Objective With few studies investigating the effectiveness of telemedicine(TM)in pediatric otolaryngology(ear,nose,and throat;ENT),its role in clinical practice is *** objective of this study was to investigate provider perspectives regarding utility of TM in pediatric ENT *** A survey gauging the relative merits of TM visits for common pediatric ENT chief complaints and postoperative visits was distributed to all pediatric ENT providers at a tertiary care,free-standing children’s *** were asked to assess the effectiveness of TM visits compared with in-person visits for completing the following tasks:history collection,physical examination,medical decision-making,and patient *** Providers rated TM visits as less useful than in-person visits for completing the most predefined tasks but did identify advantages in history taking via TM for the majority of *** with providers with≥10 years of experience,those withs of experience found TM to be more effective than the in-person appointment for making clinical decisions for patients presenting with recurrent/chronic pharyngitis,neck masses,and stridor/noisy *** regarding the utility of TM for postoperative visits were mixed,with adenoidectomy,tonsillectomy and superficial procedures being most frequently deemed appropriate for *** The introduction of TM to pediatric ENT faces limitations in detailed examination of areas not accessible without specialized *** to its strength in history taking,results suggest an asynchronous,‘store and forward’encounter followed by an in-person physical examination to confirm the diagnosis and treatment plan could be beneficial.

Quantifying relations between altered hydrology and fish community responses for streams in Minnesota

Purpose The constant work-rate to exhaustion tests must be repeated several times at each work-rate to estimate subject-level trial-to-trial variance(intra-individual variability,IIV)of critical power(CP)and work capacity(W').Alternatively,these parameters and their variance can be estimated by repeating the 3-min all-out test(3MT)fewer *** purpose of this study was to propose a method to determine subject-level repeatability of the 3MT and demonstrate the need to repeat the test multiple times to estimate *** seven cyclists performed a ramp test and four 3MTs on a *** parameters CP,W',peak power(Pp),and total work(TW)were compared across trials using repeated measures ANOVA,Bland-Altman analysis,Intraclass Cor-relation Coefficients(ICC),Typical Error(TE)of measurement,and Coefficient of Variation(CV).Results For the group,average CP and W'were 284±58 W and 10.214±3.143 *** reliability statistics,CP(ICC=0.97,TE=8 W,CV=2.94%)and W'(ICC=0.88,TE=1.11 kJ,CV=10.87%),indicated strong ***-level repeatability was determined by comparing time-to-peak power(TPp),absolute difference in Pp(δPp),and TW(δTW)for pairs of *** average IIVs estimated by the 95%confidence intervals were±15 W for CP and±1.68 kJ for W'.Conclusions Thresholds are proposed for TPp(7 s),δPp(10%),andδTW(3%)to determine subject-level repeatability of the 3MT before computing the IIV of CP and W'.It is suggested that the 3MT is repeated at least three times to estimate the IIV,which aids in personalized measurement of training improvements and performance optimization。

A mathematical model has been developed to numerically model the risk of developing Alzheimer’s disease and Chronic Traumatic Encephalopathy (CTE) as a person ages. The model was programmed in Excel to provide a working prototype computer simulation model. The model provides estimates of the cumulative risk of developing Alzheimer’s disease and CTE as age increases. A one-year step size was used. The model has two major parts: one predicts changes in slow-wave sleep as a person ages and the second component adjusts the flushing efficiency of waste products from the brain. The two components work together and interact to lower the flushing of waste components as age increases. The development of the model provides an overview of how the various factors work together that lead to the onset of Alzheimer’s disease and the associated CTE. Calibration of the coefficients in the model is based on published data sets presented in the literature. Further research and refinement of calibration coefficients should be explored.
