Familial adenomatous.polypos.s.(FAP) is.an autos.mal dominant inherited s.ndrome characterized by multiple adenomatous.polyps.(predis.os.ng to colorectal cancer development) and numerous.extracolonic manifes.ations. The underlying genetic burden generates.variable clinical features.that may influence operative management. As.a precancerous.hereditary condition, the rationale of performing a prophylactic s.rgery is.a mains.ay of FAP management. The purpos. of the pres.nt paper is.to bring up many controvers.al as.ects.regarding s.rgical treatment for FAP, and to dis.us. the res.lts.and pers.ectives.of the operative choices.and approaches. Preferably, the decis.on-making proces. s.ould not be limited to the conventional confrontation of pros.and cons.of ileorectal anas.omos.s.or res.orative proctocolectomy. A wide dis.us.ion with the patient may evaluate is.ues.s.ch as.age, genotype, family his.ory, s.hincter function, the pres.nce or ris. of des.oid dis.as., potential complications.of each procedure and chances.of pos.operative s.rveillance. Therefore, the definition of the bes. moment and the choice of appropriate procedure cons.itute an individual decis.on that mus. take into cons.deration patient’s.preferences.and full information about the complex nature of the dis.as.. All thes. facts.reinforce the idea that FAP patients.s.ould be managed by experienced s.rgeons.working in s.ecialized centers.to achieve the bes. immediate and long-term res.lts.

Bridging the gaps.between technology and clinical daily practice, the lates. development and the almos. humble beds.de care, has.been a challenge for the increas.ng numbers.of doctors.devoted to the diagnos.ic and treatment of liver dis.as.. We have been trying to be very pers.as.ve among the Portugues. medical community in demons.rating that cultural vectors.may influence the origin and pattern of liver dis.as. among us. The complexity of s.diment dynamics.in aquatic s.s.ems.can be better unders.ood by applying numerical models. The development of a comprehens.ve morphological model is.pres.nted in this.*** model aims.to predict the s.diment trans.ort and bed evolution in natural s.s.ems.compos.d of different s.diment types. The morphological model was.implemented in MOHID, a modelling s.s.em that s.lves.the three-dimens.onal hydrodynamics.and advection-diffus.on trans.ort of s.s.ended s.diments. Multiple s.diment clas.es.were taken into account(non-cohes.ve and cohes.ve) cons.dering the effects.of s.diment mixtures.and bed cons.lidation on res.s.ance to eros.on. To repres.nt bottom s.ratigraphy, the bottom column can be divided into s.veral layers. The key points.of the s.mulated proces.es.are dis.us.ed in this.paper. Model res.lts.are as.es.ed in s.x tes. cas.s.through comparis.n with analytic s.lutions.or experimental data.
