Septic arthritis of the hand is a serious disease that often results in dysfunction of the joint or even the need to perform amputation of the *** rank second in the frequency of occurrence after lesions of the knee *** points concerning the etiology,the timing of the development of cartilage destruction and the development of osteomyelitis,approaches to surgical treatment,the duration of antibiotic therapy,and the start of rehabilitation measures remain the subject of numerous *** on a search in the PubMed,Web of Science and Google Scholar databases down to 1990-2021,publications on septic arthritis of the hand were found and *** following inclusion criteria were used in our review:(1) Septic arthritis of the hand;(2) Published in a peer review journal;(3) Written in English;and(4) Full text version *** were excluded if they met any of the following criteria:(1) Letters;(2) Articles published in abstract form only;and(3) Cadaveric *** arthritis of the hand was characterized by the most frequent damage to the joints of the index and middle fingers(> 50% of cases).Up to 90% of cases,the infection enters the joint as a result of penetrating trauma,animal bites,*** aureus became the most frequently isolated microorganism(30%-55%),and its polyantibiotic-resistant form Methicillin-resistant Staphylococcus aureus was found,according to various sources,from 0% to 73% among all isolated Staphylococcus *** arthritis,Pasteurella multocida(6%-11%) is often isolated as a result of animal *** cartilage destruction in the experiment developed within 24-48 h after *** clinical studies,the development of osteomyelitis was noted when treatment was delayed by more than 10 d.X-ray data during the first two weeks were *** of surgical treatment of septic *** and surgical treatment,and with the development of osteomyelitis,the implementation of arthro

As the first line of immune defense for Mycobacterium tuberculosis （Mtb）, macrophages also provide a major habitat for Mtb to reside in the host for years. The battles between Mtb and macrophages have been constant since ancient times. Triggered upon Mtb infection, multiple cellular pathways in macrophages are activated to initiate a tailored immune response toward the invading pathogen and regulate the cellular fates of the host as well. Toll-like receptors （TLRs） expressed on macrophages can recognize pathopatterns （PAMPs） on Mtb and mediate the production of immune-regulatory cytokines such as tumor necrosis factor （TNF） and type I Interferons （IFNs）. In addition, Vitamin d receptor （VdR） and Vitamin d-1- hydroxylase are up-regulated in Mtb-infected macrophages, by which Vitamin d participates in innate immune respon- ses. The signaling pathways that involve TNF, type I IFNs and Vitamin d are inter-connected, which play critical roles in the regulation of necroptosis, apoptosis, and autophagy of the infected macrophages. This review article summarizes cur- rent knowledge about the interactions between Mtb and macrophages, focusing on cellular fates of the Mtb-infected macrophages and the regulatory molecules and cellular pathways involved in those processes.

The finite element system CAPA-3d was utilized as the numerical simulation platform,two structural FE models of orthotropic steel bridge decks were set up,the static and dynamic FE simulations under different loading conditions were carried out,the special attention was given to identify the critical wheel load location,and the maximum tensile stress distribution and the variation of strain rate inside membrane *** result shows that the FE models are capable of simulating the realistic behavior of orthotropic steel *** distributions of strains and stresses inside the surfacing materials depend highly on wheel load level,wheel load frequency,wheel position,membrane bonding strength as well as the thicknesses and characteristics of surfacing *** maximum tensile stress of membrane layers is found around 0.4MPa,which coincides with the minimum requirement for the adhesive bonding strength of membrane material proposed by the standards NF P98-282and *** maximum membrane strain rate is found around 0.1,which is an important information that can be utilized for the characterization of membrane products.1tab,28figs,7refs.

Phase-field modeling is an effective but computationally expensive method for capturing the mesoscale morphological and microstructure evolution in ***,fast and generalizable surrogate models are needed to alleviate the cost of computationally taxing processes such as in optimization and design of *** intrinsic discontinuous nature of the physical phenomena incurred by the presence of sharp phase boundaries makes the training of the surrogate model *** develop a framework that integrates a convolutional autoencoder architecture with a deep neural operator(deepONet)to learn the dynamic evolution of a two-phase mixture and accelerate time-to-solution in predicting the microstructure *** utilize the convolutional autoencoder to provide a compact representation of the microstructure data in a low-dimensional latent *** deepONet is trained in the latent space,it can be used to replace the high-fidelity phase-field numerical solver in interpolation tasks or to accelerate the numerical solver in extrapolation tasks.

