Multivariate Analysis of Extreme Physical, Biological and Chemical Patterns in the Dynamics of Aquatic Ecosystem

This study is a part of the research in monitoring systems of environmental impacts in coastal re-gions in order to develop trophic dynamic models to be used in the aquatic systems management. Meteorological influences in the variability of the nutrients, larvae concentration, dissolved oxygen (DO) and chlorophyll a were investigated in a region where upwelling occurs. Extreme seasonal variations of reanalysis, QuikSCAT, and surface stations from the southeast coast of Brazil, as well as, surface seawater data collected in Anjos Bay, Arraial do Cabo city northeast of Rio de Janeiro state, are analyzed. Seasonality and correlations are applied to verify the relationship between them, considering minimum values of sea surface temperature (SST) and sea level variation and maximum values of the other variables. Principal Component Analysis (PCA) and Hierarquical Cluster Analysis (HCA) are applied to verify spatial and temporal variances and to describe more clearly the structure of the local ecosystem. The seasonality of northeasterly extreme wind stress follows the seasonal pattern expected for the study area with peaks during spring. The SST has a well-defined seasonal pattern with maximum peaks from February to July and minimum peaks from September to January. Chlorophyll a presents higher seasonal peak in February, being in accordance with DO;both are related to the maximum primary productivity. Correlations of the physical variables (local and remote) with nutrients and larvae present a relatively similar pattern around 0.5, showing these variables have a reasonable interaction with the meteorological forcing. PCA shows a strong variability in pressure data around 0.9, which may be related to the seasonal variations in South Atlantic subtropical anticyclone (SASA) and consequently to the occurrence of upwelling in the region. AIM： To evaluated the therapeutic and prophylactic effect of thalidomide on 2, 4, 6-trinitrobenzene sulfonic acid （TNBS）-induced colitis. Thalidomide has been reported to downregulate the expression of tumor necrosis factor α （TNF-α）, IL-12, and vascular endothelial growth factor （VEGF）, hallmarks of intestinal inflammation in Crohn＇s disease （CD）. METHODS： Male Wistar rats were divided in five groups of ten animals each. Four groups received a rectal infusion of TNBS in ethanol. The first group was sacrificed 7 d after colitis induction. The second and third groups received either thalidomide or placebo by gavage and were sacrificed at 14 d. The fourth group received thalidomide 6 h before TNBS administration, and was sacrificed 7 d after induction. The fifth group acted as the control group and colitis was not induced. Histological inflammatory scores of the colon were performed and lamina propria CD4＋ T cells, macrophages, and VEGF＋ cells were detected by immunohistochemistry. TNF-α and IL-12 were quantified in the supernatant of organ cultures by ELISA. RESULTS： Significant reduction in the inflammatory score and in the percentage of VEGF＋ cells was observed in the group treated with thalidomide compared with animals not treated with thalidomide. Both TNF-α and IL-12 levels were significantly reduced among TNBS induced colitis animals treated with thalidomide compared with animals that did not receive thalidomide. TNF-α levels were also significantly reduced among the animals receiving thalidomide prophylaxis compared with untreated animals with TNBS-induced colitis. Intestinal levels of TNF-α and IL-12 were significantly correlated with the inflammatory score and the number of VEGF＋ cells. CONCLUSION： Thalidomide significantly attenuates TNBS-induced colitis by inhibiting the intestinal production of TNF-α, IL-12, and VEGF. This effect may support the use of thalidomide as an alternate approach in selected patients with CD.

Appropriateness of the study of iron deficiency anemia prior to referral for small bowel evaluation at a tertiary center

To evaluate the adequacy of the study of iron deficiency anemia (IDA) in real life practice prior to referral to a gastroenterology department for small bowel evaluation. METHODSAll consecutive patients referred to a gastroenterology department for small bowel investigation due to iron deficiency anemia, between January 2013 and December 2015 were included. Both patients referred from general practitioners or directly from different hospital departments were selected. Relevant clinical information regarding prior anemia workup was retrospectively collected from medical records. An appropriate pre-referral study was considered the execution of esophagogastroduodenoscopy (EGD) with Helicobacter pylori (H. pylori) investigation, colonoscopy with quality standards (recent, total and with adequate preparation) and celiac disease (CD) screening (through serologic testing and/or histopathological investigation). RESULTSA total of 77 patients (58.4% female, mean age 67.1 ± 16.7 years) were included. Most (53.2%) patients were referred from general practitioners, 41.6% from other hospital specialties and 5.2% directly from the emergency department. The mean pre-referral hemoglobin concentration was 8.8 ± 2.0 g/dL and the majority of anemias had microcytic (71.4%) and hypochromic (72.7%) characteristics. 77.9% of patients presented with an incomplete pre-referral study: EGD in 97.4%, with H. pylori investigation in 58.3%, colonoscopy with quality criteria in 63.6%, and CD screening in 24.7%. Patients with an appropriate study at the time of referral were younger (48.7 ± 17.7 vs 72.3 ± 12.3 years, P < 0.001). Small bowel evaluation was ultimately undertaken in 72.7% of patients, with a more frequent evaluation in patients with a quality colonoscopy at referral (78.6% vs 23.8%); P < 0.001 (OR = 11.7, 95%CI: 3.6-38.6).
