Small for size syndrome difficult dilemma: Lessons from 10 years single centre experience in living donor liver transplantation

AIM To analyze the incidence, risk factors, prevention, treatment and outcome of small for size syndrome(SFSS) after living donor liver transplantation(LDLT). METhODS Through-out more than 10 years: During the period from April 2003 to the end of 2013, 174 adult-to-adults LDLT(A-ALDLT) had been performed at National Liver Institute, Menoufiya University, Shibin Elkoom, Egypt. We collected the data of those patients to do this cohort study that is a single-institution retrospective analysis of a prospectively collected database analyzing the incidence, risk factors, prevention, treatment and outcome of SFSS in a period started from the end of 2013 to the end of 2015. The median period of follow-up reached 40.50 m, range(0-144 m). RESULTS SFSS was diagnosed in 20(11.5%) of our recipients. While extra-small graft [small for size graft(SFSG)], portal hypertension, steatosis and left lobe graft were significant predictors of SFSS in univariate analysis(P = 0.00, 0.04, 0.03, and 0.00 respectively); graft size was the only independent predictor of SFSS on multivariate analysis(P = 0.03). On the other hand, there was lower incidence of SFSS in patients with SFSG who underwent splenectomy [4/10(40%) SFSS vs 3/7(42.9%) no SFSS] but without statistical significance, however, there was none significant lower incidence of the syndrome in patients with right lobe(RL) graft when drainage of the right anterior and/or posterior liver sectors by middle hepatic vein, V5, V8, and/or right inferior vein was done [4/10(28.6%) SFSS vs 52/152(34.2%) no SFSS]. The 6-mo, 1-, 3-, 5-, 7-and 10-year survival in patients with SFSS were 30%, 30%, 25%, 25%, 25% and 25% respectively, while, the 6-mo, 1-, 3-, 5-, 7-and 10-year survival in patients without SFSS were 70.1%, 65.6%, 61.7%, 61%, 59.7%, and 59.7% respectively, with statistical significant difference(P = 0.00). Potassium and Zinc in Relation to Improve Flax Varieties Yield and Yield Components as Grown under Sandy Soil Conditions

Two field experiments were carried out at the experimental station of National Research Centre, Al-Nubaria District, El-Behira Governorate, Egypt during winter seasons of 2011/2012 and 2012/2013 to study the effect of potassium soil application at the rates (0, 50, 75 and 100 kg/fed.) with or without chelated zinc foliar application 5% at the rate of (0.03 kg/fed.) on the yield and quality of two flax varieties (Sakha-2 and Amon) in newly reclaimed sandy soil. The obtained results could be summarized as follow: Sakha-2 variety significantly surpassed Amon in plant height, technical length, number of branches/plant, seed yield/plant, straw yield/plant, 1000 seed weight, seed yield/fed., straw yield/fed., fiber %, fiber yield/fed. and oil yield/fed. Amon variety surpassed Sakha-2 in fruiting zone length, number of capsules/plant and oil % in both seasons. Potassium soil application at the rate of (100 kg/fed.) with chelated zinc foliar application 5% at the rate of (0.03 kg/fed.) gave the highest value for all studied characters.

Evaluation of Appropriateness of Proton Pump Inhibitors (PPIs) Use in the Region of Sharjah, United Arab Emirates (UAE)

Background: Proton pump inhibitors (PPIs) are drugs that reduce the production of acid in the stomach. Recently, the use of PPI has been increasing among communities, whether with or without prescription. As a part of the healthcare team, the pharmacist should not only dispense medications but also ensure the appropriate use of these medications. Studies conducted within 16 countries showed substantial variation in the appropriateness of the use of PPI drugs. Aim: To evaluate the appropriateness of PPIs use in Sharjah, UAE based on surveys answered by pharmacists, physicians, and patients. Methods: A cross-sectional survey study was conducted on December 2017 at Sharjah, UAE as an example of information obtained from the Middle East. Two different surveys were conducted on physicians and pharmacists. Both Physicians and Pharmacists were chosen randomly from Yellow Pages. The results obtained from both studies were used to develop a patient’s survey, which was distributed among University of Sharjah students, their families and random people at Shopping Malls and Clinics. Results: The results obtained from the patients’ survey showed that ~39% of PPI users from the region of Sharjah are 25 - 44 years old. Approximately 79% are using PPI according to physicians’ prescriptions. Prescriptions’ duration is varied between 1 month (39%) and 6 months (22%), where 52% of PPI users ask their physicians to prescribe PPIs when needed. Suggested reasons for the use of PPI included inappropriate food habits (52%), use of other medications (16%) or bacterial infection (13%). Around 52% of the patients did not receive any recommendations regarding the deprescribing of PPIs. According to the pharmacists’ surveys, an average sale of 5 - 10 PPI packages is reported per day, and around 50% are sold without a prescription. Behavior and Flexural Prediction of Special Cementitious Bonding Material for Fiber Reinforced Strengthening Systems

