Pan-enteric dysmotility,impaired quality of life and alexithymia in a large group of patients meeting ROmEⅡ criteria for irritable bowel syndrome

AIm:Psychological factors, altered motility and sensation disorders of the intestine can be variably associated with irritable bowel syndrome (IBS). Such aspects have not been investigated simultaneously. The aim of this paper was to evaluate gastrointestinal motility and symptoms, psychological spectrum and quality of life in a large group of IBS patients in southern ***: One hundred IBS patients (F:m=73:27, age 48±2 years, mean±SE) fulfilling ROmE Ⅱ criteria matched with 100 healthy subjects (F:m=70:30, 45±2 years). Dyspepsia,bowel habit, alexithymia, psycho-affective profile and quality of life were assessed using specific questionnaires. Basally and postprandially, changes in gallbladder volumes and antral areas after liquid meal and orocaecal transit time (OCTT)were measured respectively by ultrasonography and H2-breath test. Appetite, satiety, fullness, nausea, and epigastric pain/discomfort were monitored using visual-analogue ***: Compared with controls, IBS patients had increased dyspepsia (score 12.6±0.7 VS 5.1±0.2, P＜0.0001),weekly bowel movements (12.3±0.4 VS 5.5±0.2, P＜0.00001, comparable stool shape), alexithymia (score 59.11.1 VS40.5±1.0, P=0.001), poor quality of life and psychoaffective profile. IBS patients had normal gallbladderemptying, but delayed gastric emptying (T50:35.5±1.0 VS 26.1±0.6 min, P=0.00001) and OCTT (163.0±5.4 VS96.6±1.8min, P=0.00001). Fullness, nausea, and epigastric pain/discomfort were greater in IBS than in ***: ROmE Ⅱ IBS patients have a pan-enteric dysmotility with frequent dyspepsia, associated with psychological morbidity and greatly impaired quality of *** presence of alexithymia, a stable trait, is a novel finding of potential interest to detect subgroups of IBS patients with different patterns recoveed after therapy.

Diagnostic delay in oral squamous cell carcinoma: the role of cognitive and psychological variables

This retrospective study investigated, in two cohorts of subjects living in Southern Italy and awaiting treatment for oral squamous cell carcinoma （OSCC）, the variables related to diagnostic delay ascribable to the patient, with particular reference to the cognitive and psychological ones. A total of 156 patients with OSCC （mean age： 62 years, m/F： 2.39 ： 1） were recruited at the Universities of Palermo and Naples. Risk factors related to patient delay included： sociodemographic, health-related, cognitive and psychological variables. The analysis was conducted by considering two different delay ranges： dichotomous （≤1 month vs. 〉 1 month） and polytomous （〈1 month, 1-3 months, 〉3 months） delay. Data were investigated by univariate and multivariate analyses and a Pvalue≤0.05 was considered statistically significant. For both delay measurements, the most relevant variables were：＇Personal experience of cancer＇（dichotomous delay： P=0.05, odds ratio （0R）=0.33, 95% confidence interval （CI）=0. 11-0.99; polytomous delay： P=0.006, Chi-square= 10.224） and ＇Unawareness＇（dichotomous delay： P〈0.01, 0R=4.96, 95% CI--2.16-11.37; polytomous delay： P=0.087, Chi-square=4.77）. Also ＇Denial＇（P〈0.01, 0R=6.84, 95% CI=2.31-20.24） and ＇Knowledge of cancer＇（P=0.079, Chi-square=8.359） were found to be statistically significant both for dichotomous and for polytomous categorization of delay, respectively. The findings of this study indicated that, in the investigated cohorts, the knowledge about cancer issues is strongly linked to the patient delay. Educational interventions on the mediterranean population are necessary in order to increase the patient awareness and to emphasize his/her key role in early diagnosis of OSCC.

