Clinical usefulness of ankle brachial index and brachial-ankle pulse wave velocity in patients with ischemic stroke

Ankle brachial index （ABI） and brachial-ankle pulse wave velocity （baPWV） are widely used noninvasive modalities to evaluate atherosclerosis. Recently, evidence has increased supporting the use of ABI and baPWV as markers of cerebrovascular disease. This study sought to examine the relationship between ABI and baPWV with ischemic stroke. This study also aimed to determine which pathogenic mechanism, large artery disease （LAD） or small vessel disease （SVD）, is related to ABI or baPWV. Retrospectively, 121 patients with ischemic stroke and 38 subjects with no obvious ischemic stroke history were recruited. First, ABI and baPWV were compared between the groups. Then, within the stroke group, the relevance of ABI and baPWV with regard to SVD and LAD, which were classified by brain magnetic resonance image （MRI） and magnetic resonance angiography （MRA） or computed tomography angiography （CTA） findings, was assessed. The baPWV was higher in the stroke group than non-stroke group （1,944.18±416.6 cm/s vs. 1,749.76±669.6 crn/s, P 〈 0.01）. Regarding LAD, we found that mean ABI value was lower in the group with extracranial large artery stenosis （P 〈 0.01）, and there was an inverse linear correlation between ABI and the grade of extracranial large artery stenosis （P〈 0.01）. For SVD, there was a significant correlation between SVD and baPWV （2,057.6±456.57 cm/s in the SVD （＋） group vs. 1,491±271.62 cm/s in the SVD （-） group; P 〈 0.01）. However, the grade of abnormalities detected in SVD did not correlate linearly with baPWV. These findings show that baPWV is a reliable surrogate marker ofischemic stroke. Furthermore, baPWVand ABI can be used to indicate the presence of small vessel disease and large arterial disease, respectively.

Comparison of renal safety of tenofovir and entecavir in patients with chronic hepatitis B: Systematic review with meta-analysis

BACKGROUND Recently, the American Association for the Study of Liver Disease suggested no preference between tenofovir(TDF) and entecavir(ETV) regarding potential long-term risks of renal complications. Over the years, renal safety has become a critical concern in nucleos(t)ide analog-treated patients due to the long-term use of these drugs. However, existing studies do not show significant differences in renal dysfunction between these two drugs. Further, there is a paucity of studies comparing the long-term renal effects of TDF and *** To investigate the effects of TDF and ETV on renal function, we performed systematic review and *** Two investigators independently searched the Cochrane Library, MEDLINE, and Embase databases for randomized controlled trials and nonrandomized studies(NRSs) using the keywords 'CHB', 'Tenofovir', and 'Entecavir', and additional references were obtained from the bibliographies of relevant articles published through December 2017. The quality of each study was assessed using the Newcastle-Ottawa scale and the Grading of Recommendations Assessment,Development and Evaluation criteria. The primary outcome was the change in serum creatinine level in the TDF and ETV groups at baseline, 6 mo, 12 mo and24 *** NRSs comprising 2263 participants met the inclusion criteria. Changes in creatinine levels were higher in the TDF group than in the ETV group at 6 mo[mean difference(MD) = 0.03 mg/dL;95%CI: 0.02-0.04;I2 = 0%], 12 mo(MD =0.05 mg/dL;95%CI: 0.02-0.08;I2 = 78%), and 24 mo(MD = 0.07 mg/dL;95%CI:0.01-0.13;I2 = 93%). The change in estimated glomerular filtration rate(eGFR) was significantly higher in the TDF group than in the ETV group at 6 mo[standardized mean difference(SMD),-0.22;95%Cl:-0.36--0.08;I2 = 0%], 12 mo(SMD =-0.24;95%Cl:-0.43--0.05;I2 = 50%), and 24 mo(-0.35;95%Cl:-0.61--0.09;I2= 67%).CONCLUSION TDF statistically significantly increased serum creatinine levels and decreased the eGFR in 6-24 mo compa