Strengthening of RC structures with externally bonded FRP （fiber reinforced polymers） has become an important challenge in civil engineering. Epoxy is the main bonding agent used so far, but in the case of a fire, it is subjected to complete loss of his bonding capabilities. Mineral based composites strengthening systems consist of FRPs and a cementitious bonding agent which form a repair or strengthening system that is more compatible with the concrete substrata, and roved its efficiency. The current research introduces the use of a special cementitious material ＂Grancrete＂ as a bonding agent. Test results of 32 T-section RC beams strengthened with various FRG （fiber reinforced Grancrete） strengthening systems are presented. The results demonstrated that most of the specimens were likely to fail by debonding of the FRP from the concrete either at the ends or at intermediate flexural cracks. This paper presents an in-depth study aimed at the development of a better understanding of debonding failures in RC beams strengthened with externally bonded FRP systems. Different analytical models, published in the literature for plate end debonding, are reviewed and compared to test results. The results also demonstrated that when using U-wraps, the specimens were likely to fail by FRP sheet rupture.

This paper presents a study of the segmentation of medical *** paper provides a solid introduction to image enhancement along with image segmentation *** the first step,the morphological operations are employed to ensure image detail protection and *** objective of using morphological operations is to remove the defects in the texture of the ***,the Fuzzy C-Means(FCM)clustering algorithm is used to modify membership function based only on the spatial neighbors instead of the distance between pixels within local spatial neighbors and cluster *** proposed technique is very simple to implement and significantly fast since it is not necessary to compute the distance between the neighboring pixels and the cluster *** is also efficient when dealing with noisy images because of its ability to efficiently improve the membership partition *** results are performed on different medical image ***(Us),X-ray(Mammogram),Computed Tomography(CT),Positron Emission Tomography(PET),and Magnetic Resonance(MR)images are the main medical image modalities used in this *** obtained results illustrate that the proposed technique can achieve good results with a short time and efficient image *** results on different image modalities show that the proposed technique can achieve segmentation accuracies of 98.83%,99.71%,99.83%,99.85%,and 99.74%for Us,Mammogram,CT,PET,and MRI images,respectively.

Therapeutic Mammoplasty in Management of Breast Cancer: A Prospective Clinical Study