Achalasia is a primary motor disorder of the esophagus diagnosed manometrically in the clinical setting of dysphagia to both solids and liquids. Currently established treatment options include pneumatic dilation, laparoscopic Heller myotomy, botulinum toxin injection performed endoscopically, oral agents that relax the lower esophageal sphincter and esophagectomy for refractory, end-stage disease. despite their effectiveness, a significant proportion of patients eventually relapses and needs retreatment. In this setting, several new techniques are under investigation promising future enrichment of our therapeutic armamentarium for achalasic patients. Among them, peroral endoscopic myotomy and selfexpandable metal stents placed across the gastroesophageal junction represent the most encouraging modalities, as initial studies assessing their efficacy and safety indicate. This review highlights the role of selfexpandable metal stents in the management of patients with achalasia. Their possible position in the therapeutic algorithm of achalasia along with established and novel techniques is also assessed. Finally, the need for large prospective randomized trials is underlined in order to elucidate the numerous relevant issues.

Toxic substances released as a result of leaching from painted surfaces to the aquatic environment affect both fouling organisms and “non-target” biota. Artemia fransiscana nauplii have been considered a useful test system for the examination of toxicity for antifouling paints. In this study, we examined the effect of four “tin free” self-polishing copolymer (SPC) antifouling paints on the larval development of Artemia nauplii. Based on the L(S/V)50 values the order of toxicity of the antifouling paints was: ANTI F > SHARKSKIN > OCEAN T/F > MICRON. Furthermore, the body size of Artemia nauplii was significantly affected at lethal and above lethal L(S/V)5024h values. The body size of 48 h-aged nauplii exposed for the last 24 hours to each of the four SPC antifouling paints was significantly lower than that of the 48 h-aged controls (0.88 ± 0.030 mm). In addition, the body size of 72 h-aged nauplii maintained for the last 24 hours to pure synthetic seawater after exposure for 24 hours to each of the four SPC antifouling paints was significantly lower than that of the 72 h-aged controls (0.96 ±0.027 mm). Overall, the SPCs examined here were substantially toxic to Artemia nauplii, but with different toxicities and modes of action, as a result of the synergistic action of distinct components of the antifouling paints.

In the very interesting report published in Chinese Medical Journal concerning a 65-year-old hypertensive and hyperlipidemic male patient, with stent implantation 5 years previously, the patient developed anaphylactic shock accompanied by chest discomfort, palpitation, itchiness, nausea, vomiting, dyspnea, wheezing, abdominal pain, sweating, pale complexion, dizziness, and syncope following bread and milk consumption. The clinical symptomatology was associated with electrocardiographic and laboratory evidence of acute inferolateral myocardial infarction, and the patient recovered with epinephrine, antiallergic, and myocardial infarction protocol therapy including ticagrelor and Clexane. Coronary arteriography demonstrated lesions in the left trunk and the three coronary branches, but the stented areas were unobstructed. The allergen test revealed a Grade 2 （2.11 kUA/L） for wheat and Grade 0 for milk.

Purpose： Emergency department resuscitative thoracotomy is an intervention of last resort for the acutely dying victim of trauma. In light of improvements in pre-hospital emergency systems, improved operative strategies for survival such as damage control and improvements in critical care medicine, the most extreme of resuscitation efforts should be re-evaluated for the potential survivor, with success properly defined as the return of vital signs which allow transport of the patient to the operating room. Methods： A retrospective review of all patients at a suburban level I trauma center who underwent emergency department resuscitative thoracotomy as an adjunct to the resuscitation efforts normally delivered in the trauma receiving area over a 22 year period was performed. Survival of emergency department resuscitative thoracotomy was defined as restoration of vital signs and transport out of the trauma resuscitation area to the operating room. Results： Sixty-eight patients were identified, of whom 27 survived the emergency department resusci- tative thoracotomy and were transported to the operating room. Review of pre-hospital and initial hospital data between these potential long term survivors and those who died in the emergency department failed to demonstrate trends which were predictive of survival of emergency department resuscitative thoracotomy. The only subgroup which failed to respond to emergency department resuscitative thoracotomy was patients without signs of life at the scene who arrived to the treatment facility without signs of life. Conclusion： The patient population of the ＂potential survivor＂ has been expanded due to advances in critical care practices, technology, and surgical technique and every opportunity for survival should be provided at the outset. Emergency department resuscitative thoracotomy is warranted for any patient with thoracic or subdiaphragmatic trauma who presents in extremis with a history of signs of life at the scene or or
