Minimally Invasive Surgery for necrotizing Pancreatitis: A Case Report

Introduction: necrotizing pancreatitis management is complex and varies significantly among clinicians. Minimally invasive approaches like transgastric necrosectomy via laparoscopy are emerging as effective treatment options. This case report underscores the technique’s efficacy, clinical outcomes, and role in reducing complications. Clinical Observation: A 59-year-old male with a history of smoking and alcoholism presented with severe abdominal pain, nausea, and vomiting. Over the following weeks, he developed symptoms including asthenia, weight loss, and melena. Diagnostic workup revealed severe anemia and Balthazar E necrotizing pancreatitis, with significant intra-abdominal fluid collections and signs of infection. After initial conservative management, the patient underwent transgastric necrosectomy via laparoscopy due to deteriorating clinical status. The procedure involved removing necrotic tissue and performing a cystogastroanastomosis and jejunostomy. Postoperative care included fasting, parenteral nutrition, broad-spectrum antibiotics, and enzymatic replacement. The patient recovered well, with reduced necrotic tissue on follow-up imaging, and was discharged twelve days post-surgery [1]. Conclusion: Transgastric necrosectomy by laparoscopy is a valuable first-line surgical option for patients with symptomatic necrotizing pancreatitis, particularly in cases without prior interventions. This minimally invasive technique helps reduce major complications and mortality, offering a less invasive alternative to traditional open necrosectomy. The multidisciplinary approach and careful postoperative management were crucial to the patient’s favorable outcome. The case highlights the potential of transgastric necrosectomy as an effective treatment strategy in managing complex pancreatitis cases, including those with associated duodenal perforation [2].

Influence of biomass burning on ozone levels in the Megalopolis of Central Mexico during the COVID-19 lockdown

The massive reductions in anthropogenic emissions resulting from the COVID-19 lockdown provided a unique opportunity to evaluate the effect of mitigation measures aiming to abate air *** Mexico,the total lockdown period took place during the dry-hot season when biomass burning activity is ***,we investigate the role of biomass burning emissions on regional ozone levels in the Megalopolis of Central *** studied period covers the lockdown phases 2 and 3,and the first month of the new *** applied a factor separation technique and process analysis to estimate the pure and synergistic contributions of emission reductions under lockdown and that from biomass burning to daily ozone maximum concentrations in 7 metropolitan areas of different states in the *** results revealed that biomass burning plumes likely masked the effect of massive reductions from mobile emissions,impacted the PBL development during phase 3 and favored transition and mixed nO_(x)-limited and VOC-limited regional *** contributed to increased ozone production in the middle to lower PBL by changing the regional background levels which potentially could bias high ozone production efficiency *** the Megalopolis contribution to economic and societal development at national scale,our study suggests that ozone mitigation measures during the dry-hot season targeting mainly mobile emissions will likely be offset by biomass burning plumes.A regional and synergic policy aiming to control biomass burning would help to reduce the occurrence of high ozone levels in Central Mexico with the co-benefit of tackling short-lived climate pollutants.

Background: the quality of life (QoL) of patients with pancreatic ductal adenocarcinoma (PDAC), with its limited survival, can be affected by chemotherapy-induced toxicity. The main objective was to evaluate the effect of introducing ocoxin oral solution (OOS) in combination with standard therapy on quality of life. Methods: Thirty patients were enrolled in an exploratory, prospective, single-centre clinical trial in the oncology department of “Hermanos Ameijeiras” University Hospital in Havana, Cuba. Quality of life was measured using the EORTC QLQ-C30 questionnaire and toxicity was assessed using the nCI-CTC-AE classification version 5.0. Results: There was stability in the scores over time for overall QoL and the functional scale criteria, while in terms of symptoms, fatigue, pain and loss of appetite were reduced. no grade 3 - 4 adverse events (AEs) were recorded, and only 14.9% of toxicities were classified as grade 2, and these were considered to be unrelated to OOS. Biochemical and nutritional parameters were normalised at 12 months compared to the baseline values. Conclusions: This clinical study is the first report of the use of OOS in patients with advanced pancreatic cancer, and demonstrates that it is able to maintain optimal quality of life with reduced severity of toxicity during and after combination treatment with gemcitabine-based chemotherapy.

