Prevalence of gastric varices and results of sclerotherapy with N-butyl 2 cyanoacrylate for controlling acute gastric variceal bleeding

AIM： To study the prevalence, predictors and control of bleeding following N-butyl 2 cyanoacrylate （NBC） sclerotherapy of gastric varix （GV）. METhODS： We analyzed case records of 1436 patients with portal hypertension, who underwent endoscopy during the past five years for variceal screening or upper gastrointestinal （GI） bleeding. Fifty patients with bleeding GV underwent sclerotherapy with a mean of 2 mL NBC for control of bleeding. Outcome parameters were primary hemostasis （bleeding control within the first 48 h）, recurrent bleeding （after 48 h of esophagogastro-duodenoscopy） and in-hospital mortality were analyzed. RESULTS： The prevalence of GV in patients with portal hypertension was 15% （220/1436） and the incidence of bleeding was 22.7% （50/220）. Out of the 50 bleeding GV patients, isolated gastric varices （IGV-Ⅰ） were seen in 22 （44%）, gastro-oesophageal varices （GOV） on lesser curvature （GOV-Ⅰ） in 16 （32%）, and GOV on greater curvature （GOV-Ⅱ） in 15 （30%）. IGV-Ⅰ was seen in 44% （22/50） patients who had bleeding as compared to 23% （39/170） who did not have bleeding （P 〈 0.003）. Primary hernostasis was achieved with NBC in all patients. Re-bleeding occurred in 7 （14%） patients after 48 h of initial sclerotherapy. Secondary hemostasis was achieved with repeat NBC sclerotherapy in 4/7 （57%）. Three patients died after repeat sclerotherapy, one during transjugular intrahepatic portosystemic stem shunt （TIPSS）, one during surgery and one due to uncontrolled bleeding. Treatment failure-related mortality rate was 6% （3/50）. CONCLUSION： GV can be seen in 15% of patients with portal hypertension and the incidence of bleeding is 22.7%. NBC is highly effective in controlling GV bleeding. In hospital mortality of patients with bleeding GV is 6%.

The electrocatalytic nitrogen reduction reaction(NRR)has emerged as a promising renewable energy source and a feasible strategy as an alternative to haber-Bosch ammonia(Nh_(3))***,finding an efficient and cost-effective robust catalyst to activate and cleave the extremely strong triple bond in nitrogen(N_(2))for electrocatalytic NRR is still a ***,a FeNi@CNS nanocomposite as an efficient catalyst for N_(2) fixation under ambient conditions is *** FeNi@CNS nanocomposite was prepared by a simple water bath process and *** FeNi@CNS is demonstrated to be a highly efficient NRR catalyst,which exhibits better NRR performance with exceptional Faradaic efficiency of 9.83%and an Nh_(3) yield of 16.52μg h^(−1) cm^(−2) in 0.1 M Na_(2)SO_(4) aqueous ***,high stability and reproducibility with consecutive 6 cycles for two hours are also demonstrated throughout the NRR electrocatalytic process for 12 ***,the FeNi@CNS catalyst encourages N_(2) adsorption and activation as well as effectively suppressing competitive ***,this earth-abundant FeNi@CNS catalyst with a subtle balance of activity and stability has excellent potential in NRR industrial applications.

Prevalence of non-helicobacter pylori duodenal ulcer in Karachi,Pakistan

AIM:To determine the prevalence of non-helicobacter pylori (h pylori)-related duodenal ulcer in patients with acid-peptic diseases. METhODS: Medical records of patients who attended the Gastroenterology Department at Aga Khan University hospital from 1999 to 2001 and had endoscopic diagnosis of duodenal ulcers were reviewed. Duodenal ulcer associated with h pylori was diagnosed on the basis of endoscopy, rapid urease test and histopathology whereas histories of aspirin or other non-steroidal anti-inflammatory drugs (NSAIDs) related duodenal ulcers. Non-h pylori, non-NSAID duodenal ulcers were those without h pylori infection and history of NSAID intake. Co-morbid conditions associated were noted. RESULTS: Of 2 260 patients, 10% (217/2 260) had duodenal ulcer. Duodenal ulcer related to h pylori infection accounted for 53% (116/217), NSAID-related 10% (22/217), non-h pylori non-NSAID 29% (62/217), and 8% (17/217) had both h pylori infection and histories of NSAID intake. Fifteen percent (18/116)_patients had past histories of peptic ulcer disease in h pylori infection, while 8% (5/62) in non-h pylori non-NSAID ulcer. Co-morbid conditions in h pylori infection were seen in 23% (27/116) and 34% (21/62) in non-h pylori non-NSAID ulcer. CONCLUSION: Incidence of h pylori infection related with duodenai ulcer is common. In the presence of co-morbids, non-h pylori and non-NSAID duodenal ulcer is likely to be present.

