AIm： To determine whether different intravitreal doses of quinupristin/dalfopristin lead to electroretinographic or histological changes in the rabbit retina over one month period after injection. mETHODS： Eighteen New Zealand white rabbits were divided into three treatment groups （groups 1 to 3） and different intravitreal doses of quinupristin/dalfopristin were tested in each group. The right eye was injected with the drug and the left eye received intravitreal injection of 5% dextrose water and served as control eye. The doses delivered to each group were 0.1 mg/0.1 mL, 1 mg/0.1 mL and 10 mg/0.1 mL. Simultaneous, bilateral, dark-adapted electroretinography and clinical images of both eyes were obtained in all groups before injection （baseline） and after 7, 14, 21 and 28d, followed by enucleation for histological examination. RESULTS： Subjects in the group 1 showed no signs of toxicity in the electroretinogram when compared with groups 2 and 3 （Kruskall-Wallis test, p=0.000）. By day 7, no electrical response to light stimuli was recorded in the treated eyes in groups 2 and 3, consistent with severe damage due to retinal toxicity. Light microscopy revealedno significant histopathological changes in the group 1, while rabbits in groups 2 and 3 had signs of granulomatous inflammation in most cases. CONCLUSION： Intravitreal 0.1 mg/0.1 mL doses of quinupristin/dalfopristin do not lead to electroretinographic or histological signs of retinal toxicity compared with 1 mg/0.1 mL and 10 mg/O.1 mL in this rabbit model.

Electrospun Gelatin Constructs with Tunable Fiber Orientation promote Directed Angiogenesis

The field of therapeutic angiogenesis has been predominantly concentrated in modalities that incorporate pro-angiogenic growth factors and/or cells within polymeric constructs that are implanted into the ischemic region. There is growing evidence that construct architecture can significantly affect growth factor activity, cellular viability and differentiation potential. Electrospinning is an attractive but simple scaffold fabrication technique that offers several advantages over traditional fabrication approaches to prepare highly organized structures for therapeutic angiogenesis applications. We recently described the fabrication of nanofibrous scaffolds with aligned fiber orientation that directed cell migration and orientation (*** umbilical vein endothelial cells). Herein we demonstrate the ability of bFGF containing nanofibrous gelatin B scaffolds with controlled fiber orientation to promote capillary formation in vivo. Aligned scaffolds loaded with bFGF induced the highest levels of reperfusion (73% increased in LDpI ratios by day 21 post ischemia induction) in comparison to all other groups including scaffolds with random fiber orientation. Furthermore, the newly formed vasculature, assessed by confocal microscopy, had a parallel alignment along the axis of the scaffold’s fibers. In contrast, no vessel directionality was observed in the animals treated with scaffolds with random fiber orientation in the presence or absence of bFGF.

Viral blips during long-term treatment with standard or double dose lamivudine in HBe antigen negative chronic hepatitis B

AIm:To evaluate safety and effect on hepatitis B virus(HBV)suppression of a long-term treatment with lamivudine(LAm)at standard(100 mg/d)or double(200 mg/d)dose in chronic hepatitis ***:This was a case study with matched controls(1:3)in patients with chronic hepatitis B with anti-HBe ***:Twelve patients received LAm 200 mg/d and 35 LAm 100 mg/d,for a median of 28 mo.A primary response(pR;*** HBV-DNA with Amplicor assay)was achieved in 100% of LAm-200 patients and 83% of LAm-100 patients.A virological breakthrough occurred in 16.7 and 24.7%,respectively,of the pR-patients,with the appearance of typical LAm resistance mutations in all but one *** blips(*** HBV-DNA below 80 IU/mL in patients who tested negative at Amplicor assay)were detected using a real time polymerase chain reaction(pCR)and occurred in seven out of nine patients with subsequent BT and in four out of 32 patients with end-of-study response(77.7% vs 12.5%;p = 0.001)at chi-square test).At the end of the study,51.4% of LAm-100 patients and 83.3% of LAm-200 patients had remained stably HBV-DNA ***-dose LAm was well ***:Long-term treatment of anti-HBe positive chronic hepatitis B with double dose lamivudine causes a more profound and stable viral suppression ascompared to conventional treatment.

Endoscopic resection of non-ampullary duodenal adenomas: Is cold snaring the promised land?