Compelling shreds of evidence derived from both clinical and experimental research have demonstrated the crucial contribution of receptor for advanced glycation end products(RAGE)axis activation in the development of neoplasms,including gastric cancer(GC).This new actor in tumor biology plays an important role in the onset of a crucial and long-lasting inflammatory milieu,not only by supporting phenotypic changes favoring growth and dissemination of tumor cells,but also by functioning as a pattern-recognition receptor in the inflammatory response to Helicobacter pylori *** the present review,we aim to highlight how the overexpression and activation of the RAGE axis contributes to the proliferation and survival of GC cells as and their acquisition of more invasive phenotypes that promote dissemination and ***,the contribution of some single nucleotide polymorphisms in the RAGE gene as susceptibility or poor prognosis factors is also discussed.

Compelling evidence derived from clinical and experimental research has demonstrated the crucial contribution of chronic inflammation in the development of neoplasms,including gallbladder *** this regard,data derived from clinical and experimental studies have demonstrated that the receptor of advanced glycation end-products(RAGE)/AGEs axis plays an important role in the onset of a crucial and long-lasting inflammatory milieu,thus supporting tumor growth and *** are formed in biological systems or foods,and food-derived AGEs,also known as dietary AGEs are known to contribute to the systemic pool of *** they bind to RAGE,the activation of multiple and crucial signaling pathways are triggered,thus favoring the secretion of several proinflammatory cytokines also involved in the promotion of gallbladder cancer invasion and *** the present review,we aimed to highlight the relevance of the association between high dietary AGEs intakes and high risk for gallbladder cancer,and emerging data supporting that dietary intervention to reduce gallbladder cancer risk is a very attractive approach that deserves much more research efforts.

Use of δ^(18)O, δ^(13)C and nO_(3)^(-) to identify hydrogeochemical processes related to contamination in an aquifer located in central Mexico

In this work,an isotopic analysis of δ^(18)O,δ^(13)C,and nO_(3)^(-) concentrations was carried out to identify the origin and the processes related to the contamination of an aquifer located in the state of Guanajuato,*** research identified the possible sources of δ^(13)C in *** groundwater flow,CO_(2) participates in different hydrogeochemical reactions in which the dissolution of carbonates or biochemical processes related to biodegradation stand *** data of δ^(13)C,δ^(18)O,and the hydrogeochemical behavior of nO_(3)^(-) and HCO_(3)^(-) in water,in addition to isotopic data and the chemical composition of limestones in the study area,were determined to establish the isotopic signature and the processes undergone by the *** isotopic signature of rock and water samples indicated that metamorphic limestones contributed with carbon dioxide to deep groundwater,while in the upper aquifer,bacterial metabolic reactions during nitrification–denitrification could modify the isotopic signature of δ^(13)C in some wells,although atmospheric contribution also plays a *** modification of the carbon isotopic component is related to the precipitation of calcite in specific regions of the study area,input of atmospheric CO_(2),and soil(*** possible participation of C4-type plants in the assimilation-release of carbon).This process is not confirmed or completely ruled out in this study since agriculture is excessively developed throughout the *** joint interpretation of isotopic values and the hydrogeochemical behavior of major and conservative elements help in identifying possible pollution processes in which different carbon sources are related.