研究了金属间化合物厚度对蜂窝状棒材热性能和力学性能的影响。在200℃下采用重复静液挤压制备Al/Cu蜂窝状棒材。在制备过程中,Al/Cu界面生成1μm厚的金属间化合物层。经420℃退火0.5-2 h后续热处理后,可获得不同厚度的金属间化合物层,最厚可达10.1μm。经鉴定,金属间化合物层为Al2Cu（θ）、AlCu（η2）和Al4Cu9（γ1）相。随着金属间化合物层厚度的增加,纵向和截面方向上的热导率分别减小11.9%（从（268±4.8）到（236±4.4）W/（m·K））和10.4%（从（210±3.2）到（188±2.8）W/（m·K））,Al/Cu蜂窝状棒材的抗拉强度和伸长率分别是（103±8.4）MPa和（73±6.2）%。当金属间化合物层厚度达到7.7μm时,抗拉强度增加至（131±6.5）MPa,随着金属间化合物层厚度增加至10.1μm时,抗拉强度降低至（124±3.9）MPa。随着金属间化合物层厚度增加,蜂窝状棒材的伸长率急剧下降至（29±2.5）%。

Quasi-type II heterostructured nanocrystals(NCs)have been of particular interest due to their great potential for controlling the interplay of charge ***,the lack of material choices for quasi-type II NCs restricts the accessible emission wavelength from red to near-infrared(NIR),which hinders their use in light-emitting applications that demand a wide range of visible ***,we demonstrate a new class of quasi-type II nanoemitters formulated in ZnSe/ZnSe_(1-X)Te_(X)/ZnSe seed/spherical quantum well/shell heterostructures(SQWs)whose emission wavelength ranges from blue to *** a given geometry,ZnSe_(1-X)Te_(X) emissive layers grown between the ZnSe seed and the shell layer are strained to fit into the surrounding media,and thus,the lattice mismatch between ZnSe_(1-X)Te_(X) and ZnSe is effectively *** addition,composition of the ZnSe_(1-X)Te_(X) emissive layer and the dimension of the ZnSe shell layer are engineered to tailor the distribution and energy of electron and hole wave *** from the capabilities to tune the charge carriers on demand and to form defectfree heterojunctions,ZnSe/ZnSe_(1-X)Te_(X)/ZnSe/ZnS NCs show near-unity photoluminescence quantum yield(PLQY>90%)in a broad range of emission wavelengths(peak PL from 450nm to 600 nm).Finally,we exemplify dichromatic white NC-based light-emitting diodes(NC-LEDs)employing the mixed layer of blue-and yellow-emitting ZnSe/ZnSe_(1-X)TeX/ZnSe/ZnS SQW NCs.

Impact of Bar-Code Medication Administration and Electronic Medication Administration Record System in Clinical Practice for an Effective Medication Administration Process

Clinical innovations are ideations resulting from collective experiences that enhance the “norm” and embrace an avenue for change with an invention. As such, collective data that were suggestive of increased medication errors that compromised patient safety initiated the exploration of methods that could reduce multifactorial human errors. The pursuit for an appropriate system followed with the discovery of barcode medication administration system (BCMA) and electronic medication administration system (e-MAR). Prior to the adoption of BCMA and e-MAR, it was crucial to assess the impact of the new medication administration system and the rate of medication administration errors recorded, specifically those that resulted in harm. The purpose of the study was to evaluate BCMA and e-MAR usage outcomes, clinical practices, policies, and processes impacting nurses administering medications in the clinical environment using BCMA and e-MAR system. Thus, an annotated literature review was conducted prior to the implementation of the innovation which analyzed various studies that elaborated on their study methods, data collection and analysis that deliberated on the advantages and disadvantages of barcode medication administration system. It is evident in the researched journals that increased compliance was observed with appropriate guidance, processes and policies in place. There was also a significant reduction in reported errors. The incorporation of barcode technology with electronic medication administration record (e-MAR) had greatly improved the efficiency of the BCMA system. BCMAs method was one of the proposed solutions to medication administration errors and to enhance patient safety measures. As such, the innovation could significantly reduce medication error resulting from intrinsic and extrinsic factors. Ionizing radiation is one of the most effective tools in cancer therapy. In a previous study, we reported that protein tyrosine kinase (PTK) inhibitors modulate the radiation responses in the human chronic myelogenous leukemia (CML) cell line K562. The receptor tyrosine kinase inhibitor, genistein, delayed radiation-induced cell death, while non-recepter tyrosine kinase inhibitor, herbimycin A (HMA) enhances radiation-induced apoptosis. In this study, we focused on the modulation of radiation-induced cell death by genistein and performed PCR-select suppression subtractive hybridization (SSH) to understand its molecular mechanism. We identified human thymidine kinase 1 (TK1), which is cell cycle regulatory gene and confirmed expression of TK1 mRNA by Northern blot analysis. Expression of TKl mRNA and TK1 enzymatic activity were parallel in their increase and decrease. TK1 is involved in G1-S phase transition of cell cycle progression. 公共外交是指一国政府通过文化交流、信息项目等形式,了解、获悉情况和影响国外公众,以提高本国国家形象和国际影响力,进而增进本国国家利益的外交方式。公共外交与传统外交不同之处在于,前者以文化和意识形态等“软实力”为手段与外国人进行沟通。传统意义的公共外交是指一国政府与国外民众的外交形式,它的活动手段包括文化交流项目、国际广播和互联网等,其主要表达形式是信息和语言。而在当今日新月异的互联网时代,公众通过互联网等现代信息手段能够更广泛地参与到公共外交活动中来,或者有更多的公众行为具有公共外交效果。特别韩国的公共外交是除了中央政府推进的公共外交,民间也开展了多种形态的公共外交。这篇论文是公共外交理论的基本内容进行介绍,在此基础对韩国公共外交的基本情况进行概述,并阐述电视剧在韩国公共外交中的地位。以电视剧为例,分析韩国对华公共外交的发展演变过程,主要是分为四个阶段,分别是2000年至2005年,2006年至2010年,2011年至2015年,2016年至今。重点梳理每一个阶段韩国对华公共外交存在怎样的特点。探讨为什么会出现发展演变过程,影响韩国对华公共外交发展演变的因素有哪些?政治关系、经济贸易、文化政策等因素都可能会影响到上述的演变过程。因此,论文要分析韩国对中国公共外交的模式和韩国公共外交的特征,以及公共外交的文化交流对两国产生了什么样的影响、潜在的问题是什么、还有公共外交发展的现状和对政治、经济、文化的影响的研究,并对公共外交推进的现状进行了分析,探索其方向发展。通过分析本文认为在对中国韩剧公共外交中,政治关系友好是公共外交开展的前提,经济贸易频繁是公共外交发展的动力,文化政策规范是公共外交持续的力量。

