Background: Following coronary artery bypass grafting (CABg), delirium emerges as a prevalent complication. This study aimed to assess the correlation between elevated serum cortisol levels and the occurrence of postoperative delirium subsequent to off-pump CABg. Methods: Conducted in the Department of Cardiac Surgery at BSMMU from October 2020 to September 2022, this comparative cross-sectional study included a total of 44 participants. Subjects, meeting specific criteria, were purposefully assigned to two groups based on off-pump CABg. group A (n = 22) consisted of patients with normal serum cortisol levels, while group B (n = 22) comprised individuals with high serum cortisol levels on the first postoperative day. Delirium onset was assessed at the bedside in the ICU on the 1st, 2nd, 3rd, 4th, and 5th postoperative days using standard tools, namely the Richmond Agitation Sedation score (RASS) and The Confusion Assessment Method (CAM-ICU). Data were collected based on the presence or absence of delirium. Statistical analysis utilized SPSS version 26.0, employing an independent Student’s t-test for continuous data and chi-square and Fischer’s exact test for categorical data. A p-value ≤ 0.05 was considered statistically significant. Results: group-A had a mean age of 54.50 ± 17.97, and group-B had a mean age of 55.22 ± 15.45, both with a male predominance (81.81% and 86.36% respectively). The mean serum cortisol level was significantly higher in group B (829.71 ± vs. 389.98 ± 68.77). Postoperative delirium occurred in 27.3% of group B patients, statistically significant compared to the 4.5% in group A. However, patients in group B who developed delirium experienced significantly longer postoperative ICU and hospital stays (79.29 ± 12.27 vs. 11.44 ± 2.85, p ≤ 0.05). The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large Hadron Collider,as Standard Model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced *** paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS *** taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet *** in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 geV and 2 TeV and for multiple tagger working points.

目的搜集原发性眼附属器淋巴瘤患者的影像资料,进行影像学分析,总结其影像特点,从而提高对该疾病的认识。方法回顾性分析经手术病理证实的19例原发性眼附属器淋巴瘤患者的CT及MRI特点,其中CT检查18例,MRI检查12例,同时进行CT及MRI检查者11例。结果 19例分别位于眼睑(n=10)、泪腺(n=5)、泪囊(n=3),眼外肌(n=1)。CT表现为眼附属器高密度软组织占位,CT值50~56HU,病灶无包膜,呈匍匐铸型生长,MRI呈等T1WI、等或稍长T2WI信号,增强扫描后明显强化。CT及MRI均可显示肿瘤与周围结构的关系,眼环受累(n=3),眼外肌受累(n=6)、泪腺受累(n=4),同时累及泪腺、眼环及眼外肌(n=2),以包绕为主,占位效应不明显。结论原发性眼附属器淋巴瘤较少见,临床工作中需不断总结经验,提高认识,以减少误诊。

Substituted 2-benzylidene-1-benzofuran-3-ones are commonly known as aurones. This class of bioactive heterocycles belongs to flavonoid family. The article intends to put forth the rational design and synthesis of a new series of aurones using 3’,5’-dibromo-2’,4’-dihydroxychalcones and copper bromide in presence of DMF-water mixture (8:2, v/v) for the first time. Preliminary bioassay shows that most of compounds have good trypanocidal activity against Trypanosoma cruzi at 10 μg/mL. Few compounds are equally potent to the standard drugs Benznidazole and nifurtimox. The structures of the newly synthesized products 2a-n were established by elemental analysis, FTIR, 1H nMR, 13C nMR and mass spectroscopic studies.

目的观察盐酸米多君干预前后血管迷走性晕厥（VVS）儿童血流动力学变化，探讨盐酸米多君对VVS患儿血流动力学的影响。方法2007—04～2009—05在中南大学湘雅二医院儿童晕厥专科就诊或住院的6～17岁[平均（11．60±2．83）岁]62例不明原因晕厥（UPS）或晕厥先兆、直立倾斜试验（HUTT）阳性的VVS患儿，分为口服补液盐组（n=36）和盐酸米多君组（n=26），根据HUTT结果评价疗效并监测其血流动力学变化。治疗6个月后，如没有晕厥或晕厥先兆发作则停药并继续随访。结果盐酸米多君组HUTT转阴率与口服补液盐组比较差异无统计学意义（P〉0．05）；盐酸米多君组晕厥及晕厥先兆复发率低于口服补液盐组（P〈0．05）；两组干预前后平卧位血压和心率比较差异无统计学意义（P〉0．05）。盐酸米多君组干预后血压差值（△舒张压）在倾斜起始较基础平卧位显著缩小（P〈0．05）。结论盐酸米多君可减少VVS儿童体位改变时血流动力学变化，改善直立不耐受症状。

Predictors of Death in Under-Five Children with Sepsis Attending an Urban Diarrheal Treatment Centre in Bangladesh

