Ambulatory blood-pressure monitoring, antihypertensive therapy and the risk of fall injuries in elderly hypertensive patients

Background Fall injuries are common among the elderly. The aim of this study was to investigate whether blood-pressure patterns, as measured by 24-h ambulatory blood pressure monitoring （ABPM）, or intensification of antihypertensive therapy following the 24-h ABPM, may be associated with fall injuries in hypertensive elderly patients. Methods In a retrospective study, community-based elderly patients （age ≥ 70 years） who were referred to 24-h ABPM were evaluated for fall injuries within one-year post-ABPM. We compared the clinical characteristics, 24-h ABPM patterns and the intensification of hypertensive therapy following 24-h ABPM, between patients with and without a fall injury. Results Overall 1032 hypertensive elderly patients were evaluated. Fifty-five （5.3%） had a fall injury episode in the year following ABPM. Patients with a fall injury were significantly older, and with higher rates of previous falls. Lower 24-h diastolic blood-pressure （67.3 ± 7.6 vs. 70.7 ± 8.8 mmHg; P 〈 0.005） and increased pulse-pressure （74.7 ± 14.3 vs. 68.3 ± 13.7 mmHg; P 〈 0.005）, were found in the patients with a fall injury, compared to those without a fall injury. After adjustment for age, gender, diabetes mellitus and previous falls, lower diastolic blood-pressure and increased pulse-pressure were independent predictors of fall injury. Intensification of antihypertensive treatment following the 24-h ABPM was not associated with an increased rate of fall injury. Conclusions Low diastolic blood-pressure and increased pulse-pressure in 24-h ABPM were associated with an increased risk of fall injury in elderly hypertensive patients. Intensification of antihypertensive treatment following 24-h ABPM was not associated with an increased risk of fall injury.

The amount of piggery wastewater as domestic livestock is *** volume of piggery wastewater produced is less than the volume of other wastewaters,but piggery wastewater has a heavy impact on wastewater streams due to an extremely high concentration of nitrogen and *** this study,laboratory reactors were operated using piggery wastewater and the effluent of anaerobic digester from piggery wastewater *** purpose of this study was to induce the nitritation process,which is an economically advantageous nitrogen removal method that converts ammonium nitrogen into *** results showed that the effluent of anaerobic digester from piggery wastewater was more efficient than raw piggery wastewater in terms of inducing *** can be deduced that nitritation is largely affected by an organic fraction of piggery *** can also be concluded that a small amount of biodegradable organic matter in piggery wastewater is efficient in inducing nitritation.

Let Lcd be a cd-homogeneous C*-algebra over whose cd-homogeneous C*-subalgebra restricted to the subspace Tr × T2 is realized as C(Tr) A1/d Mc(C), and of which no non-trivial matrix algebra can be factored *** lenticular noncommutative torus Lpcd is defined by twisting in by a totally skew multiplier p on Tr+2 × Zm-2. It is shown that is isomorphic to if and only if the set of prime factors of cd is a subset of the set of prime factors of p, and that Lpcd is not stablyisomorphic to if the cd-homogeneous C*-subalgebra of Lpcd restricted to some subspace LkiLki (ni) is realized as the crossed product by the obvious non-trivial action of Zki on a cd/ki-homogeneous C*-algebra over S2ni+1 for ki an integer greater than 1.

Novel translational strategies in colorectal cancer research

Defining translational research is still a complex task. In oncology, translational research implies using our basic knowledge learnt from in vitro and in vivo experiments to directly improve diagnostic tools and therapeutic approaches in cancer patients. Moreover, the better understanding of human cancer and its use to design more reliable tumor models and more accurate experimental systems also has to be considered a good example of translational research. The identification and characterization of new molecular markers and the discovery of novel targeted therapies are two main goals in colorectal cancer translational research. However, the straightforward translation of basic research findings, specifically into colorectal cancer treatment and vice versa is still underway. In the present paper, a summarized view of some of the new available approaches on colorectal cancer translational research is provided. Pros and cons are discussed for every approach exposed.

Non-point source analysis of a railway bridge area using statistical method:Case study of a concrete road-bed

In an effort to protect the quality of the water system, interest in non-point source pollution is increasing. Recently, studies of non-point sources pollution are continuing in relation to various land- use areas, but such studies have not been fully conducted in railway facility sites. Using monitoring data of railway bridge area with concrete road-bed, the runoff characteristics, pollutant unit loads, and first flush criteria were assessed. Railway bridge area with concrete road-bed typically show the first flush effect, and the pollutant unit load was determined to be higher than other public facilities areas. Further, the first flush criteria show an effective rainfall amount of 7 mm. In other words, from the runoff of railway facilities, considerable amounts of non-point source pollutants are occurred, indicating the need to create best management practices which are adequate for railway facility sites.

Evaluation and hedonic value in mate choice

Mating preferences can show extreme variation within and among individuals even when sensory inputs are conserved. This variation is a result of changes associated with evaluative mechanisms that assign positive, neutral, or negative hedonic value to stimuli--that is, label them as attractive, uninteresting, or unattractive. There is widespread behavioral evidence for differences in genes, environmental cues, or social experience leading to marked changes in the hedonic value of stimuli. Evaluation is accomplished through an array of mechanisms that are readily modifiable through genetic changes or environmental inputs, and that may often result in the rapid acquisition or loss of behavioral preferences. Reversals in preference arising from ＂flips＂ in hedonic value may be quite common. Incorporating such discontinuous changes into models of preference evolution may illuminate our understanding of processes like trait diversification, sexual conflict, and sympatric speciation.
