Cerebral Venous Thrombosis: A Rare Complication of Spinal Anesthesia

Introduction: Cerebral venous thrombosis (CVT) is a rare complication of spinal anesthesia. The diagnosis is difficult when it occurs by isolated headache, following post spinal puncture headache (PSPh). We report a case of cerebral venous thrombosis complicating PSPh, following a spinal anesthesia. Observation: A 23-year-old man was admitted to the emergency room for PSPh appeared 6 hours after a spinal aesthesia for removal of osteosynthesis equipment from the right tibial plateau. The headache was fronto-occipital, relieved by lying down. Six days later, headache became permanent, not relieved by morphine, associated with vomiting. The neurological examination was normal. Brain magnetic resonance Magnetic venography showed superior sagittal sinus and right transverse sinus thrombosis. After administration of low molecular weight heparin at curative dose, and warfarin under INR control, the evolution was marked by a rapid regression of headaches on the same day. After a 1-year setback, the patient was asymptomatic. Conclusion: CVT is a rare complication of spinal anesthesia and potentially severe if diagnosed lately. Urgent brain imaging must be performed for any atypical headache or the onset of neurological disorders so as not to misunderstand cerebral vein thrombosis. Controlling the spine anesthesia would reduce the inadvertent perforation of the dura-mother that would lead to CVT complicating post spinal puncture headaches.

Physico-Chemical and Isotopic (¹⁸O, ²h and ³h) Characterization of the Upper Dallol Maouri Watershed (Niger)

Dallol Maouri is one of the fossil valleys on the left bank of the Niger River. Its watershed drains water from valleys in the Tahoua and dosso regions of Niger. The watercourse is not continuous and has areas of surface water accumulation (ponds) and areas of spreading and infiltration. The surface water potential in this area is limited, with most of the ponds having a temporary to the semi-permanent regime. Groundwater resources are, therefore, the main sources of withdrawal for human food, agriculture, and livestock. Climate change through the variation in rainfall, the often-anarchic multiplication of catchment works (wells and boreholes) and agricultural activities can have impacts on the qualitative and quantitative evolution of surface and groundwater. The objective of this study is to characterize the surface and underground water of the upper dallol Maouri through the physicochemical parameters of the water and the use of the isotopes of the water molecule (18O, 2h and 3h). A total of 53 water samples were analyzed at the laboratory of the Regional Directorate of hydraulics and Sanitation of Tillabéri (Niger) for chemical parameters and 73 samples were analyzed at the Radio Analysis and Environment Laboratory (LRAE) of the National School of Engineers of Sfax (ENIS) of Tunis for isotopy. The water of the ponds is characterized by three types of facies: calcium bicarbonate, calcium and magnesium bicarbonate, and calcium nitrate chloride. The groundwater is 75% of the chloride-nitrate-calcium facies and the CT3/CT2 groundwater is calcium and magnesium bicarbonate. The results of the stable isotopes made it possible to highlight the presence of four distinct water groups: ponds characterized by highly evaporated water, the sheets of Continental Terminal 1 and 2 (CT1 and CT2) with an isotopic cachet of old water, the water table consisting of a two-layer system in the southwestern part of the area. Artificial induction of labor (AIL) is the set of techniques intended to induce childbirth in order to reduce maternal-fetal mortality. The aim of this paper was to study the maternal and fetal prognosis after the artificial induction of labor with misoprostol. It was a descriptive prospective study conducted from September 1, 2018 to February 28, 2019 at the maternity unit of the Referral health Center (RhC) in commune I of Bamako. It was about any pregnant woman at term who had received misoprostol as part of the artificial induction of labor. 102 cases of artificial induction with misoprostol 200 μg were collected out of 3641 deliveries, or a frequency of 2.8%. The age group 28 - 32 years accounted for 56.86% with an average age of 28.8 years. Multiparous represented 54.90% of the number. The history of diabetes and hypertension involved 37.25% of the pregnant women. The main indication was premature rupture of membranes (PRM) (40.2%). Induction was performed between the 37th and 41st week of amenorrhea (69.6%). The trigger index according to BISHOP was greater than 7 (50.98%). Induction was done only with 3/4 of misoprostol tablet 200 μg (43.14%). The average labor time was 7 hours (89.22%). The vaginal birth concerned 92.16% of pregnant women. An APGAR10 was noted in 97.85% of newborns after 05 minutes. AIL with misoprostol was practiced at the Center. The main indication was the premature rupture of the membranes. It improves the maternal and fetal prognosis.