Glucose metabolism and neurogenesis in the gerbil hippocampus after transient forebrain ischemia

Recent evidence exists that glucose transporter 3（GLUT3） plays an important role in the energy metabolism in the *** previous studies have been conducted using focal or hypoxic ischemia models and have focused on changes in GLUT3 expression based on protein and m RNA levels rather than tissue *** the present study,we observed change in GLUT3 immunoreactivity in the adult gerbil hippocampus at various time points after 5 minutes of transient forebrain *** the sham-operated group,GLUT3 immunoreactivity in the hippocampal CA1 region was weak,in the pyramidal cells of the CA1 region increased in a time-dependent fashion 24 hours after ischemia,and in the hippocampal CA1 region decreased significantly between 2 and 5 days after ischemia,with high level of GLUT3 immunoreactivity observed in the CA1 region 10 days after *** a double immunofluorescence study using GLUT3 and glial-fibrillary acidic protein（GFAP）,we observed strong GLUT3 immunoreactivity in the ***3 immunoreactivity increased after ischemia and peaked 7 days in the dentate gyrus after ischemia/*** a double immunofluorescence study using GLUT3 and doublecortin（DCX）,we observed low level of GLUT3 immunoreactivity in the differentiated neuroblasts of the subgranular zone of the dentate gyrus after ***3 immunoreactivity in the sham-operated group was mainly detected in the subgranular zone of the dentate *** results suggest that the increase in GLUT3 immunoreactivity may be a compensatory mechanism to modulate glucose level in the hippocampal CA1 region and to promote adult neurogenesis in the dentate gyrus.

BACKGROUND Even at present,we are in the middle of the novel coronavirus disease 2019(COVID-19)pandemic and are facing challenges in trial and ***,emergency surgery for patients with suspected COVID-19 is burdensome not only for patients but also for healthcare ***,we established a surveillance system in the emergency room and established principles for managing patients suspected of COVID-19 who require emergency *** SUMMARY A 67-year-old man was diagnosed with appendicitis in March *** wife was diagnosed with COVID-1910 d earlier,and the patient was in close contact with *** patient tested negative twice on an upper respiratory COVID-19 reverse transcription–polymerase chain reaction screening test,but chest X-ray and chest computed tomography revealed patchy ground-glass opacity in both upper lobes of the patient’s *** same emergency surgery procedure for patients with confirmed COVID-19 was applied to this patient suspected of having the disease to ensure that surgery was not delayed while waiting for the reverse transcription–polymerase chain reaction results.A few hours after surgery,the upper respiratory tract specimen taken in the emergency room was negative for COVID-19 but the lower respiratory tract specimen was found to be positive for the *** When COVID-19 is suspected,emergency surgery should be performed as for confirmed COVID-19 without delay.