在现代农业中,农业生产高度依赖于大量使用化学肥料。由于化肥的施用方法和/或管理方法不当,其养分利用效率普遍较低,不可避免地造成许多严重的生态问题,如污染地下水和破坏土壤结构,以及影响作物品质等不良结果。与化肥农业相比,施用各种类型的有机肥料或生态友好型农业肥料被认为是发展环保农业的基础。其中,生物堆肥被认为是一种优质肥料,可以改善土壤质量,增加作物产量。但由于其矿化速度慢,某些堆肥中的大量养分不能及时满足作物的快速生长需求。因此,生物堆肥通常与化肥一起使用。因其结合了有机和化学肥料的优势,近些年常被应用于更高效环保的现代可持续农业中。然而,由于堆肥类型的不同和作物对养分的要求不同,需要人们掌握更详细信息以获得最大的生产效益,如最佳肥料混合比率和施用时间等。此外,土壤分解者（如蚯蚓）的分解能力在提高堆肥养分释放速度和改善土壤条件方面也发挥着重要作用。因此,研究有机肥料和化学肥料的混合比例,以及蚯蚓添加的现代可持续农业生物技术,对提高有机肥料在农业生产中的作用和减少农业化肥用量具有重要意义。本研究在植物生长室中开展了两项实验,以研究牛粪堆肥与化肥的不同混合比例和肥料施用时间（实验Ⅰ）,以及牛粪堆肥与化肥的不同混合比例和蚯蚓添加（实验Ⅱ）对土壤指标、作物产量、品质、植物性状的影响。具体而言,该研究以番茄（Solanum Lycopersicum L.）作为目标作物,在实验I中设置了对照（不施肥）和四个堆肥与化肥的混合比例处理,包括OF0（0堆肥和100%化学肥料）,OF30（30:70）,OF50（50:50）,OF70（70:30）,和两个肥料施用时间（N25和N50）,在实验II中设置了四个肥料混合比例,包括OF0（0:100）,OF40（40:60）,OF70（70:30）,and OF100（100:0）,以及添加和不添加蚯蚓实验处理。测量了土壤指标（有机质、有效氮和有效磷含量）、植物形态性状（株高、分枝数、根茎比、比叶面积、总根长和比根长）、生理性状（总叶绿素、总类胡萝卜素和叶N含量）、植物生物量（地上和地下生物量）、果实产量（果实数量和重量、果实长度、果实直径和果实单重）和质量（番茄果实的总糖、p h、维生素C、水分和番茄红素含量）以及实验后期蚯蚓的存活数量等指标。主要结论如下:（1）所有使用堆肥混合肥的土壤指标,植物性状和产量均比不施肥处理好。此外,30%化肥+70%堆肥处理（OF70）的有效氮、磷和有机质含量,以及植物生物量和果实产量均优于纯化肥处理（OF0）。（2）施氮时间,以及其与肥料混合配比处理的交互作用对所有指标的影响均不显著,各处理间的果实质量也基本相似。（3）与OF0处理相比,添加蚯蚓显著提高了植物生物量和产量,尤其是在全有机肥条件下（OF100）。并且OF100处理提高了蚯蚓的存活率。（4）OF100处理（有机肥含量高）显著提高了蚯蚓数量,而纯化肥处理（无有机肥）则显著降低了蚯蚓数量。（5）与OF0处理相比,无论蚯蚓是否添加,OF100处理中的所有土壤参数（有效氮磷,有机质）均显著提高。（6）肥料及其与蚯蚓的相互作用对番茄植株的形态性状和生理性状没有显著影响,而生理性状除胡萝卜素外,其他均受蚯蚓处理影响显著,且添加蚯蚓处理的植物叶片N浓度和总叶绿素数值较高。

Large spatial shifts of reflected light beam off biaxial hyperbolic materials

Many optical systems that deal with polarization rely on the adaptability of controlling light reflection in the lithography-free nanostructure. In this study, we explore the Goos–h?nchen(Gh) shift and Imbert–Fedorov(IF) shift in a biaxial hyperbolic film on a uniaxial hyperbolic substrate. This research statistically calculates and analyzes the Gh shift and IF shift for the natural biaxial hyperbolic material(NBhM). We select the surface with the strongest anisotropy within the NBhM and obtain the complex beam-shift spectrum. By incorporating the NBhM film, the Gh shift caused by a transversely magnetic incident-beam on the surface increases significantly compared with that on the uniaxial hyperbolic material. The maximum of Gh shift can reach 86 λ0at about 841 cm-1when the thickness of NBhM is 90 nm, and the IF shift can approach 2.7 λ0for a circularly-polarized beam incident on a 1700-nm-thick NBhM. It is found that the spatialshift increases when a highly anisotropic hyperbolic polariton is excited in hyperbolic material, where the shift spectrum exhibits an oscillating behaviour accompanied with sharp shift peak(steep slope). This large spatial shift may provide an alternative strategy to develop novel sub-micrometric optical devices and biosensors.

The FAIR principles have been widely cited,endorsed and adopted by a broad range of stakeholders since their publication in *** intention,the 15 FAIR guiding principles do not dictate specific technological implementations,but provide guidance for improving Findability,Accessibility,Interoperability and Reusability of digital *** has likely contributed to the broad adoption of the FAIR principles,because individual stakeholder communities can implement their own FAIR ***,it has also resulted in inconsistent interpretations that carry the risk of leading to incompatible ***,while the FAIR principles are formulated on a high level and may be interpreted and implemented in different ways,for true interoperability we need to support convergence in implementation choices that are widely accessible and(re)-*** introduce the concept of FAIR implementation considerations to assist accelerated global participation and convergence towards accessible,robust,widespread and consistent FAIR *** self-identified stakeholder community may either choose to reuse solutions from existing implementations,or when they spot a gap,accept the challenge to create the needed solution,which,ideally,can be used again by other communities in the ***,we provide interpretations and implementation considerations(choices and challenges)for each FAIR principle.