Role of Laparoscopic Surgery in the Management of Female Infertility at the Department of Gynecology of hôpital du Mali

Infertility is a socio-cultural drama in Africa, especially in Mali and remains difficult for couples to overcome. Laparoscopy, also called minimally invasive surgery or keyhole surgery, is an operative technique that permits to explore the pelvis and perform an appropriate therapeutic procedure. The objective of our work was to assess the role of laparoscopic surgery in the treatment of female infertility at hopital du Mali. It was a retrospective descriptive study that was conducted over a period of 5 years (January 2013 to December 2018). Any patients followed for infertility and who underwent laparoscopic surgery in the gynecology department over a period of two (2) years were included. The outcome of laparoscopic surgery was evaluated in terms of conception of pregnancy. We had collected 103 infertile patients out of 2984, with a frequency of 3.45%. The mean age of our patients was 30.2 years. housewives and out-of-school women accounted for 68.93% and 54.37% respectively. Nulligravida represented 41.75%. Regarding infertility, it was primary in 41.75% and secondary in 58.25%, with an average duration of 6.2 years. Systematic chlamydial serology was positive in 62.14% of our patients. hysterosalpingography (hSG) revealed bilateral tubal obstruction in 53.33% of cases. Laparoscopy found tubal adhesions in 97.80% of cases. Adhesiolysis was the main procedure performed with 84.47% bilateral tubal patency achieved in the methylene blue test. Among our patients who had contracted a pregnancy (22.99%), 85% had carried their pregnancies to term. Tubal damage was the main cause of infertility. Adhesiolysis remains the main laparoscopic procedure for treatment.

Flow Pattern and Residence Time of Groundwater within Volta River Basin in Benin(Northwestern Benin)

The knowledge about groundwater flow conditions within Volta river basin in Benin, has restricted information coming to piezometry, water storage time, water path, and water quality. A good groundwater resources assessment of this sudano-sahelian area, is a huge condition for the sustainable management of water resources, which since the part of the 20th century is facing a severe drought that leads to a greater aridity. This article provides a summary with the main elements of carbon isotope (13C and 14C) as well as tritium (3h) coupled with hydrogeological and hydrochemical data. The goal is to improve the initial water recharging and the groundwater flow system within the aquifer. Two main results can be produced from the groundwater chemistry. First, the interactions between groundwater and clay minerals related to the residence time of groundwater are indicated by a slight evolution of Ca-hCO3 and Ca-Mg-hCO3 to Na-hCO3. Beside that towards water types Cl-NO3 indicates the anthropogenic influence on groundwater, related to agricultural activities and sanitation conditions. The carbon-14 activity measured on the TDIC is between 17.29 and 85.47 pmC. Therefore, it contains some samples covering a wide period of time from now to the holocene implying a continuous system recharging over time. All the data confirm the assumption of a homogeneous, largely unified aquifer system with a multi-layer structure, but it also points out the low resource sustainability and a strong anthropogenic contamination of the most superficial horizons.

Uterine leiomyomas (myomas) are the most common benign tumors of the female genital tract. They affect 20% to 25% of women of childbearing age and are 3 to 9 times more common in black women. We initiated this study in order to report the socio-demographic aspects and the indications for leiomyomas surgery at Hôpital du Mali. This was a retro-prospective descriptive study, conducted in the gynecology department of hôpital du Mali from January 1, 2015 to December 31, 2019. Any patients, regardless of their age, in whom a leiomyomas had been detected and surgically treated were included. Objective: To describe the screening for uterine cancer by visual methods. Method: We conducted a prospective and descriptive study over a period of 12 months from January to December 2020. Direct observation of providers and interview of patients were performed for data collection. Results: Out of 3400 patients, 1024 were enrolled for the study, or 30.11%. The average age of the patients was 41 years with the extremes of 17 to 87 years. Women aged 40 to 45 were the most represented with 58.78%, (n = 602). Housewives 85.83% (n = 879), they had a primary education level in 71.09% of cases (n = 728), the vast majority were married 92.28% (n = 945). Multiparous represented 58.78% (n = 602) of the sample, nulliparous 18.26% (n = 187), women living in rural areas 30.17% (n = 309), in urban areas 69.82% (n = 715). The test was positive in 43 patients (4.19%) but squamocylindrical junction was not visible in 17% of cases. The preparation of acetic acid and lugol: good preparation in 100% of cases;