Compelling pieces of evidence derived from both clinical and experimental research has demonstrated the crucial contribution of diabetes mellitus(DM)as a risk factor associated with increased cancer incidence and mortality in many human neoplasms,including gastric cancer(GC).DM is considered a systemic inflammatory disease and therefore,this inflammatory status may have profound effects on the tumor microenvironment(TME),particularly by driving many molecular mechanisms to generate a more aggressive *** is an active driver in the modification of the behavior of many cell components of the TME as well as altering the mechanical properties of the extracellular matrix(ECM),leading to an increased ECM ***,DM can alter many cellular signaling mechanisms and thus favoring tumor growth,invasion,and metastatic potential,as well as key elements in regulating cellular functions and cross-talks,such as the microRnAs network,the production,and cargo of exosomes,the metabolism of cell stroma and resistance to *** the present review,we intend to highlight the mechanistic contributions of DM to the remodeling of TME in GC.

Modeling and multi-objective optimization of a gasoline engine using neural networks and evolutionary algorithms

In this paper, a multi-objective particle swarm optimization （MOPSO） algorithm and a nondominated sorting genetic algorithm II （nSGA-II） are used to optimize the operating parameters of a 1.6 L, spark ignition （SI） gasoline engine. The aim of this optimization is to reduce engine emissions in terms of carbon monoxide （CO）, hydrocarbons （HC）, and nitrogen oxides （nOx）, which are the causes of diverse environmental problems such as air pollution and global warming. Stationary engine tests were performed for data generation, covering 60 operating conditions. Artificial neural networks （Anns） were used to predict exhaust emissions, whose inputs were from six engine operating parameters, and the outputs were three resulting exhaust emissions. The outputs of Anns were used to evaluate objective functions within the optimization algorithms： nSGA-II and MOPSO. Then a decision-making process was conducted, using a fuzzy method to select a Pareto solution with which the best emission reductions can be achieved. The nSGA-II algorithm achieved reductions of at least 9.84%, 82.44%, and 13.78% for CO, HC, and nOx, respectively. With a MOPSO algorithm the reached reductions were at least 13.68%, 83.80%, and 7.67% for CO, HC, and nOx, respectively.

Quantification of the Mixture of Hydrothermal and Fresh Water in Tectonic Valleys

This study was conducted to identify the origin, hydrogeochemical processes and evolution of groundwater in a tectonic valley. This study was carried out with the aim of quantifying the proportions of groundwater flows contributing to the water chemistry abstracted in a zone of convergence favored by the presence of active faults. The study area is located in the Trans-Mexican Volcanic Belt. End members methodology was applied to identify the mixing of hydrothermal with fresh groundwater, where changes in the aquifer geology result in distinct groundwater chemical signatures. Ternary mixing was quantified using conservative elements. Moreover, other evolutionary processes, such as ion exchange and silicate weathering occur due to changes in the geology of the area. In ternary mixing, each of the end members is associated with the lithology through which it circulates. The local flow contributes 70% of the water to the system, the intermediate flow contributes 14%, and the regional flow contributes 16%. Three types of water are produced: na-HCO3, due to the interaction of water with volcanic rocks of rhyolitic composition, na-Mg-HCO3, due to the interaction of water with volcanic rocks of basaltic-andesite composition, and Ca-HCO3, due to the interaction of water with sedimentary calcareous rocks.

Simulation of erosion-deposition processes at basin scale by a physically-based mathematical model

The development and application of the physically-based and spatially-distributed mathematical model CTSS8-SED is presented. The model simulates hydrologic-hydraulic processes produced by storm events and related soil erosion and sediment transport processes at basin scale in lowland areas. The model simulates (i) storm runoff,(ii) soil detachment by raindrop impact and overland flow (gross sediment yield),(iii) sediment transport by overland flow and associated erosion-deposition processes and (iv) sediment transport by stream flow and riverbed erosion-deposition processes. A quasi two-dimensional representation of water flow and sediment transport routing is made by means of interconnected cells approach. The model is applied to simulate two flooding events in the Luduea Creek basin (Santa Fe, Argentina) occurred in April 1994 and March 2007 due to extraordinary rainfalls.