Background: The introduction of oncoplasticbreast surgery, with a concomitant contralateral symmetrization procedure, allows the surgical oncologist the freedom to perform wider excisions, with more favorable cosmetic outcomes. Objective: To assess the feasibility of bilateral breast reduction for the management of breast cancer, and to determine the most suitable surgical technique of therapeutic mammoplasty for each patient. And to evaluate the cosmetic outcome during the postoperative period. Methods: This study was from June 2014 until June 2016, and included 86 female patients with breast cancer;divided in 3 groups;group A;had bilateral superior pedicle reduction mammoplasty, group B;had bilateral inferior pedicle reduction mammoplasty, group C;had bilateral batwing mastopexy. 1 month postoperative, patients were asked to answer a 5-scale questionnaire evaluating their own cosmetic outcome. Criteria they were asked to evaluate were: symmetry, shape and volume, projection, correction of ptosis, visibility of the scars and overall satisfaction. A similar questionnaire was answered by a panel made up of a surgeon and breast-care nurse after seeing the pre- and post-operative photos of the patients in question. Results: Group A had a mean tumor size of 2.7 ± 0.69 cm, least resected margins 1.2 - 4.3 cm, Group B had a mean tumor size of 3.2 ± 1.19 cm, least resected margins 0.7 - 3.8 cm, Group C had a mean tumor size of 3.4 ± 1.12 cm, least resected margins 1.7 - 5.2 cm. Cosmetic results fared high above average. Group C fared better in overall satisfaction but Group B fared better in ptosis correction, projection and symmetry. The panel answers showed difference regarding shape and volume;the only two criteria to be almost of the same result among the three groups. In the literature,numerous techniques have been employed to decrease noise in medical image modalities,including X-Ray(XR),Ultrasonic(Us),Computed Tomography(CT),Magnetic Resonance Imaging(MRI),and Positron Emission Tomography(PET).These techniques are organized into two main classes:the Multiple Image(MI)and the Single Image(SI)*** the MI techniques,images usually obtained for the same area scanned from different points of view are used.A single image is used in the entire procedure in the SI *** denoising techniques can be carried out both in a transform or spatial *** paper is concerned with single-image noise reduction techniques because we deal with single medical *** most well-known spatial domain noise reduction techniques,including Gaussian filter,Kuan filter,Frost filter,Lee filter,Gabor filter,Median filter,homomorphic filter,Speckle reducing anisotropic diffusion(SRAD),Nonlocal-Means(NL-Means),and Total Variation(TV),are ***,the transform domain noise reduction techniques,including wavelet-based and Curvelet-based techniques,and some hybridization techniques are ***,a deep(Convolutional Neural Network)CNN-based denoising model is proposed to eliminate Gaussian and Speckle noises in different medical image *** model utilizes the Batch Normalization(BN)and the ReLU as a basic *** a result,it attained a considerable improvement over the traditional *** previously mentioned techniques are evaluated and compared by calculating qualitative visual inspection and quantitative parameters like Peak Signal-to-Noise Ratio(PSNR),Correlation Coefficient(Cr),and system complexity to determine the optimum denoising algorithm to be applied *** on the quality metrics,it is demonstrated that the proposed deep CNN-based denoising model is efficient and has superior denoising performance over the traditionaldenoising techniques.

The Role of Laparoscopy and Laparoscopic Ultrasound in Staging and in Palliation of Upper Gastro-Intestinal Malignancies: The Egyptian National Cancer Institute Experience

Background: Surgical resection of upper gastrointestinal malignancies occasionally carries substantial morbidity due to inaccurate pre-operative staging. The potential to prevent needless laparotomy by means of precise staging is the pouring force behind the use of diagnostic laparoscopy (DL). Objective: To assess the role of laparoscopy and laparoscopic ultrasound (LUS) in proper staging of upper gastro intestinal malignancies, and in potential palliation in advanced cases for pain (by neurolytic celiac plexus block) or gastric outlet obstruction (by laparoscopic bypass surgery). Study design: In this prospective study, 62 patients with lower esophageal, gastric and peri-pancreatic carcinomas were joined after written informed consent. All patients were examined with laparoscopy and LUS with the help of frozen section analysis to any doubtful metastatic site, peritoneal fluid and ascitic fluid analysis. Results: DL helped us to avoid needless laparotomy in 22.5% of patients, reducing its post-operative complications, hospital stay and cost. DL also helped us to do palliative management either in the form of gastric bypass or laparoscopic celiac plexus block. Conclusion: we praise the use of DL as a safe, effective and complimentary method to the other routine imaging modalities, in proper staging and palliation for upper gastrointestinal malignancies.

Poor Knowledge of Tuberculosis among the Population of Jeddah and Makkah Saudi Arabia, a high-Risk Area

SETTING: Millions of pilgrims visit Makkah (Mecca), Saudi Arabia, from regions of endemic tuberculosis. Little consideration has been given to the potential impact of this disease on the local population. OBJECTIVES: To assess knowledge about TB among residents of Makkah and Jeddah (Makkah’s principal travel gateway). METhODS: A cross-sectional survey of 1004 residents. Data were analyzed by gender. RESULTS: Spread of TB by coughing was a transmission route known by 44.2% of males and 59.6% of females (P hat TB is caused by bacteria;71.6% of females and 52.3% of males knew TB is infectious (P hes to TB prevention, 48.0% of respondents answered patient isolation and 15.2% vaccination. Overall, 50.6% of males and 38.3% of females (P hemoptysis occurred. With a persistent cough, 65.4% of respondents would do nothing or wait before consulting a healthcare provider. high numbers of “don’t know” responses were recorded throughout the survey. CONCLUSIONS: Within the population across pilgrimage areas in Saudi Arabia, knowledge is lacking concerning TB transmission, the cause of the disease, means of prevention and the success of treatment, highlighting an urge need for better public education.
