目的:牙槽嵴缺损常会使种植治疗方案复杂化。骨再生的方法有许多种,其中包括引导性骨再生(guided bone regeneration,GBR)。本研究的目的是对当前可获得的应用于种植前或种植同期GBR中的不同类型的膜的相关知识进行总结。膜的主要作用是阻止上皮细胞和结缔组织细胞进入需要再生的区域,创造和维持多能干细胞及成骨细胞可自由生长的空间。数据来源:对以该主题发表的英文文章进行文献检索。选择了一定数量的研究用以回顾不同类型膜的主要特征,应用和效果。可吸收膜由天然或合成的聚合物制成,如胶原蛋白和脂肪族聚酯。其中,最常使用的是胶原蛋白,它们与牙周结缔组织具有相似的胶原蛋白成分。其他可用的材料有人,猪和牛的心包膜,人羊膜和绒毛膜,以及人脱细胞冻干真皮基质。GBR中使用的不可吸收膜有致密聚四氟乙烯(dense-polytetrafluoroethylene,d-PTFE)膜,膨体聚四氟乙烯(expanded-polytetrafluoroethylene,e-PTFE)膜,钛网和钛加强聚四氟乙烯膜。结论:不可吸收膜最常见的并发症是膜暴露。对这两种类型的膜来说,膜暴露都会对最终骨再生效果产生不利影响。垂直骨增量宜采用不可吸收膜。对于复合骨缺损,两种类型的膜都可获得良好的效果。

Spinal fusion remains the gold-standard treatment for several pathological spine conditions. Although, autologous Iliac Crest Bone Grafting is considered the goldstandard graft choice to promote spinal fusion; however, it is associated with significant donor site morbidity and a limited graft quantity. Therefore, several bone graft alternatives have been developed, to augment arthrodesis. The purpose of this review is to present the results of clinical studies concerning the use of demineralized bone matrix(DBM), alone or as a composite graft, in the spinal fusion. A critical review of the English-language literature was conducted on Pubmed, using key word "demineralized bone matrix", "DBM", "spinal fusion", and "scoliosis". Results had been restricted to clinical studies. The majority of clinical trials demonstrate satisfactory fusion rates when DBM is employed as a graft extender or a graft *** number of prospective randomized controlled trials(4 studies), have been performed comparing DBM to autologous iliac crest bone graft in spine fusion. The majority of the clinical trials demonstrate comparable efficacy of DBM when it used as a graft extender in combination with autograft, but there is no clinical evidence to support its use as a standalone graft material. Additionally, high level of evidence studies are required, in order to optimize and clarify the indications of its use and the appropriate patient population that will benefit from DBM in spine arthrodesis.

The intestinal lumen is a host place for a wide range of microbiota and sets a unique interplay between local immune system, inflammatory cells and intestinal epithelium, forming a physical barrier against microbial invaders and toxins. Bacterial translocation is the migration of viable or nonviable microorganisms or their pathogen-associated molecular patterns, such as lipopolysaccharide, from the gut lumen to the mesenteric lymph nodes, systemic circulation and other normally sterile extraintestinal sites. A series of studies have shown that translocation of bacteria and their products across the intestinal barrier is a commonplace in patients with liver disease. The deterioration of intestinal barrier integrity and the consulting increased intestinal permeability in cirrhotic patients play a pivotal pathophysiological role in the development of severe complications as high rate of infections, spontaneous bacterial peritonitis, hepatic encephalopathy, hepatorenal syndrome, variceal bleeding, progression of liver injury and hepatocellular carcinoma. Nevertheless, the exact cellular and molecular mechanisms implicated in the phenomenon of microbial translocation in liver cirrhosis have not been fully elucidated yet.

