BACKGROUND:Numerous medical conditions require timely medication administration in the emergency department(ED).Automated dispensing systems(ADSs)store premixed common doses at the point-of-care to minimize time to administration,but the use of such automation to improved time to medication administration has not been *** vancomycin is a commonly used empiric antimicrobial,we sought to quantify the effect of using an ADS on time to drug delivery in patients presenting to the *** study aimed to determine the efficacy of utilizing an ADS to improve time to administration of vancomycin and determine any negative effects on dosing ***:The institional review board approved the retrospective quality improvement study took place in a single,urban academic tertiary care ED with an annual census of 80 *** subjects were all patients receiving vancomycin for the management of sepsis between March 1 to September 30,2008 and the same time period in *** primary outcome was the proportion of patients who received vancomycin within one hour of bed placement and the secondary outcome was dosing ***:Sixty-three patients had weight and dosing information available(29 before and 34after intervention)and were included in the *** intervention,no patient received vancomycin in less than 60 minutes,while after intervention 14.7%of the patients received it in less than 60minutes(difference in proportions 14.7%,95%CI 0.39%-30.0%,P=0.04).A similar proportion of the patients received correct dosing before and after intervention(44.8%vs.41.2%,difference in proportions 3.7%,95%CI-20.0%-26.7%,P=0.770).CONClUSION:The use of an ADS may improve the timing of medication administration in patients presenting to the ED without affecting dosing appropriateness.

Chrysanthellum americanum (l.) Vatke is a medicinal plant used by the traditional healers to treat epilepsy and associated memory impairment. This work aims at evaluating the anticonvulsant effects of Chrysanthellum americanum aqueous extract in mice pilocarpine model of epilepsy and associated memory loss. Mice were administered orally Chrysanthellum americanum aqueous extract (27.69, 69.22, 138.45, 276.9 mg/kg, prepared from the whole part) for test groups, intraperitoneally 300 mg/kg sodium valproate for the positive control group or orally 10 ml/kg distilled water for the negative control group, respectively, during a period of seven consecutive days. On the first day, temporal lobe epilepsy was induced by intraperitoneal injection of 360 mg/kg pilocarpine one hour after the administration of different treatment to mice, and the occurrence of status epilepticus was evaluated. On the second day, the anticonvulsant property was measured after the intraperitoneal injection of a sub-convulsive dose of picrotoxin (1 mg/kg). On the seventh day, the anti-amnesic properties of the extract were evaluated in the epileptic mice using the T-maze and open field paradigms. The results show that Chrysanthellum americanum extract significantly (p Chrysanthellum americanum (276.9 mg/kg) likewise sodium valproate (300 mg/kg) significantly (p Chrysanthellum americanum aqueous extract has anticonvulsant effects against pilocarpine induced-epileptic seizures and memory impairment. These properties could be mediated by the amelioration of antioxidant defense system and cholinergic neurotransmission in epileptic mice, which could partly justify the use of Chrysanthellum americanum in the traditional medicine for the treatment of epilepsy.

Inflammatory bowel diseases are characterised by inflammation that compromises the integrity of the epithelial barrier. The intestinal epithelium is not only a static barrier but has evolved complex mechanisms to control and regulate bacterial interactions with the mucosal surface. Apical tight junction proteins are critical in the maintenance of epithelial barrier function and control of paracellular permeability. The characterisation of alterations in tight junction proteins as key players in epithelial barrier function in inflammatory bowel diseases is rapidly enhancing our understanding of critical mechanisms in disease pathogenesis as well as novel therapeutic opportunities. Here we give an overview of recent literature focusing on the role of tight junction proteins, in particular claudins, in inflammatory bowel diseases and inflammatory bowel disease associated colorectal cancer.

