The cavitation has received considerable attention for decades because of its negative influence on the performance and the safety of the hydraulic *** this study,a large eddy simulation is carried out to numerically investigate the unsteady cavitating flow around a trailing-truncated NACA 0009 hydrofoil for determining the underlying physical *** types of cavitation morphologies are identified:The large-scale bubble cluster and the von Kármán vortex cavity,named as the cloud cavitation and the wake vortex cavitation,*** is shown that the velocity profiles obtained over the hydrofoil suction surface are in good agreement with the experimental data,indicating the accuracy of the current *** dynamic evolution of the sheet/cloud cavity is also well reproduced,covering the sheet cavity breakup,the sheet/cloud transformation,and the collapse of the cloudy bubble *** wake-vortex cavitation is caused by the blunt geometry at the hydrofoil trailing edge,where pairs of vortex cavities are *** the cloud and vortex cavities significantly affect the lift oscillation,which makes it difficult to decompose the *** fundamental shedding mechanisms of the wake vortex cavitation are discussed based on the finite-time Lyapunov exponent ***,the suction-side bubble grows and squeezes the giant pressure bubble away from the trailing *** the pressure bubble detaches,a new counterclockwise vortex or a new bubble appears at the pressure side,thus lifting the ridge towards the suction trailing edge and generating a strong vortex eye that pinches off the trailing portion of the suction-side bubble.

Suspended sediment transport analysis in two Italian instrumented catchments

Suspended sediment transport in streams is an effective indicator of soil erosion at the watershed scale. This process was studied using the data obtained from two continuous monitoring stations installed in Italian watersheds, the Rio Cordon and Torrente CarapeUe. The catchments have substantially different climatic, morphological and land use characteristics. The Rio Cordon, a small Alpine watershed, has been monitored since 1986, while in the Torrente Carapelle, a medium-size Mediterranean watershed, the monitoring station has provided data since 2007. Several flood events with suspended sediment transport were isolated in the two catchments, excluding those determined by snowmelt in the Rio Cordon as this process does not affect the Carapelle watershed. Analysis of the events showed different behavior in terms of hysteresis loop trends between water discharge （m3 S-1） and suspended sediment concentration （g 1-1） values, as the Rio Cordon confirms clockwise relationships most frequently, while counter-clockwise trends represent the majority of cases for the Carapelle Torrent. The different behavior of the two watersheds was further analyzed by evaluating relationships between the main hydrological parameters related to suspended sediment transport. Runoff controls the total sediment budget in both catchments. In contrast, it was noted that the runoff process does not interfere in the magnitude of the suspended sediment transport in the Rio Cordon catchment, while variations due to the larger size of the catchment area characterize the events in the Carapelle watershed. Lastly, a flow peak threshold that determines the advance or delay of the hydrograph peak with respect to the sedigraph peak was established for the Carapelle Torrent, while it was not reported in the Rio Cordon.

Objective: To evaluate transperineal laser ablation (TPLA) with Echolaser® (Echolaser® TPLA, Elesta S.p.A., Calenzano, Italy) as a treatment for benign prostatic hyperplasia (BPH) and prostate cancer (PCa) using the Delphi consensus ***: Italian and international experts on BPH and PCa participated in a collaborative consensus project. During two rounds, they expressed their opinions on Echolaser® TPLA for the treatment of BPH and PCa answering online questionnaires on indications, methodology, and potential complications of this technology. Level of agreement or disagreement to reach consensus was set at 75%. If the consensus was not achieved, questions were modified after each round. A final round was performed during an online meeting, in which results were discussed and ***: Thirty-two out of forty invited experts participated and consensus was reached on all topics. Agreement was achieved on recommending Echolaser® TPLA as a treatment of BPH in patients with ample range of prostate volume, from 80 mL (80%), comorbidities (100%), antiplatelet or anticoagulant treatment (96%), indwelling catheter (77%), and strong will of preserving ejaculatory function (100%). Majority of respondents agreed that Echolaser® TPLA is a potential option for the treatment of localized PCa (78%) and recommended it for low-risk PCa (90%). During the final round, experts concluded that it can be used for intermediate-risk PCa and it should be proposed as an effective alternative to radical prostatectomy for patients with strong will of avoiding urinary incontinence and sexual dysfunction. Almost all participants agreed that the transperineal approach of this organ-sparing technique is safer than transrectal and transurethral approaches typical of other techniques (97% of agreement among experts). Botulinum toxin for chronic anal fissure after biliopancreatic diversion for morbid obesity

AIM: To study the effect of botulinum toxin in patients with chronic anal fissure after biliopancreatic diversion (BPD) for severe obesity. METHODS: Fifty-nine symptomatic adults with chronic anal fissure developed after BPD were enrolled in an open label study. The outcome was evaluated clinically and by comparing the pressure of the anal sphincters before and after treatment. All data were analyzed in univariate and multivariate analysis. RESULTS: Two months after treatment, 65.4% of the patients had a healing scar. Only one patient had mild incontinence to flatus that lasted 3 wk after treatment, but this disappeared spontaneously. In the multivariate analysis of the data, two registered months after the treatment, sex (P = 0.01), baseline resting anal pressure (P = 0.02) and resting anal pressure 2 mo after treatment (P < 0.0001) were significantly related to healing ***: Botulinum toxin, despite worse results than in non-obese individuals, appears the best alternative to surgery for this group of patients with a high risk of incontinence.

The objective of this study was to investigate whether medical therapy can reduce sperm aneuploidy levels and irfiprove the results of intracytoplasmic sperm injection （ICSI） in patients with severe idiopathic oligoasthenoteratospermia （OAT）. Thirty-three infertile couples requiring ICSI because of severe idiopathic OAT after at least one unsuccessful ICSI cycle were considered. Semen parameters （concentration, motility and morphology）, the percentage of aneuploid sperm and the results of ICSI （the number of oocytes fertilized, embryos transferred, biochemical pregnancies, clinical pregnancies and live births） were compared before and after a 3-month course of treatment with L-carnitine 1 g given twice per day＋acetyI-L-carnitine 500 mg given twice per day＋one 30-mg cinnoxicam tablet every 4 days. Aneuploidy was assessed using fluorescent in situ hybridisation （FISH） performed on chromosomes X, Y, 13, 15, 16, 17, 18, 21 and 22. The results showed that 22 of the 33 patients had a reduced frequency of aneuploid sperm and improved sperm morphology after treatment （group 1）, and 11 showed no change （group 2）. The numbers of biochemical pregnancies, clinical pregnancies and live births were significantly higher in group I than in group 2. No significant difference was found between the groups regarding the numbers of oocytes fertilized and embryos transferred. The side effects were negligible. The numbers of ICSI pregnancies and live births in severe idiopathic OAT patients improved with a course of L-carnitine, acetyI-L-carnitine and cinnoxicam.