Background: Inland wetlands are crucial for biodiversity conservation, especially in highly-urbanized landscapes. In the European Union, many wetlands are included in the EU ‘Natura 2000’ network, the main tool for biodiversity conservation over the continent, which requires the development of site-specific management plans. Clear and feasible recommendations are necessary to provide site managers with effective tools for the maintenance of biodiversity in these unstable environments. Birds are excellent umbrella species, therefore a management targeted at increasing habitat suitability for focal bird species would likely benefit broader wetland biological communities. methods: During spring–summer 2017, we collected presence/absence data for 10 bird species of conservation interest at a site scale for 21 Natura 2000 sites. We also carried out a point count survey to detect presence/absence of four reedbed-dwelling species at 75 points. At the site level, we estimated landscape characteristics from regional GISlayers, whereas fine-scaled habitat composition was recorded on the field within a 100 m-buffer around the 75 points. We analysed the effect of the extent of different habitats on species’ occurrence probability by means of multi-species binomial multivariate adaptive regression splines (mARS) at both scales. We also run species-specific mARS models to compare their performance with those of multi-species models. Results: At the site scale, the extent of the reedbeds/mires was positively associated with the occurrence of all species of conservation concern. At the point-count scale, reedbed extent positively predicted species’ occurrence, but only in presence of patches of clear shallow water. Species-specific mARS models showed qualitatively similar results for some species, but generally were outperformed by multi-species ones. The Phlegrean Fields are an area in the west of Naples （Italy）, with a huge interest in geophysical community being a volcanic caldera among the most dangerous in the world. Various techniques of monitoring exist. Among all, the control of ground deformations and variations in sea level has considerable importance. Time series of ground deformation and tidal data in this area have been analysed to highlight these important geophysical features and these results are compared with those obtained from similar data in other time periods. With regard to first mentioned, tiltmetric data have been analysed. These ones come from the tiltmeter network sited in Pozzuoli. Instead, the tidal data come from the tide gauge in Pozzuoli. Data have been analysed by means of a wavelet approach, using a Continuos Wavelet Transform and using, as so-called ＂Wavelet mother＂, a Gabor-morlet wavelet. For each time series, the principal harmonic constituents result： lunar semidiurnal （m2）, solar semidiumal （S2） and lunar diurnal （K1）. Other harmonic constituents, having frequencies higher than 1/hour, are *** last ones could be interpreted as seiches and they could be linked up with generation of discrete plumes of rising magma. Frequencies at which there is the occurrence of these seiches are in agreement with previous studies.

Aims: The paper explores the hypothesis that the speed of light c is decreasing over time at rate (dc/dt)=-H? c, H being the Hubble constant. This hypothesis differs from the so-called tired light, in which the velocity c is supposed to vary during the journey of photons in the empty space for some frictional mechanism. In the hypothesis of the author the speed c, during this journey, is assumed constant. In this way the problems of the tired hypothesis are overcome. methods: The paper links the variation dc/dt with the Hubble constant and infers a value of dc/dt from the difference between the value of the variation of the Earth-moon distance measured by the Lunar Laser Ranging Experiment and the tidal effect. Results: Under the hypothesis c time varying, we explain: 1) The cosmological redshift. 2) The anomalous acceleration ap ≈ -8 × 10-10 m?sec-2, measured for some spacecrafts. 3) The high redshift of supernovae Ia, which seems to evidence an acceleration of the expansion of the universe. 4) The peripheral motion of stars in galaxies around their rotational centre. 5) The dilation of the light curves observed for supernovae Ia.

Palo-Corna di Savallo: la maggiore di esse è costituita dal collassodi un lungo tratto della cresta nella zona centro-orientale del gruppo. A giudizio dell’Autore il fattoreprincipale del processo, ritenuto tuttora in atto, è determinato dalla sovrapposizione della zolla in Dolo-mia Principale su un substrato plastico di litotipi delle Formazioni di S. Giovanni Bianco e dell’Arenariadi V. Sabbia. Viene esaminato il possibile concorso di eventi sismici. The slope deformations of the Group monte Inferni, monte Palo, Corna di Savallo inthe Brescian Alps (Northern Italy). The Author describes the slope deformations of the southern sideof the dolomitic Group of monte Inferni, monte Palo, Corna di Savallo. The main deformation is causedby the collapse of part of the east-central ridge of the Group. According to the Author, the processis due to the overlapping of the Dolomia Principale above a plastic layer of lithotypes of the S. GiovanniBianco formation and Valsabbia sandstone. The possibility of sismic events is also taken into account.

Background:Contralateral delayed endolymphatic hydrops(CDEH)is a clinical entity characterized by fluctuating low frequency hearing loss and/or vertigo,mimicking meniere’s disease(mD),that manifests after the appearance of severe non-hydropic hearing loss(NHHL)at the other ***:to describe the clinical features and the course of 57 patients affected by ***:this is a retrospective study;57 patients affected by CDEH,out of 1065 patients seen in the same period and affected by mD,were subjected to otoscopy,PTA threshold evaluation,impedance testing,ABR,research of positioning nystagmus,vestibular function evaluated by means of bithermal caloric test under video-oculographic,and mRI with ***:the CDEH was definite in 24 cases(42%),probable in 2(4%)and possible in 31(54%).The mean PTA threshold at the hydropic ear was 41 *** the last follow-up,40 patients(70%)did not report vertigo or fluctuating hearing *** the 17 patients who still reported symptomatology,11(64%)were affected by fluctuating hearing loss alone,4(23%)reported a subjective worsening of hearing loss and 2(12%)an acute vertigo ***:contralateral delayed endolymphatic hydrops is a relatively rare form of meniere disease that manifests more frequently as a definite form or with fluctuating low-frequency hearing *** prognosis at a long term follow-up is relatively good in terms of vertigo *** delayed endolymphatic hydrops rarely determines a severe hearing loss in the better ear.