Objective: To analyze the relationships between mothers’ pre-pregnancy body mass index (BMI) and levels of glucose and insulin in breast-milk. Methods: Participants were 32 breast-feeding mothers who were assigned to either normal weight (group 1: n = 21, BMI between 18.5 and 24.9 kg/m2) or overweight/obese (group 2: n = 11, BMI ≥ 25 kg/m2) based on their pre-pregnancy BMI. Breast-milk samples were collected at six weeks postpartum. Glucose and insulin concentrations were measured in the breast-milk samples. To control for any extreme dietary influences on breast-milk glucose concentrations, mothers completed 24-hour food recalls. Results: Mean milk-glucose values of groups 1 and 2 were 32.6 ± 21.8 mg/dl and 51.9 ± 20.5 mg/dl respectively. Milk-borne insulin values were 4.5 ± 7.6 μIU/ml and 30.1 ± 56.3 μ IU/ml, respectively. Independent-samples t tests found that glucose levels of group 2 were greater than those of group 1, t(30) = 2.43, p = 0.02;and milk-borne insulin was greater in group 2 than in group 1, t(30) = 2.075, p = 0.05. Mothers’ pre-pregnancy BMI was found correlated with both breast-milk glucose (r = 0.483, p = 0.005) and insulin (r = 0.565, p = 0.001). Conclusion: This study found that in comparison with normal weight mothers, over-weight/ obese mothers have higher concentrations of glucose and insulin in their breast-milk. Further studies are needed to explain the underlying mechanisms, and possible consequences of infant exposure to higher levels of glucose and insulin in breast-milk.

Tomato production is influenced by shoot branching,which is controlled by different *** we produced tomato plants overexpressing the cytokinin-deactivating gene CYTOKININ OXYDASE 2(CKX2).CKX2-overexpressing(CKX2-OE)plants showed an excessive growth of axillary shoots,the opposite phenotype expected for plants with reduced cytokinin content,as evidenced by lC-MS analysis and ARR5-gus *** TCP transcription factor SlBRC1b was downregulated in the axillary buds of CKX2-OE and its excessive branching was dependent on a functional version of the GRAS-family gene lATERAl SUPPRESSOR(lS).Grafting experiments indicated that increased branching in CKX2-OE plants is unlikely to be mediated by root-derived *** CKX2-OE plants with transgenic antisense plants for the strigolactone biosynthesis gene CAROTENOID ClEAVAGE DIOXYGENASE(CCD7-AS)produced an additive phenotype,indicating independent effects of cytokinin and strigolactones on increased *** the other hand,CKX2-OE plants showed reduced polar auxin transport and their bud outgrowth was reduced when combined with auxin ***,CKX2-OE basal buds did not respond to auxin applied in the decapitated *** results suggest that tomato shoot branching depends on a fine-tuning of different hormonal balances and that perturbations in the auxin status could compensate for the reduced cytokinin levels in CKX2-OE plants.

The Bambili maars are twin contemporaneous maars embedded in trachytic rocks. The two maars are separated by a low lying inter-Maar dome whose inner walls dip towards the Southern Maar. The Bambili Northern Maar (BNM) contains water and its floor is found at an altitude lower than the Bambili Southern Maar (BSM) (50 m) which is swampy and drains into the BNM. Both maars have very steep vertical walls. Three different units are identified in the area. Unit 1 is 1 m thick sequence, characterize by the presence of matrix supported fine-grained fallouts (ash and lapilli size) with stratified diffused bedding planes. This unit is further subdivided into three different layers: l1, l2 and l3 from bottom to top with increasing sizes of particles respectively. This layer shows reverser grading and its grain sizes rarely exceed 2 cm and this unit shows no depositional structures. Unit 2 is an unstratified unit predominantly made up of tephra deposits. within these deposits are found blocks of sizes ranging between 2 and 10 cm of different petrographic types. They include scoria, basalts, trachyte, and granite cognate. The last unit, Unit 3 is distinctly stratified characterized by well sorted, heterolithologic, graded bedding layers which are matrix supported. Base on the composition and grain sizes, this unit was subdivided into 7 horizons: h1 - h7. within this unit, two sequences of deposits are distinguished: U1 and U2. Both sequences are characterized by alternation of lapilli and tuff beds. The clast comprises of juvenile materials, mudstone which are highly vesicular and altered rock fragments. Other products identified within the area are ferolithic volcanic bombs (5 - 20 cm), xenocrystals (2 - 4 cm) within finer deposits. They are elongated and show visible twinned planes. Explosion breccias are equally identified occurring as bolders. Along the inner walls of the maars are also found large basaltic and trachytic blocks which are remnants of pre-maar unit within maar
