Predictive factors of short term outcome after liver transplantation: A review

Liver transplantation represents a fundamental therapeutic solution to end-stage liver disease. The need for liver allografts has extended the set of criteria for organ acceptability, increasing the risk of adverse outcomes. Little is known about the early postoperative parameters that can be used as valid predictive indices for early graft function, retransplantation or surgical reintervention, secondary complications, long intensive care unit stay or death. In this review, we present state-of-the-art knowledge regarding the early post-transplantation tests and scores that can be applied during the first postoperative week to predict liver allograft function and patient outcome, thereby guiding the therapeutic and surgical decisions of the medical staff. Post-transplant clinical and biochemical assessment of patients through laboratory tests (platelet count, transaminase and bilirubin levels, INR, factor V, lactates, and Insulin Growth Factor 1) and scores (model for end-stage liver disease, acute physiology and chronic health evaluation, sequential organ failure assessment and model of early allograft function) have been reported to have good performance, but they only allow late evaluation of patient status and graft function, requiring days to be quantified. The indocyanine green plasma disappearance rate has long been used as a liver function assessment technique and has produced interesting, although not univocal, results when performed between the 1th and the 5th day after transplantation. The liver maximal function capacity test is a promising method of metabolic liver activity assessment, but its use is limited by economic cost and extrahepatic factors. To date, a consensual definition of early allograft dysfunction and the integration and validation of the above-mentioned techniques, through the development of numerically consistent multicentric prospective randomised trials, are necessary. BACKGROUND Thromboembolic complications are relatively common causes of increased morbidity and mortality in the perioperative period in liver transplant *** postoperative portal vein thrombosis(PVT,incidence 2%-2.6%)and early hepatic artery thrombosis(HAT,incidence 3%-5%)have a poor prognosis in transplant patients,having impacts on graft and patient *** the present study,we attempted to identify the predictive factors of these complications for early detection and therefore monitor more closely the patients most at risk of thrombotic *** To investigate whether intraoperative thromboelastography(TEG)is useful in detecting the risk of early postoperative HAT and PVT in patients undergoing liver transplantation(LT).METHODS We retrospectively collected thromboelastographic traces,in addition to known risk factors(cold ischemic time,intraoperative requirement for red blood cells and fresh-frozen plasma transfusion,prolonged operating time),in 27 patients,selected among 530 patients(≥18 years old),who underwent their first LT from January 2002 to January 2015 at the Liver University Transplant Center and developed an early PVT or HAT(case group).Analyses of the TEG traces were performed before anesthesia and 120 min after *** retrospectively compared these patients with the same number of nonconsecutive control patients who underwent LT in the same study period without developing these complications(1:1 match)(control group).The chosen matching parameters were:Patient graft and donor characteristics[age,sex,body mass index(BMI)],indication for transplantation,procedure details,United Network for Organ Sharing classification,BMI,warm ischemia time(WIT),cold ischemia time(CIT),the volume of blood products transfused,and conventional laboratory coagulation *** distributed continuous data are reported as the mean±SD and compared using one-way Analysis of Variance(ANOVA).Nonnormally distributed continuous data are rep

In astrophysics, the boundary conditions for plasma phenomena are provided by nature and the astronomer faces the problem of understanding them from a variety of observations [Hester J J et al 1996 Astrophys. J. 456 225], on the other hand, in laboratory plasma experiments the electromagnetic boundary conditions become a major problem in the set-up of the machine that produces the plasma, an issue that has to be investigated step by step and to be modified and adapted with great patience, in particular in the case of an innovative plasma confinement experiment. The PROTO-SPHERA machine [Alladio F et al 2006 Nucl. Fusion 46 S613] is a magnetic confinement experiment, that emulates in the laboratory the jet + torus plasma configurations often observed in astrophysics: an inner magnetized jet of plasma centered on the(approximate) axis of symmetry and surrounded by a magnetized plasma torus orthogonal to this jet. The PROTO-SPHERA plasma is simply connected, i.e., no metal current conducting rod is linked to the plasma torus, while instead it is the inner magnetized plasma jet(in the following always called the plasma centerpost) that is linked to the torus. It is mandatory that no spurious plasma current path modifies the optimal shape of the plasma centerpost. Moreover, as the plasma torus is produced and sustained, in absence of any applied inductive electric field, by the inner plasma centerpost through magnetic reconnections [Taylor J B and Turner M F 1989 *** 29 219], it is required as well that spurious current paths do not surround the torus on its outboard, in order not to lower the efficiency of the magnetic reconnections that maintain the plasma torus at the expense of the plasma centerpost. Boundary conditions have been corrected,up to the point that the first sustainment in steady state has been achieved for the combined plasma.

