This case report describes a 69-year-old man presen-ting with an extensive subcutaneous emphysema in his neck and generalized peritonitis caused by a lower gastrointestinal tract perforation. This case emphasizes that subcutaneous emphysema patients with negative thoracic findings should be scrutinized for signs of retroperitoneal hollow viscus perforation.

This paper addresses fully space-time adaptive magnetic field computations. We describe an adaptive Whitney finite element method for solving the magnetoquasistatic formulation of Maxwell＇s equations on unstructured 3D tetrahedral grids. Spatial mesh re- finement and coarsening are based on hierarchical error estimators especially designed for combining tetrahedral H（curl）-conforming edge elements in space with linearly implicit Rosenbrock methods in time. An embedding technique is applied to get efficiency in time through variable time steps. Finally, we present numerical results for the magnetic recording write head benchmark problem proposed by the Storage Research Consortium in Japan.

Headwater streams play a major role for provision of ecosystem services,*** *** investigated a high-altitude headwater catchment of the Kharaa River(including 411st-order rivers)to understand the impact of land cover(especially forest cover),environment and human usage on runoff,chemical water quality and macroinvertebrate fauna in a river basin under discontinuous permafrost conditions in an arid,sparsely populated region of *** verify our hypotheses that different landuses and environmental impacts in permafrost headwaters influence water quality,we investigated 105 sampling sites,37 of them at intermittent stream sections without water *** was positively impacted by land cover types steppe,grassland and forest and negatively by shrubland,forest burnt by wild fires(indicating a reduction of permafrost)and *** quality was affected by altitude,longitude and latitude,shrub growth and water *** diversity of macroinvertebrates was driven by water temperature,iron content of the water,flow velocity,and subbasin size(adjusted R^(2)=0.54).Sample plots clustered in three groups that differed in water chemistry,macroinvertebrate diversity,species composition and *** study confirms that steppes and grasslands have a higher contribution to runoff than forests,forest cover has a positive impact on water quality,and diversity of macroinvertebrates is higher in sites with less nutrients and *** excellent ecological status of the upper reaches of the Kharaa is severely threatened by forest fires and human-induced climate change and urgently needs to be conserved.

Breast cancer is a ubiquitous disease and one of the leading causes of death in women in western societies. With overall increasing survival rates, the number of patients who need post-mastectomy reconstruction is on the rise. Especially since its psychological benefits have been broadly recognized, breast reconstruction has become a key component of breast cancer treatment. Evolving from the early beginnings of breast reconstruction with synthetic implants in the 1960 s, microsurgical tissue transfer is on the way to become the gold standard for post oncology restoration of the breast. Particularly since the advent of perforator based free flap surgery, free tissue transfer has become as safe option for breast reconstruction with low morbidity. The lower abdominal skin and subcutaneous fat tissue typically offer enough volume to create an aesthetically satisfying breast mound. Nowadays, the most commonly used flap from this donor site is the deep inferior epigastric artery perforator flap. If the lower abdomen is not available as a donor site, the gluteal area and thigh provide a number of flaps suitable for breast recon-struction. If the required breast volume is small, and there is enough tissue available on the upper medial thigh, then a transverse upper gracilis flap may be a practicable method to reconstruct the breast. In case of a higher amount of required volume, a gluteal artery perforator flap is the best choice. However, what is crucial in addition to selecting the best flap option for the individual patient is the timing of the operation. In patients with confirmed post-mastectomy radiation therapy, it is advisable to perform microvascular breast reconstruction only in a delayed fashion.

The initial inflammatory phase of bone fracture healing represents a critical step for the outcome of the healing process. However, both the mechanisms initiating this inflammatory phase and the function of immune cells present at the fracture site are poorly understood. In order to study the early events within a fracture hematoma, we established an in vitro fracture hematoma model： we cultured hematomas forming during an osteotomy （artificial bone fracture） of the femur during total hip arthroplasty （THA） in vitro under bioenergetically controlled conditions. This model allowed us to monitor immune cell populations, cell survival and cytokine expression during the early phase following a fracture. Moreover, this model enabled us to change the bioenergetical conditions in order to mimic the in vivo situation, which is assumed to be characterized by hypoxia and restricted amounts of nutrients. Using this model, we found that immune cells adapt to hypoxia via the expression of angiogenic factors, chemoattractants and pro-inflammatory molecules. In addition, combined restriction of oxygen and nutrient supply enhanced the selective survival of lymphocytes in comparison with that of myeloid derived cells （i.e., neutrophils）. Of note, non-restricted bioenergetical conditions did not show any similar effects regarding cytokine expression and/or different survival rates of immune cell subsets. In conclusion, we found that the bioenergetical conditions are among the crucial factors inducing the initial inflammatory phase of fracture healing and are thus a critical step for influencing survival and function of immune cells in the early fracture hematoma.

In this work, we present our theory and principles of the mathematical foundations of Lobachevskian (hyperbolic) astrophysics and cosmology which follow from a mathematical interpretation of experimental data in a Lobachevskian non-expanding Universe. Several new scientific formulas of practical significance for astrophysics, astronomy, and cosmology are presented. A new method of calculating (from experimental data) the curvature of a Lobachevskian Universe is given, resulting in an estimated curvature-K on the order of 10−52 m−2. Our model also estimates the radius of the non-expanding Lobachevskian Universe in a Poincare ball model as approximately 14.9 bly. A rigorous theoretical explanation in terms of the fixed Lobachevskian geometry of a non-expanding Universe is provided for experimental data acquired in the Supernova Project, showing an excellent agreement between experimental data and our theoretical formulas. We present new geometric equations relating brightness dimming and redshift, and employ them to fully explain the erroneous reasoning and erroneous conclusions of Perlmutter, schmidt, Riess and the 2011 Nobel Prize Committee regarding “accelerated expansion” of the Universe. We demonstrate that experimental data acquired in deep space astrophysics when interpreted in terms of Euclidean geometry will result in illusions of space expansion: an illusion of “linear space expansion”—Hubble, and an illusion of “accelerated (non-linear) space expansion”—Perlmutter, schmidt, Riess.
