Long-term outcomes and failure patterns after laparoscopic intersphincteric resection in ultralow rectal cancers

BACKGROUND Intersphincteric resection(ISR),the ultimate anus-preserving technique for ultralow rectal cancers,is an alternative to abdominoperineal resection(APR).The failure patterns and risk factors for local recurrence and distant metastasis remain controversial and require further *** To investigate the long-term outcomes and failure patterns after laparoscopic ISR in ultralow rectal *** Patients who underwent laparoscopic ISR(LsISR)at Peking University First Hospital between January 2012 and December 2020 were retrospectively *** analysis was performed using the Chi-square or Pearson's correlation *** factors for overall survival(OS),local recurrence-free survival(LRFS),and distant metastasis-free survival(DMFS)were analyzed using Cox *** We enrolled 368 patients with a median follow-up of 42 *** recurrence and distant metastasis occurred in 13(3.5%)and 42(11.4%)cases,*** 3-year OS,LRFS,and DMFS rates were 91.3%,97.1%,and 90.1%,respectively Multivariate analyses revealed that LRFS was associated with positive lymph node status[hazard ratio(HR)=5.411,95%confidence interval(CI)=1.413-20.722,P=0.014]and poor differentiation(HR=3.739,95%CI:1.171-11.937,P=0.026),whereas the independent prognostic factors for DMFS were positive lymph node status(HR=2.445,95%CI:1.272-4.698,P=0.007)and(y)pT3 stage(HR=2.741,95%CI:1.225-6.137,P=0.014).CONCLUSION This study confirmed the oncological safety of LsISR for ultralow rectal *** differentiation,(y)pT3 stage,and lymph node metastasis are independent risk factors for treatment failure after LsISR,and thus patients with these factors should be carefully managed with optimal neoadjuvant therapy,and for patients with a high risk of local recurrence(N+or poor differentiation),extended radical resection(such as APR instead of ISR)may be more effective.

In this paper,we consider the derivatives of intersection local time for two independent d-dimensional symmetricα-stable processes X^(α) and X^(α)with respective indices α and α.We first study the sufficient condition for the existence of the derivatives,which makes us obtain the exponential integrability and H?lder *** we show that this condition is also necessary for the existence of derivatives of intersection local time at the ***,we also study the power variation of the derivatives.

Wear Mechanism of Cemented Carbide Tool in High Speed Milling of Stainless Steel

Adhesion of cutting tool and chip often occurs when machining stainless steels with cemented carbide tools. Wear mechanism of cemented carbide tool in high speed milling of stainless steel 0Cr13Ni4 Mo was studied in this work. Machining tests on high speed milling of 0Cr13Ni4 Mo with a cemented carbide tool are conducted. The cutting force and cutting temperature are measured. The wear pattern is recorded and analyzed by high?speed camera, scanning electron microscope(SEM) and energy dispersive X?ray spectroscopy(EDS). It is found that adhesive wear was the dominant wear pattern causing tool failure. The process and microcosmic mechanism of the tool’s adhesive wear are analyzed and discussed based on the experimental results. It is shown that adhesive wear of the tool occurs due to the wear of coating, the a nity of elements Fe and Co, and the grinding of workpiece materials to the tool material. The process of adhesive wear includes both microcosmic elements di usion and macroscopic cyclic process of adhe?sion, tearing and fracture.

Numerical investigation of non-uniform temperature fields for proppant and fluid phases in supercritical CO_(2)fracturing

The non-uniform temperature distribution in supercritical CO_(2)(Sc-CO_(2))fracturing influences the density,viscosity,and volume expansion or shrinkage rate of Sc-CO_(2),impacting proppant *** study presents a coupled computational fluid dynamics-discrete element method and heat transfer model to examine the effects of proppant bed shape and the heat transfers of proppant-wall,proppant-fluid,and fluid-wall on the fluid and proppant temperature *** Sc-CO_(2)volume expansion is assessed under various temperature conditions by evaluating the volume-averaged Sc-CO_(2)*** factors,including proppant size,shape,thermal conductivity,concentration,temperature difference,and injection velocity,are carefully analyzed to elucidate their *** findings elucidate the existence of four distinct zones in the fluid temperature *** zone exhibits different magnitudes of temperature change under diverse conditions and undergoes dynamic transformations with the development of the proppant *** fluid-wall heat transfer and the fluid temperatures in Zones C and D are significantly subject to the fluid injection velocity(governing the heating duration),the temperature difference between fluid and formation(impacting the magnitude of heat flux),and the proppant bed shape(controlling the effective heating area).Additionally,the proppant-wall and proppant-fluid heat transfers determine the temperatures of both the proppant bed and the fluid within Zone B,showing a strong correlation with proppant thermal conductivity,proppant size,injection velocity,and temperature *** proposed coupled model provides valuable insights into the temperature distributions and flow behaviors of temperature-dependent fracturing fluids and proppants.