Due to the high risk of morbidity and mortality associated with surgical resection in this tract,endoscopic resection(ER)has taken the place of surgical resection as the first line treatment for non-ampullary duodenal ***,due to the anatomical characteristics of this area,which enhance the risk of post-ER problems,ER in the duodenum is particularly *** to a lack of data,no ER technique for superficial non-ampullary duodenal epithelial tumours(SNADETs)has yet been backed by strong,high-quality evidence;yet,traditional hot snare-based techniques are still regarded as the standard *** having a favourable efficiency profile,adverse events during duodenal hot snare polypectomy(HSp)and hot endoscopic mucosal resection,such as delayed bleeding and perforation,have been reported to be *** events are primarily caused by electrocautery-induced ***,ER techniques with a better safety profile are needed to overcome these *** snare polypectomy,which has already been shown as a safer,equally effective pro-cedure compared to HSp for treatment of small colorectal polyps,is being increasingly evaluated as a potential therapeutic option for non-ampullary duodenal *** aim of this review is to report and discuss the early outcomes of the first experiences with cold snaring for SNADETs.

Normal serum alanine aminotransferase activity in uncomplicated obesity

AIm: To evaluate serum alanine aminotransferase (ALT)activity in a well-characterized group of uncomplicated obese subjects and its correlation with insulin resistance,plasma adiponectin, and leptin ***: One hundred and five uncomplicatedobese subjects (87 women, 18 men, age 34.3±9.6 years,BmI 39.9±8.3 kg/m2)were studied. Serum ALT activity was evaluated. Insulin sensitivity was assessed by euglycemic hyperinsulinemic clamp (m index) and fasting insulin. plasma leptin and adiponectin levels were also ***: Serum ALT concentration in the whole group of uncomplicated obese subjects was 17.73±6.33 U/L with none of the subjects presenting ALT levels greater than 43 U/L and only 9 (11%) women and 3 (19%) men showed ALT levels ＞19 and ＞30 U/L for women and men,respectively. No significant difference was detected in serum ALT levels between severe obese subjects (BmI ＞40 kg/m2) and those with BmI ＜40 kg/m2 (18.63±6.25 vs 17.26±6.02 U/L). ALT was significantly correlated with fasting insulin (r = 0.485, p = 0.02) and triglycerides (r= 0.358, p= 0.03).CONCLUSION: Serum ALT activity is practically normal in uncomplicated obese subjects, independently of their obesity degree. These findings suggest the role of obesityrelated comorbidities and not of BmI as main risk factors for elevated ALT levels in obese subjects.

Focal epithelial hyperplasia in a human immuno-deficiency virus patient treated with laser surgery

Focal epithelial hyperplasia(FEH), or Heck's disease, is a rare disease of the oral mucosa; it is mostly found in children or young adults who are immunosuppressed and who live in regions with low socioeconomic status. It is characterized by asymptomatic papules on the oral mucosa, gingiva, tongue, and lips. Healing can be spontaneous, and treatment is indicated if there are aesthetic or functional complications. Human papillomavirus, especially genotypes 13 and 32, has been associated with FEH and is detected in the majority of lesions. Histopathologically, FEH is characterized by parakeratosis, epithelial hyperplasia, focal acanthosis, and fusion and horizontal outgrowth of epithelial ridges. A 37-year-old male patient was referred to the Department of Oral and maxillofacial Sciences at the Sapienza University of Rome, complaining of numerous exophytic lesions in his mouth. He stated that the lesions were not painful but he had experienced occasional bleeding after incidental masticatory trauma. He had received no previous treatment for the oral lesions. His medical history revealed that he was human immuno-deficiencyvirus positive and was a smoker with numerous, asymptomatic oral papules clinically and histologically corresponding to FEH. The labial and buccal mucosa were especially affected by lesions. Surgical treatment was performed using a 532-nm potassium titanyl phosphate laser(SmartLite, Deka, Florence, Italy) in continuous mode with a 300 μm fiber and power of 1.4 W(power density 1980.22 W/cm2). After anesthesia without vasoconstrictors, the lesions were tractioned with sutures or an Allis clamp and then completely excised. The lesions were preserved in 10% formalin for histological examination, which confirmed the clinical diagnosis of FEH. In this case, the laser allowed excellent control of bleeding, without postoperative sutures, and optimal wound healing.
