院内卒中救治绿色通道建立、优化及区域救治网络的建立

目的机械取栓时代,探索如何因地制宜建立急性卒中救治体系,让更多急性卒中患者能在时间窗内及时到达有条件的医院,从而提高区域性急性卒中急诊救治的整体水平。方法从2014年开始,在院内及医院所在的区域,由单位行政部门及区域卫生行政部门牵头协调,制定卒中救治的相关政策、规定,陆军军医大学第一附属医院神经内科作为技术主导,开展卒中诊治的培训及帮助各级部门、医院建立相应的卒中救治流程、规范。观察我院卒中救治的关键绩效指标及协作单位救治水平的改善情况。结果 (1)院内急诊通道的建立:由院长亲自参与,院医疗科作为卫生行政部门进行督导、协调,院内参与卒中救治的急救部、神经内科、神经外科、神经影像科、检验科、心电图室及护理保障中心积极参与,使急性缺血性卒中的入院-静脉溶栓(door-to-needle time,DNT)时间由绿色通道建立前的71 min缩短至53 min,入院-动脉穿刺(door-to-puncture time,DPT)时间由104 min缩短至82 min,入院-血运重建(door-to-revascularization time,DRT)时间由144 min缩短至117 min。(2)区域性卒中救治网络的建立:极大提高我院急性缺血性卒中的救治能力,2014年本科静脉溶栓及血管内治疗总病例数为7例,2016年提高到112例,区域内初级卒中中心的卒中防治关键绩效指标明显改善。结论院内卒中救治绿色通道及区域性卒中救治网络的建立,能提高卒中救治水平及救治率,改善区域性卒中救治水平。应进一步改进卒中救治通道及网络并进行更大范围的推广。

A deterministic quantum key distribution scheme using two non-orthogonal entangled states is proposed. In the proposed scheme, communicators share key information by exchanging one travelling photon with two random and secret polarization angles. The security of the distributed key is guaranteed by three checking phases in three-way channel and the communicators＇ secret polarization angles.

Collisions between a moving mass and an anti-collision device increase structural responses and threaten structural *** active mass damper(AMD)with stroke limitations is often used to avoid ***,a strokelimited AMD control system with a fixed limited area shortens the available AMD stroke and leads to significant control *** solve this problem,the design approach with variable gain and limited area(VGLA)is proposed in this ***,the boundary of variable-limited areas is calculated based on the real-time status of the moving *** variable gain(VG)expression at the variable limited area is deduced by considering the saturation of AMD ***,numerical simulations of a stroke-limited AMD control system with VGLA are conducted on a high-rise building *** numerical simulations show that the proposed approach has superior strokelimitation performance compared with a stroke-limited AMD control system with a fixed limited ***,the proposed approach is validated through experiments on a four-story steel frame.

When the time variable in quantum signal processing is discrete, the Fourier transform exists on the vector space of n-tuples over the Galois field F2, which plays an important role in the investigation of quantum signals. By using Fourier transforms, the idea of quantum coding theory can be described in a setting that is much different from that seen that far. Quantum BCH codes can be defined as codes whose quantum states have certain specified consecutive spectral components equal to zero and the error-correcting ability is also described by the number of the consecutive zeros. Moreover, the decoding of quantum codes can be described spectrally with more efficiency.

Studies have shown that micro-wedge vortex generators(MVG)can effectively control the flow separation of supersonic boundary *** order to improve the flight stability of spinning projectile,the original standard 155 mm projectile was taken as an example,and the micro-vanes were mounted at the projectile shoulder to investigate the separation control on the aerodynamic characteristics of *** simulations were performed with the use of DES method for the flow fields of projectiles with and without micro-vanes,and the characteristics of the boundary layer structures and aerodynamic data were compared and *** results show that the micro-vanes can be used to inhibit separation of fluid on projectile surface,and improve the flight stability and firing dispersion of projectile.

目的探讨Ⅱ~Ⅲ期胃癌患者根治术中淋巴结清扫数目与远期复发、死亡的关系。方法回顾性分析2018年12月至2020年12月期间本院收治的186例Ⅱ~Ⅲ期胃癌并行根治术患者的临床资料,并对患者随访至2022年12月。根据国际抗癌联盟要求和术中淋巴结清扫数目分为≥25枚组(88例)和15~25枚组(98例),在控制性别、年龄、肿瘤大小、肿瘤部位等变量的前提下,采用COX风险回归分析淋巴结清扫数目与胃癌患者远期复发和死亡的关系。结果随访2年后,186例患者中胃癌复发61例,死亡38例,复发率和死亡率分别为32.80%和20.43%。单因素分析显示,≥25枚组患者胃癌的复发率为25.00%低于15~25枚组的39.80%(P<0.05);≥25枚组患者的死亡率为12.50%低于15~25枚组的27.55%(P<0.05);COX风险回归分析显示,淋巴结清扫数目是胃癌患者远期复发和死亡的危险因素(HR=2.776、1.669,P<0.05)。结论 Ⅱ~Ⅲ期胃癌患者根治术中淋巴结清扫数目与远期复发、死亡显著相关,且淋巴结清扫数目≥25枚患者胃癌远期复发、死亡的风险较低。

The Effects of Dietary Fatty Acid Pattern on Layer's Performance and Egg Quality

The feeding trial was conducted to determine the effects of flaxseed on layer＇s performance and egg quality, and the effects of the dietary ratio of ω-6 fatty acid to ω-3 fatty acid on fatty acid deposition in yolk using flaxseed as resource of ω-3 fatty acid and sunflower seed as resource of ω-6 fatty acid. 192 20-wk-old Newroman commercial hens were randomly divided into 4 treatment groups with 6 replicates containing 8 birds. The rations corresponding with the four treatments were formulated with 4% flaxseed ＋ 9% sunflower seed, 8% flaxseed ＋ 18% sunflower seed, 15% flaxseed, and soybean meal, respectively. Sunflower seed was unshelled and flaxseed was whole. Ratios of ω-6 fatty acid to ω-3 fatty acid in 4 dietary treatments were 5.67, 5.21, 1.07, and 11.83, respectively. It can be concluded that long-term feeding 15% flaxseed had no significant （P〉 0.05） influence on egg production, egg weight, eggshell quality, yolk weight, and percentage of yolk weight. Dietary ratio of ω-6 fatty acid to ω-3 fatty acid had impact on the 0-3 fatty acid levels in yolk, nevertheless yolk fatty acid profiles were different from those of dietary fatty acid compositions.