A Novel Design of a Pre-separation Panel Structure to Protect Against the Impact of an Explosion in an Aircraft Plenum Chamber

To reduce the damage of the pressurizing panel structure of a fuselage caused by an explosion at the“least risk bomb location”in an aircraft structure,a new pre-separation panel structure was designed to resist blast ***,the dynamic strain response and morphology of impact damage of the new pre-separation panel were measured in an impact damage ***,the commercial software LS-DYNA was used to calculate the propagation of the blast shock wave,and the results were compared with empirical equations to verify the rationality of the numerical calculation ***,the fluid–structure coupling method was used to calculate the damage process of the pre-separation panel structure under the impact of an explosion wave and an impact *** calculated results were in good agreement with the test results,which showed the rationality of the calculation method and the *** residual strength of the damaged pre-separation panel was significantly higher than that of the original damaged *** results show that the new pre-separation panel structure is reasonable and has certain significance for guiding the design of plenum chambers with strong resistance to implosion for aircraft fuselages.

Preparation, characterization and photocatalytic activity of N-doped TiO_2 nanocrystals

N-doped TiO 2 nanocrystals were prepared using titanium alkoxide as precipitant with different proportional materials. The products were characterized by X-ray diffraction, scanning electron microscopy, transmission electron microscopy, and UV-vis diffuse reflectance spectra. It is confirmed experimentally that the photocatalytic activity of N-doped TiO 2 is much higher than that of Degussa P25, when used for the degradation of crystal violet. The degradation kinetics follows an apparent first-order reaction, which is consistent with a generally observed Langmuir-Hinshelwood mechanism. The doping of TiO 2 with nitrogen significantly increases the absorption in the region of visible light. The energy of the band gap of N-doped TiO 2 is 2.92 eV. The better performance of N-doped TiO 2 can be explained by the fact that it is also excited with longer-wavelength light.

A Novel Disturbance Observer Based Fixed-Time Sliding Mode Control for Robotic Manipulators With Global Fast Convergence

This paper proposes a new global fixed-time sliding mode control strategy for the trajectory tracking control of uncertain robotic ***,a fixed-time disturbance observer(FTDO) is designed to deal with the adverse effects of model uncertainties and external disturbances in the manipulator *** an adaptive scheme is used and the adaptive FTDO(AFTDO) is developed,so that the priori knowledge of the lumped disturbance is not ***,a new non-singular fast terminal sliding mode(NFTSM) surface is designed by using an arctan function,which helps to overcome the singularity problem and enhance the robustness of the *** on the estimation of the lumped disturbance by the AFTDO,a fixed-time non-singular fast terminal sliding mode controller(FTNFTSMC)is developed to guarantee the trajectory tracking errors converge to zero within a fixed *** settling time is independent of the initial state of the *** addition,the stability of the AFTDO and FTNFTSMC is strictly proved by using Lyapunov ***,the fixed-time NFESM(FTNFTSM) algorithm is validated on a 2-link manipulator and comparisons with other existing sliding mode controllers(SMCs) are *** comparative results confirm that the FTNFTSMC has superior control performance.

Non-contact and full-field online monitoring of curing temperature during the in-situ heating process based on deep learning

Online monitoring of the curing temperature field is essential to improving the quality and efficiency of the manufacturing process of composite *** embedded sensor-based technologies have difficulty monitoring the full temperature field or have to introduce heterogeneous items that could have an undesired impact on the *** this paper,a non-contact,full-field monitoring method based on deep learning that predicts the internal temperature field of composite parts in real time using surface temperature measurements of auxiliary materials is *** the proposed method,an average temperature monitoring accuracy of 97%is achieved in various heating *** addition,this method also demonstrates satisfying feasibility when a stronger thermal barrier covers the *** method was experimentally validated during the self-resistance electric heating process,in which the monitoring accuracy reached 93.1%.This method can potentially be applied to automated manufacturing and process control in the composites industry.

Humanized immune system(HIS)mice have been developed and used as a small surrogate model to study human immune function under normal or disease *** variations are found between models,most HIS mice show robust human T cell ***,there has been unsuccessful in constructing HIS mice that produce high-affinity human antibodies,primarily due to defects in terminal B cell differentiation,antibody affinity maturation,and development of primary follicles and germinal *** this review,we elaborate on the current knowledge about and previous attempts to improve human B cell development in HIS mice,and propose a potential strategy for constructing HIS mice with improved humoral immunity by transplantation of human follicular dendritic cells(FDCs)to facilitate the development of secondary follicles.