Background:Minimally invasive surgery(MIS)is the technique of choice in selected patients for the treatment of liver *** robotic approach is considered today the natural evolution of *** application of the robotic technique in liver transplantation(LT)has been recently evaluated,especially in the living *** aim of this paper is to review the current role of the MIS and robotic donor hepatectomy in the literature and to evaluate the possible future implication in the transplant ***:We conducted a narrative review using PubMed and Google Scholar for reports published so far,using the following keywords:minimally invasive liver surgery,laparoscopic liver surgery,robotic liver surgery,robotic living donation,laparoscopic donor hepatectomy and robotic donor ***:Several advantages have been claimed in favor of robotic surgery:three-dimensional(3-D)imaging with stable and high-definition view;a more rapid learning curve than the laparoscopic one;the lack of hand tremors and the freedom of *** to open surgery,the benefits showed in the studies evaluating the robotic approach in the living donation are:less postoperative pain,the shorter period before returning to normal activity despite sustaining longer operation ***,the 3-D and magnification view makes the technique excellent in distinguishing the right plane of transection,vascular and biliary anatomy,associated with high precision of the movements and a better bleeding control(essential for donor safety)and lower rate of vascular ***:The current literature does not fully support the superiority of the robotic approach versus laparoscopic or open method in living donor *** donor hepatectomy performed by teams with high expertise and in properly selected living donors is safe and ***,further data are necessary to evaluate properly the role of robotic surgery in the field of living donation.

Precision Tests of the Nonlinear Mode Coupling of Anisotropic Flow via High-Energy Collisions of Isobars

Valuable information on dynamics of expanding fluids can be inferred from the response of such systems to perturbations in their initial *** apply this technique in high-energy^(96)Ru+^(96)Ru and^(96)Zr+^(96)Zr collisions to scrutinize the expansion dynamics of the quark-gluon plasma,where the initial geometry perturbations are sourced by the differences in deformations and radial profiles between^(96)Ru and^(96)Zr,and the collective response is captured by the change in anisotropic flow Vn between the two collision *** a transport model,we analyze how the nonlinear coupling between lower-order flow harmonics V_(2)and V_(3)to the higher-order flow harmonics V_(4)and V_(5),expected to scale as V_(4)NL= χ_(4)V_(2)2and V_(5)NL= χ_(5)V_(2)V_(3),gets modified as one moves from^(96)Ru+^(96)Ru to^(96)Zr+^(96)Zr *** find that these scaling relations are valid to high precision:variations of order 20% in V_(4)NLand V_(5)NLdue to differences in quadrupole deformation,octupole deformation,and nuclear skin modify χ_(4)and χ_(5)by about 1–2%.Percent-level deviations are however larger than the expected experimental uncertainties and could be ***,collisions of isobars with different nuclear structures are a unique tool to isolate subtle nonlinear effects in the expansion of the quark-gluon plasma that would be otherwise impossible to access in a single collision system.

The study of the chiroptical properties of thin-film samples of two chiral benzo[1,2-b:4,5-b’]dithiophene-based oligothiophenes with optoelectronic properties was carried out at Diamond B23 beamline using the synchrotron radiation electronic circular dichroism imaging(SR-ECDi)*** experimental results support the hypothesis that the non-reciprocal circularly polarized absorption observed in these materials is due to the combined linear dichroism and linear birefringence *** mapping at 50-μM spatial resolution revealed that several species coexist in the mesoscopic chiral domains likely due to different aggregated phases,which were identified in the films by SR-ECDi but could not be detected with benchtop ECD instruments.
