Nonfunctional neuroendocrine tumors of the pancreas(NF-PNETs) are a heterogeneous group of neoplasms. Although rare, the incidence of NF-PNETs is increasing significantly. The classification of PNETs has evolved over the past decades and is now based on a proliferation grading system. While most NF-PNETs are slow growing, tumors with more aggressive biology may becomeincurable once they progress to unresectable metastatic disease. Tumors of higher grade can be suspected preoperatively based on the presence of calcifications, hypoenhancement on arterial phase computed tomography, positron emission technology avidity and lack of octreotide scan uptake. Surgery is the only curative treatment and is recommended for most patients for whom complete resection is possible. Liver-directed therapies(thermal ablation, transarterial embolization) can be useful in controlling unresectable hepatic metastatic disease. In the presence of unresectable progressive disease, somatostatin analogues, everolimus and sunitinib can prolong progression-free survival. This article provides a comprehensive review of NF-PNETs with special emphasis on recent advances in diagnosis and management.

The 2009 swine-origin influenza virus (S-OIV,H1N1 subtype) has developed into a new pandemic influenza as announced by the World Health *** order to uncover clues about the determinants for virulence and pathogenicity of the virus,we characterized the functional modules of the surface glycoprotein hemagglutinin (HA),the most important protein in molecular epidemiology and pathogenesis of influenza *** analyzed receptor binding sites,basic patch,neutralization antibody epitopes and T cell epitopes in the HA protein of the current S-OIV according to the corresponding functional and structural modules previously characterized in other H1 HA molecules or HA molecules of other *** compared their differences and similarities *** on the amino acids defined as the functional and structural modules,the HA protein of 2009 S-OIV should specifically bind to the human 2,*** D225G/E mutation in HA,which is found in some isolates,may confer dual binding specificity to the 2,3and 2,6-receptor based on previously reported *** HA variant contains two basic patches,one of which results in increased basicity,suggesting enhanced membrane fusion *** 2009 S-OIV HA also has an extra glycosylation site at position *** of the five antibody neutralization epitopes identified in A/RP/8/34(H1N1) were exposed,but the other was hidden by a glycosylation *** previously identified cytotoxic T cell epitopes in various HA molecules were summarized and their corresponding sequences in 2009 S-OIV HA were *** results are critical for understanding the pathogenicity of the virus and host immune response against the virus.

Focusing incident power into an area of high concentration is of significant interest for various *** optics,this has been traditionally achieved with lenses where a higher curvature and lens permittivity typically result in shorter focal distances(low f/D).In this work,we present designs and techniques for collecting,refracting and guiding incident light into an area of high power concentration(a hot spot)at extremely short ***,a flat low-profile focusing mechanism is presented using a hetero-junction of anisotropic metamaterials(MTMs).The hetero-junction is formed from two cleaved finite slabs of low(near zero)permittivity anisotropic MTMs with rotated optical *** MTMs have near zero longitudinal permittivity while matched in the transverse *** MTMs are shown to provide a unique ability to bend the transverse magnetic or p-polarized light away from the normal and along the interface,contrary to conventional dielectrics,and with minimal reflections;hence allowing for a low profile *** in the optical regime are presented using periodic bilayers of metal and *** proposed hetero-junction focusing device concentrates the normally incident plane wave and/or beam into a corresponding focal region similar to a lens via multiple *** hetero-junction is capable of creating a hot spot very close to the device,much closer than dielectric lenses and it significantly outperforms the size requirements of thick high curvature lenses with low f/D *** proposed designs can find applications in various scenarios including solar and thermo photovoltaics,photodetectors,concentrated photovoltaics,non-imaging optics,micro-and nano-Fresnel lenses.

Inflammatory bowel disease(IBD)comprises two distinct conditions:ulcerative colitis and Crohn’s disease,both of which are chronic,relapsing disorders carrying significant morbidity,mortality and healthcare *** growing attention to coordinated healthcare for patients with chronic systemic diseases,this review focuses on the care of IBD patients in remission,their concerns,quality of life,follow-up,the role of primary care physicians and the IBD-specific aspects of long-term *** did an extensive PubMed search for articles pertaining to IBD patients in remission and,along with the authors’experience,formulated a comprehensive *** difficulties faced by IBD patients in remission include but are not limited to education and employment concerns,psychosocial issues,problems related to health insurance,nutrition,fertility and *** review also addresses newer treatment modalities,the debatable effects of smoking on IBD and the importance of *** in remission can be a challenge due to its multifaceted nature;however,with a coordinated approach by gastroenterologists and other involved practitioners,several of these issues can be addressed.

The progress in treatment against hepatitis B virus(HBV)with the development of effective and well tolerated nucleotide analogues(NAs)has improved the outcome of patients with HBV decompensated cirrhosis and has prevented post-transplant HBV *** review summarizes updated issues related to the management of patients with HBV infection before and after liver transplantation(LT).A literature search using the PubMed/Medline databases and consensus documents was ***-transplant therapy has been initially based on lamivudine,but entecavir and tenofovir represent the currently recommended first-line NAs for the treatment of patients with HBV decompensated *** LT,the combination of HBV immunoglobulin(HBIG)and NA is considered as the standard of care for prophylaxis against HBV *** combination of HBIG and lamivudine is related to higher rates of HBV recurrence,compared to the HBIG and entecavir or tenofovir *** HBIG-free prophylactic regimens,entecavir and tenofovir should be the first-line *** choice of treatment for HBV recurrence depends on prior prophylactic therapy,but entecavir and tenofovir seem to be the most attractive ***,liver grafts from hepatitis B core antibody(anti-HBc)positive donors can be safely used in hepatitis B surface antigen negative,preferentially anti-HBc/anti-hepatitis B surface antibody positive recipients.
