Sorafenib is used worldwide as a first-line standardsystemic agent for advanced hepatocellular carcinoma(hCC) on the basis of the results of two large-scale Phase Ⅲ trials. Conversely,hepatic arterial infusion chemotherapy(hAIC) is one of the most recommended treatments in Japan. Although there have been no randomized controlled trials comparing sorafenib with hAIC,several retrospective analyses have shown no significant differences in survival between the two therapies. Outcomes are favorable for hCC patients exhibiting macroscopic vascular invasion when treated with hAIC rather than sorafenib,whereas in hCC patients exhibiting extrahepatic spread or resistance to transcatheter arterial chemoembolization,good outcomes are achieved by treatment with sorafenib rather than hAIC. Additionally,sorafenib is generally used to treat patients with Child-Pugh A,while hAIC is indicated for those with either Child-Pugh A or B. Based on these findings,we reviewed treatment strategies for advanced hCC. We propose that sorafenib might be used as a first-line treatment for advanced hCC patients without macroscopic vascular invasion or Child-Pugh A,while hAIC is recommended for those with macroscopic vascular invasion or Child-Pugh A or B. Additional research is required to determine the best second-line treatment for hAIC non-responders with Child-Pugh B through future clinical trials.

Efficacy of treatment with rebamipide for endoscopic submucosal dissection-induced ulcers

AIM:To prospectively compare the healing rates of endoscopic submucosal dissection(ESD)-induced ulcers treated with either a proton-pump inhibitor(PPI)or ***:We examined 90 patients with early gastric cancer who had undergone *** patients were administered an intravenous infusion of the PPI lansoprazole(20 mg)every 12 h for 2 d,followed by oral administration of lansoprazole(30 mg/d,5 d).After7-d treatment,the patients were randomly assigned to 2 groups and received either lansoprazole(30 mg/d orally,n=45;PPI group)or rebamipide(300 mg orally,three times a day;n=45;rebamipide group).At 4and 8 wk after ESD,the ulcer outcomes in the 2 groups were ***:No significant differences were noted in patient age,underlying disease,tumor location,helicobacter pylori infection rate,or ESD-induced ulcersize between the 2 *** both 4 and 8 wk,the healing rates of ESD-induced ulcers were similar in the PPI-treated and the rebamipide-treated patients(4 wk:PPI,27.2%;rebamipide,33.3%;P=0.5341;8 wk:PPI,90.9%;rebamipide,93.3%;P=0.6710).At 8 wk,the rates of granulation lesions following ulcer healing were significantly higher in the PPI-treated group(13.6%)than in the rebamipide-treated group(0.0%;P=0.0103).Ulcer-related symptoms were similar in the2 treatment groups at 8 *** medication cost of 8-wk treatment with the PPI was 10945 yen vs 4889 yen for *** ulcer bleeding or complications due to the drugs were observed in either treatment ***:The healing rate of ESD-induced ulcers was similar with rebamipide or PPI treatment;however,rebamipide treatment is more cost-effective and prevents granulation lesions following ulcer healing.

Cap polyposis is a rare intestinal disorder. Characteristic endoscopic findings are multiple inflammatory polypoid lesions covered by caps of fibrous purulent exudate. Although a specific treatment has not been established, some studies have suggested that eradication therapy for helicobacter pylori(h. pylori) is effective. We report a case of a 20-year-old man with cap polyposis presenting with hematochezia. Colonoscopy showed the erythematous polyps with white caps from the sigmoid colon to rectum. histopathological findings revealed elongated, tortuous, branched crypts lined by hyperplastic epithelium with a mild degree of fibromusculosis in the lamina propria. Although h. pylori eradication was instituted, there was no improvement over six months. We then performed en bloc excision of the polyps by endoscopic submucosal dissection(ESD), which resulted in complete resolution of symptoms. ESD may be a treatment option for cap polyposis refractory to conservative treatments. We review the literature concerning treatment for cap polyposis and clinical outcomes.

Recent progress in plasma control studies on the improvement of plasma performance in heliotron J is reviewed. The supersonic molecular beam injection （SMBI） fueling is successfully applied to heliotron J plasma. A supersonic h2-beam is effectively injected to increase fueling efficiency and generate a peaked density profile. Local fueling with a short-pulsed SMBI can increase the core plasma density and avoid the degradation arising from edge cooling. Second harmonic electron cyclotron current drive （ECCD） experiments were conducted by launching a focused Gaussian beam with a parallel refractive index of -0.05 ≤ Nil 〈 0.6. Results show that the electron cyclotron （EC） driven current is determined not only by Nil but also by local magnetic field （B） structure where the EC power is deposited. Detailed analysis of the observed NI and B dependences is in progress with a ray-tracing simulation using the TRAVIS code. Fast ion velocity distribution was investigated using fast protons generated by ion cyclotron resonant frequency （ICRF） minority heating. For the standard configuration in heliotron J, charge ex- change neutral particle analysis （CX-NPA） measurements show higher effective temperature of fast minority protons in the on-axis resonance case compared to that in the hFS （high field side） off-axis resonance case. however, the increase in bulk ion temperature in the hFS resonance case is larger than that in the on-axis resonance.

We present a rare case of fecaloma, 7 cm in size, in the setting of systemic scleroderma. A colonoscopy revealed a giant brown fecaloma occupying the lumen of the colon and a colonic ulcer that was caused by the fecaloma. The surface of the fecaloma was hard, large and slippery, and fragmentation was not possible despite the use of various devices, including standard biopsy forceps, an injection needle, and a snare. however, jumbo forceps were able to shave the surface of the fecaloma and break it successfully by repeated biting for 6 h over 2 d. The ability of the jumbo forceps to collect large mucosal samples was also appropriate for achieving fragmentation of the giant fecaloma.
