Breath test for differential diagnosis between small intestinal bacterial overgrowth and irritable bowel disease:An observation on non-absorbable antibiotics

AIM： To estimate the prevalence of small intestine bacterial overgrowth （SIBO） among patients with an earlier diagnosis of irritable bowel disease （IBS） in our geographical area, and to collect information on the use of locally acting non-absorbable antibiotics in the management of SIBO. METHODS： A non-interventional study was conducted in 73 consecutive patients with a symptom-based diagnosis.. RESULTS： When the patients underwent a ＂breath test＂, 33 （45.2%） showed the presence of a SIBO. Arcer treatment with rifaximin 1200 mg/d for seven days in 32 patients, 19 （59.4%） showed a negative ＂breath test＂ one week later as well as a significant reduction of symptoms, thus confirming the relationship between SIBO and many of the symptoms claimed by patients. In the other 13 patients, ＂breath test＂ remained positive, and a further cycle of treatment with ciprofloxacin 500 mg/d was given for 7 additional days, resulting in a negative ＂breath test＂ in one patient only. CONCLUSION：（1） about half of the patients with a symptomatic diagnosis of IBS have actually SIBO, which is responsible for most of the symptoms attributed to IBS; （2） only a ＂breath test＂ with lactulose （or with glucose in subjects with an intolerance to lactose） can provide a differential diagnosis between IBS and SIBO, with almost identical symptoms; and （3） the use of non-absorbable antibiotics may be useful to reduce the degree of SIBO and related symptoms; it must be accompanied, however, by the correction of the wrong alimentary habits underlying SIBO.

Industrial discharge water, and especially that from the surface treatment industry (ST), contains non-negligible amounts of pollutants even though the legislation is fully respected. In spite of this, no detailed studies list the exact chemical composition of these effluents. The present study reports the results of analyses performed over a 6-month period involving 15 standard water parameters. Over 160 substances including 33 metals, 58 volatile organic compounds (VOCs), 16 polycyclic aromatic hydrocarbons (PAHs), 24 chlorophenols (CPs), 16 alkylphenols (APs), 5 chloroanilines (CAs) and 7 polychlorobiphenyls (PCBs) were monitored. The industrial effluents presented polycontamination involving metals, minerals and organics with a high degree of qualitative and quantitative variability. Of the 160 substances monitored, 46 were regularly found: 25 inorganics including 8 metals (Co, Cr, Cu, Fe, Ni, Pb, Sn, Zn) and 21 organics (4 PAHs, 10 VOCs, 4 CPs and 3 APs). Eighteen were systematically presented at quantifiable levels.

An historical collection of more than one hundred samples of minerals and ore, used in the second half of the XVIII century was found and acquired during Munich Mineralientage 2014. The samples contained in numbered glass vials but lacking description, were prepared for teaching purpose about determinative mineralogy and ore recognition. All samples were analysed and identified. The identification effort drove the authors along a historical excursus about the didactics of mineralogy and the dry method analysis, nowadays neglected.

BACKGROUND Patients with pancreatic cystic neoplasms(PCN), without surgical indication at the time of diagnosis according to current guidelines, require lifetime imagebased surveillance follow-up. In these patients, the current European evidencedbased guidelines advise magnetic resonance imaging(MRI) scanning every 6 mo in the first year, then annually for the next five years, without reference to any role for trans-abdominal ultrasound(US). In this study, we report on our clinical experience of a follow-up strategy of image-based surveillance with US, and restricted use of MRI every two years and for urgent evaluation whenever suspicious changes are detected by *** To report the results and cost-efficacy of a US-based surveillance follow-up for known PCNs, with restricted use of *** We retrospectively evaluated the records of all the patients treated in our institution with non-surgical PCN who received follow-up abdominal US and restricted MRI from the time of diagnosis, between January 2012 and January 2017. After US diagnosis and MRI confirmation, all patients underwent US surveillance every 6 mo for the first year, and then annually. A MRI scan was routinely performed every 2 years, or at any stage for all suspicious US *** this communication, we reported the clinical results of this alternative followup, and the results of a comparative cost-analysis between our surveillance protocol(abdominal US and restricted MRI) and the same patient cohort that has been followed-up in strict accordance with the European guidelines recommended for an exclusive MRI-based surveillance *** In the 5-year period, 200 patients entered the prescribed US-restricted MRI surveillance follow-up. Tikuna indigenous believe that the saltlicks are natural sacred spaces in the Amazon Rainforest. Those are health and nutrition spaces of important species of fauna, especially mammals and birds. The perception, knowledge, usage and spiritual management of saltlicks for Tikuna help the conservation of more than 130 species of flora identified in forest inventories. Saltlicks forests have few studies in the Colombian Amazon region. The objective of this study was to identify and compare the composition and structure in a sample of 6 saltlicks, and control forests, present in the perimeter of each one of them, to know if they have similarities or differences. The genres (or species as far as possible) and frequent botanical families were identified. The information was obtained through fieldwork with inventories carried out in the saltlicks and control forests in the years 2015, 2016 and 2017, with the help of local guides and knowledgeable people about flora of Tikuna community (gregorio family, Cascabel clan), San Martin de Amacayacu village, TICOYA resguardos. In the saltlicks sample, 336 trees in 29 families were identified. The families with more number of trees are Arecaceae, Myristicaceae and Fabaceae. In control forests 345 trees in 27 families were identified;the families with more number of trees are Arecaceae, Myristicaceae, and Sapotaceae. Control forests present a little higher value of richness and diversity that saltlick forests, but they are not statistically significant. The family Arecaceae is the one that predominates in the 6 saltlicks and control forests inventories without being dominant. Control forests have higher values of richness and diversity, but they are not statistically significant. In both samples of inventoried forests, they have little dominance of species. Their species have equity in the saltlicks and control forests.

Effect of Solution Concentration on the Electrospray/Electrospinning Transition and on the Crystalline Phase of PVDF

A study was conducted regarding the effect of concentration of poly (vinylidene fluoride) (PVDF)/N,N-dimethylformamide (DMF) and PVDF/DMF/acetone solutions on the transition between electrospray and electrospinning and on the formation of the ? and ? crystalline phases of PVDF. The crystalline phases present in the samples, crystallinity and morphology were determined by Fourier transform infrared spectroscopy (FTIR), differential scanning calorimetry (DSC) and scanning electron microscopy (SEM), respectively. Low concentration solutions resulted in films consisting of small droplets (electrospray) containing predominantly the ? phase. High concentration solutions resulted in a non-woven mesh of nano-to-micron diameter fibers (electrospinning) containing exclusively the ? phase. These results showed that, the formation of this phase in the electrospinning is related mainly to the solvent evaporation rate, and not to drawing experienced by the polymer during the process. Solvent type affected the amount of crystalline phase present, the boundary concentration between the two processes and the average diameter of fibers. Meshes processed by electrospinning display a degree of crystallinity higher than the films obtained by electrospray.