An insufficient cellular immune response seems to be critical for the immunopathogenesis of chronic hepatitis B virus *** have previously demonstrated no differences of T-lymphocyte subsets in blood between inactive hepatitis B s antigen(HBsAg) carriers and patients with HBeAg-negative chronic active hepatitis *** study investigated the peripheral blood cytokine profile in patients with HBeAg-negative chronic active hepatitis B infection(Group A,n = 21) and inactive HBsAg carriers(Group B,n = 13).Serum cytokines [interferon(IFN)-γ,tumor necrosis factor-α,interleukin(IL)-1b,IL-4,IL-12,IL-10,IL-2,IL-5,IL-8] were analyzed by using flow *** with chronic active disease presented with significantly decreased levels of IFN-γ and IL-10 compared to inac-tive carriers(P = 0.048 and P = 0.008,respectively).In HBeAg-negative chronic active hepatitis B patients,a significant negative correlation of IFN-γ levels with serum hepatitis B viral load was noted(P = 0.021).In conclusion,patients with HBeAg-negative chronic active hepatitis B and HBsAg inactive carriers display a different cytokine *** Th1 response observed in patients with chronic active hepatitis B could be implicated in the persistence of virus replication and ongoing progression of liver disease.

背景本研究中，我们探讨了微创髋关节成形术后创口持续输注0．3％罗哌卡因对疼痛和吗啡用量的影响。方法在这项双盲研究中，前瞻性纳入连续67例在蛛网膜下隙麻醉下择期行微创髋关节成形术的患者。外科医师将15cm长的有孔导管放入关节囊外。患者随机分为两组：在关闭切口前，创口内单次注入20ml的0．3％罗哌卡因（R-组）或0．9％生理盐水（P-组）。随后用弹性泵以8ml／h的速度分别持续泵入0．3％的罗哌卡因或者安慰剂至术后48小时。所有患者均给予静脉吗啡自控镇痛。记录48小时内患者吗啡用量、静息和运动时疼痛以及血浆罗哌卡因总浓度和游离浓度。术后随访3个月。结果两组-般资料和手术数据相似。术后第一个48小时期间，R-组平均吗啡用量显著低于P组：45．4±9．5VS69．7±9．6（P〈0．001）。R-组吗啡用量在术后第一个24小时期间平均减少14．4mg[95％可信区间（confidenceinterval，CI）12．6～16．1]，在术后第二个24小时期间平均减少20．8mg（95％CI为19．1～22．4）。R组患者静息和运动时疼痛评分均较低（P〈0．0001）。R-组患者平均满意度较基线增加了22．7％（95％CI为15．9～16．1）。R-组血浆罗哌卡因总浓度和游离浓度均低于中毒浓度。R-组在第24小时和第48小时时血浆罗哌卡因游离浓度分别是0．141μmol／ml和0．11μmol／ml。在术后3个月时，两组臀部疼痛和镇痛药用量相似，但是R．组患者创口接触（31．2；95％CI27．7～34．7）不适感和压迫不适感（24；95％CI为20．1-27．9）明显减轻（P〈0．0001）。结论微创髋关节置换术后关节囊外创口持续输注0．3％罗哌卡因是-种减少吗啡用量，改善术后镇痛质量的有效方法，术后3个月时该方法依然具有-定优势。

Steganography aims to hide the messages from unauthorized persons for various purposes,e.g.,military correspondence,financial transaction *** the data during transmission is of utmost importance these *** confidentiality,integrity,and availability of the data are at risk because of the emerging technologies and complexity in software applications,and therefore,there is a need to secure such systems and *** are various methodologies to deal with security issues when utilizing an open system like the *** research proposes a new technique in steganography within RGB shading space to achieve enhanced security compared with existing *** evaluate our approach with the help of diverse image quality evaluation techniques including MSE(Mean Square Error),RMSE(Root Mean Square Error),PSNR(Peak Signal-to-Noise Ratio),MAE(Mean Absolute Error),NCC(Normalized Cross-Correlation)and SSIM(Structural Similarity Index).Our experimental results demonstrate improved strength,intangibility,and security when contrasted with existing techniques and vindicate the effectiveness of this exploration *** proposed approach achieved a 3.6701%average higher score for PSNR Correlation than the next best existing ***,in PSNR with a variable amount of cipher embedded in the same images of the same dimensions,the proposed approach attained a 5.22%better *** the same size of cipher in images of different size resulted a 3.56%better score.
