OH, 12.2%. The mean age of the patients was 60.83 years ± 10.01 years and hypertensive patients, 47 of whom the M/F sex ratio was 0.81. In our study, blood pressure was not controlled in 36.2% of our patients with a predominance of grade 3 hypertension, a rate of 55.32%. Renal failure, anemia, heart failure and stroke were the main comorbidities associated with the occurrence of HO. Ebstein disease is a rare congenital malformation whose clinical presentation differs according to the anatomical form and age of the patient. In adults, it presents mainly in the form of right or global heart failure or rhythm disorders. Survival is exceptionally long in some forms. We report the case of a 70-year-old patient with Ebstein’s disease in right heart failure.

Introduction: Cystic lymphangiomas are rare benign malformative tumors of the lymphatic system of obscure etiopathogenesis. The cervico-facial location remains the most common (75%). Although benign, these tumors remain potentially fatal, due to possible compression of the upper aero-digestive tract. The aim of this work is to study the epidemiological, diagnostic and therapeutic characteristics of cervico-mandibular congenital cystic lymphangiomas in the pediatric surgery department of the Donka National Hospital (HND) Conakry. Patients and methods: This is a retrospective and descriptive study of 13 files lasting 7 years from January 2015 to December 31, 2021. The files of children whose age is less than or equal to 15 years operated on cervical tumor with histological evidence of cystic lymphangioma were retained. The data were analyzed using SPSS statistical software 21 and anonymously. Results: The incidence of this study was 1.86 cases per year and a sex ratio of 0.62 in favor of girls. The average age was 8 months 19 days. In the antecedents, we only find poorly monitored pregnancies. The average size of the tumors was 11.85 cm. Cervical ultrasound and standard x-ray of the cervical mass were the only examinations performed. Total surgical excision of the cervical tumor was performed in all patients. The mass was polycystic on exploration. The histological examination of the surgical specimens was in favor of a cystic lymphangioma. The surgical consequences were simple in 11 patients (84.62%) and complicated by parietal suppuration in 2 cases (15.38%). There were no cases of recurrence after one year of follow-up. Conclusion: Cervico-mandibular cystic lymphangiomas are the most frequent locations of congenital lymphangiomas in children. Their severity is linked to the risk of compression of the aero-digestive tracts. Their diagnosis must be confirmed by the histology of the surgical specimen. Face of the Myocardial Infarction and Diabetes: Epidemiological and Clinical Aspects in the Cardiology Department of the University Hospital Ignace Deen about 46 Cases

The purpose of this study was to describe myocardial infarction in diabetics at the Cardiology Department Ignace Deen. Introduction: New international recommendations defined myocardial infarction, any coronary syndrome accompanied by a modification of cardiac troponin even minimal Tc or Ic [1]. Then for diabetes is defined by the World Health Organization (WHO) a blood glucose greater than 1.10 g/l and/or capillary blood glucose greater than 2.26 g/l after eight hours of fasting [2].?Patients and Methods: Were eligible for both male and female patients at any age admitted to cardiology for myocardial infarction and receiving an ECG echocardiogram;A biological evaluation including blood glucose and troponin Tc or Ic. Discussion: The risk of myocardial infarction is clearly increased in diabetics and is a determinant of coronary heart disease severity and a potential test for other cardiovascular risk factors at the Ignatius Deen Cardiac Department with a frequency of 16.08%. Our result is lower than that of DIOUMM et al. in the Fanh cardiology department of Dakar, Senegal [6], which should reduce the frequency of 22.3%. The high frequency of this pathology in the study DIOUMM is related to the size of their sample on the one hand and secondly the duration of the study, the service capacity on the other hand that could be explained. Conclusion: Diabetes mellitus is a scary medium and long-term disease that causes complications. In addition to infectious and metabolic complications, heart and coronary heart attacks are becoming more common. The tare is dominated by atherosclerosis, which must be prevented through the analysis of diabetic responsibility, with cardiovascular control of risk factors and monitoring of myocardial ischemia, and good cooperation between diabetes specialists, cardiologists and patients.

Jugular Venous Thrombosis, a Rare Location of the Embolic Thrombo Venous Disease about a Case in Guinea

Observation: This patient was a 40-year-old housewife with dysphonia, physical asthenia, palpitations, fever and cervical tumefaction that had been going on for 2 months, no known cardiovascular risk factor, such as medical history, ischemic stroke. Heart sounds were regular at 110 bpm, blood pressure = 120/80 mmhg, to the lungs there are sibilant rattles. Elsewhere, there is a painful left lateral cervical tumefaction febrile to the touch. Temperature = 38°C. The rest of the exam is peculiar. Conclusion: Jugular vein thrombosis is a rare variety of unusual localization of venous thromboembolism. It must be suspected in the presence of a painful cervical swelling and confirmed by magnetic resonance imaging or to scan with contrast or ultrasound. Anticoagulant therapy should be instituted as soon as possible to avoid the formidable complication of pulmonary embolism.