Sacrospinous fixation (SSF) or Richter’s intervention (RI) aims to treat genital prolapse by securing the posterior vaginal wall to the small sacrospinous ligament. It is performed by low approach and includes a dissection of the pararectal space, visual exposure of the sacrospinous ligament and a needle with strait needle holder with nonabsorbable threads. It is often associated with a more complex corrective procedure, including cystocele cure, vaginal hysterectomy and posterior myorrhaphy. The objective of this study is to report the results of SSF in the gynecology department of hopital du Mali. A descriptive study was conducted from September 2014 to September 2015 concerning 37 patients operated on for uterine prolapse (UP). All patients with grade III UP were included in our study in whom a unilateral hysterectomy (Uh) and sacrospinous fixation (SSF) were performed. All the patients were scheduled. Preoperatively they had benefited from an assessment and a pre-anesthetic consultation. hospitalization of at least 24 hours prior to the operation was required. Postoperative follow-up was two years with a physical examination at 3 months, 9 months and 15 months, and phone calls between physical consultations. During the study period, we performed 37 RIs. The mean age of the patients was 48 years with extremes of 41 to 73 years. The large multiparity was found out in 35 cases (94.59%), the pauciparous were two with 3 deliveries for each. Long labor of more than 18 hours was found out in 9 patients (24.32%) and home delivery in 13 cases (35.13%). The duration of the occurrence of prolapse was at least two years and 35 patients were going through menopause. The type of anesthesia used for the surgery was spinal anesthesia for all patients. The average duration of the operation was 90 minutes. Complications occurred in three patients or 8.10% of cases, two cases of acute urine retention and one case of hematoma of the para-rectal space. Chemical Variability and Antibacterial Activity of Eucalyptus camaldulensis Essential Oils from Senegal

1,8-cineole, also known as eucalyptol, is the main constituent and the most important of the leaf oils of many species of the genus Eucalyptus. In present study, the essential oils isolated by hydrodistillation from the leaves of Eucalyptus camaldulensis from six different locations were analyzed by GC-FID and GC-MS. Essential oil yields ranged from 0.33% to 4.35%. In total, 34 constituents were identified, accounting for 94.0% to 99.8% of the total compositions. 1,8-cineole (46.4% - 84.4%) was the main constituent of all the essential oil samples, except for one sample collected in the Fatick region, which had p-cymene (46.4%) and 1,8-cineole (26.0%) as major compounds. The essential oil showed excellent activity against S. aureus, E. coli and E. faecalis (IZ = 25.3 ± 1.2 mm;18.7 ± 0.6 mm;17.8 ± 0.3 mm, respectively) and moderate activity against P. aeruginosa (IZ = 10.8 ± 0.8 mm). They may have potential applications in food and pharmaceutical products.

Epidemiological and Serological Profile of hepatitis B Virus in an Urban Area in Mali

The objective of our study was to evaluate hepatitis B virus (hBV) infection in an urban population. This longitudinal study was conducted in Bamako District and Kati Commune. After a preparatory phase, the persons who accepted the protocol were assessed for hBsAg. hBsAg carriers had blood collection for hBeAg assay, viral load assessment, genotyping, DNA mutation testing, and severity of hepatic fibrosis and necrosis. At the end of this study, 1475 persons were included, of which 195 had hBsAg positive confirmed, that is to say 13.97%. The mean age of hBsAg positive patients was 35.11 ± 11.12 years with a sex ratio of 2.68. hBeAg was found in 8.9% of the patients tested for this antigen. The viral load was undetectable in 10.52% of patients and greater than 2000 IU/mL in 32.24% of cases. Fibrosis ≥ F2 and necrosis ≥ A2 were found in respectively 19.72% and 6.80% of cases. Genotype E was found in 91.6 patients and an R249S mutation observed in 39.04% of cases. Conclusion: hBV infection has a serious impact on socio-economic development in Mali because it affects mainly the young male population, hence the need to organize preventive measures effectively.