SETTIng: Special Care Ward (SCW) of the Dhaka Hospital of icddr,b, Bangladesh. OBJECTIVE: To evaluate the clinical and laboratory predictors of death in under-five children with clinically defined sepsis presenting with diarrhea. METHODS: We prospectively enrolled all the diarrheal children (n = 151) aged 0 to 59 months with clinical sepsis admitted in the SCW during September’2007 through December’2007. Comparison was made between deaths (n = 23) and survivors (n = 128). Sepsis is defined as presence of inflammation [abnormal WBC count (>11 × 109/L or, 9/L or, band and neutrophil ratio ≤0.10] plus presence or presumed presence of infection with thermo-instability [hypo (≤35.0℃) or hyperthermia (≥38.5℃)], tachycardia, tachypnea, and/or the indications of altered organ function (altered mental status and bounding pulse) in the absence of clinical dehydration or after correction of dehydration. RESULTS: The median (inter-quartile range) age (months) of the children who survived and died was comparable [4.0 (2.0, 12.0) vs. 1.5 (0.8, 10.0);p = 0.703]. In the logistic regression analysis, after adjusting for potential confounders, such as abnormal WBC count, use of intravenous fluid, patient with fatal outcome more often presented with hypernatremia (odds ratio = 16.48, 95% confidence interval = 2.21 -?123.12;p = 0.006), lobar consolidation (odds ratio = 19.9, 95% confidence interval = 2.99 - 132.80;p = 0.002), hypoxemia (odds ratio = 14.78, 95% confidence interval = 1.38 157.90;p = 0.026) and severe under-nutrition (odds ratio = 7.57, 95% confidence interval = 1.24 - 46.11;p = 0.028). COnCLUSIOnS: Our data suggest that children under-five with clinical sepsis who present with lobar pneumonia, hypoxaemia, severe acute malnutrition and hyperntaemia are at higher risk of death and identification of these simple factors may help clinicians to take prompt initiative for the aggressive management of such children especially in a resource-limited setting like Bangladesh.

Sediment Characteristics of Tidal Deposits at Mandvi, gulf of Kuchchh, gujarat, India: geophysical, Textural and Mineralogical Attributes

The textural and mineralogical characteristics of subsurface sediments along with ground Penetrating Radar (gPR) data collected along the Western margin of India were used to understand the depositional environments and the provenance of sediments. Textural attributes such as mean, standard deviation, skewness and Kurtosis show fluctuations due to fluvio-marine interaction and it is well understood that the bottom sediments are enriched with muddy sand, which may be deposited by mean of tidal invasion. The abundance of very coarse silt to very fine sand indicates the prevalence of low energy environment. The Linear Discriminate Function analysis indicates the fluvio marine deposition of sediments under low energy condition. gPR data upto a depth of 5 m shows the stratigraphic sequences of very fine sands and coarse silt. Optically Stimulated Luminescence (OSL) dating of sediments implies that the tidal sediments had deposited between ~30 ± 1 and ~90 ± 4 years before Present. XRD studies results the dominance of K-feldspar and deficiency of clay minerals, indicating that most of the sediments are derived from hinterland. Studies indicate that the Provenance of sediments in the Mandvi area is not related to Indus delta even though a considerable part of gulf sediments are deposited by the Indus River.

The Basidiomycota constitutes a major phylum of the kingdom Fungi and is second in species numbers to the *** present work provides an overview of all validly published,currently used basidiomycete genera to date in a single *** outline of all genera of Basidiomycota is provided,which includes 1928 currently used genera names,with 1263 synonyms,which are distributed in 241 families,68 orders,18 classes and four *** provide brief notes for each accepted genus including information on classification,number of accepted species,type species,life mode,habitat,distribution,and sequence ***,three phylogenetic analyses with combined LSU,SSU,5.8s,rpb1,rpb2,and ef1 datasets for the subphyla Agaricomycotina,Pucciniomycotina and Ustilaginomycotina are conducted,*** time estimates are provided to the family level with 632 species from 62 orders,168 families and 605 *** study indicates that the divergence times of the subphyla in Basidiomycota are 406-430 Mya,classes are 211-383 Mya,and orders are 99-323 Mya,which are largely consistent with previous *** this study,all phylogenetically supported families were dated,with the families of Agaricomycotina diverging from 27-178 Mya,Pucciniomycotina from 85-222 Mya,and Ustilaginomycotina from 79-177 *** times as additional criterion in ranking provide additional evidence to resolve taxonomic problems in the Basidiomycota taxonomic system,and also provide a better understanding of their phylogeny and evolution.

目的探讨还原型辅酶Ⅱ氧化酶(nADPH oxidase,nOX2)抑制剂夹竹桃麻素(Apocynin)对急性坏死性胰腺炎(acute necrotic pancreatitis,AnP)模型大鼠肾损伤的保护作用。方法 30只Wistar大鼠随机分为3组:假手术组(SO组)、急性坏死性胰腺炎组(AnP组)、Apocynin治疗组(APO组),每组各10只大鼠。AnP组采用逆行胆胰管注射牛磺胆酸钠溶液1 ml/kg(STC)制备AnP模型。APO组在造模前30 min股静脉注射Apocynin(50 mg/kg),其余同AnP组。SO组开腹后仅翻动十二指肠和胰腺。造模后12 h处死大鼠,下腔静脉采血,留取各组大鼠肾组织以4%多聚甲醛固定。全自动生化仪检测血清淀粉酶(AMY)、脂肪酶(LIPA)、肌酐(Cr)、尿素(BUn)水平,肾脏组织固定行HE染色,光学显微镜下观察肾脏病理改变,免疫组化及免疫荧光法检测肾脏组织半胱天冬酶-3(Caspase-3)、核转录因子-κB(nF-κB)、肿瘤坏死因子-α(TnF-α)、肠黏膜髓过氧化酶(MPO)水平。结果 (1)AnP组大鼠血清AMY、LIP、Cr、BUn水平及组织病理改变较SO组明显升高(Pnin后降低(PnP组大鼠Caspase-3、nF-κB、TnF-α表达水平较SO组升高(Pnin后有所降低(PnP组大鼠髓过氧化酶(MPO)表达水平较SO组升高(Pnin后有所降低(PnOX2抑制剂Apocynin可减轻AnP模型大鼠肾脏损伤。
