Brain protective effect of dexmedetomidine vs propofol for sedation during prolonged mechanical ventilation in non-brain injured patients

BACKGROUND Dexmedetomidine and propofol are two sedatives used for long-term sedation.It remains unclear whether dexmedetomidine provides superior cerebral protection for patients undergoing long-term mechanical ventilation.AIM To compare the neuroprotective effects of dexmedetomidine and propofol for sedation during prolonged mechanical ventilation in patients without brain injury.METHODS Patients who underwent mechanical ventilation for>72 h were randomly assigned to receive sedation with dexmedetomidine or propofol.The Richmond Agitation and Sedation Scale(RASS)was used to evaluate sedation effects,with a target range of-3 to 0.The primary outcomes were serum levels of S100-βand neuron-specific enolase(NSE)every 24 h.The secondary outcomes were remifentanil dosage,the proportion of patients requiring rescue sedation,and the time and frequency of RASS scores within the target range.RESULTS A total of 52 and 63 patients were allocated to the dexmedetomidine group and propofol group,respectively.Baseline data were comparable between groups.No significant differences were identified between groups within the median duration of study drug infusion[52.0(IQR:36.0-73.5)h vs 53.0(IQR:37.0-72.0)h,P=0.958],the median dose of remifentanil[4.5(IQR:4.0-5.0)μg/kg/h vs 4.6(IQR:4.0-5.0)μg/kg/h,P=0.395],the median percentage of time in the target RASS range without rescue sedation[85.6%(IQR:65.8%-96.6%)vs 86.7%(IQR:72.3%-95.3),P=0.592],and the median frequency within the target RASS range without rescue sedation[72.2%(60.8%-91.7%)vs 73.3%(60.0%-100.0%),P=0.880].The proportion of patients in the dexmedetomidine group who required rescue sedation was higher than in the propofol group with statistical significance(69.2%vs 50.8%,P=0.045).Serum S100-βand NSE levels in the propofol group were higher than in the dexmedetomidine group with statistical significance during the first six and five days of mechanical ventilation,respectively(all P<0.05).CONCLUSION Dexmedetomidine demonstrated stronger protective effects on the brain compare

The Lower Cambrian shale gas in the western Hubei area, South China has a great resource prospect, but the gas-in-place（GIP） content in different sedimentary facies varies widely, and the relevant mechanism has been not well understood. In the present study, two sets of the Lower Cambrian shale samples from the Wells YD4 and YD5 in the western Hubei area, representing the deep-water shelf facies and shallowwater platform facies, respectively, were investigated on the differences of pore types, pore structure and methane adsorption capacity between them, and the main controlling factor and mechanism of their methane adsorption capacities and GIP contents were discussed. The results show that the organic matter（OM） pores in the YD4 shale samples are dominant, while the inorganic mineral（IM） pores in the YD5 shale samples are primary, with underdeveloped OM pores. The pore specific surface area（SSA） and pore volume（PV） of the YD4 shale samples are mainly from micropores and mesopores, respectively,while those of the YD5 shale samples are mainly from micropores and macropores, respectively. The methane adsorption capacity of the YD4 shale samples is significantly higher than that of the YD5 shale samples, with a maximum absolute adsorption capacity of 3.13 cm3/g and 1.31 cm3/g in average,respectively. Compared with the shallow-water platform shale, the deep-water shelf shale has a higher TOC content, a better kerogen type and more developed OM pores, which is the main mechanism for its higher adsorption capacity. The GIP content models based on two samples with a similar TOC content selected respectively from the Wells YD4 and YD5 further indicate that the GIP content of the deep-water shelf shale is mainly 3 4 m3/t within a depth range of 1000—4000 m, with shale gas exploration and development potential, while the shallow-water platform shale has normally a GIP content of little shale gas potential. Density-based ship detection in SAR images:Extension to a self-similarity perspective

Nonlocal self-similarity is an important property of Synthetic Aperture Radar(SAR)images to characterize the repetitiveness of features embodied by SAR images within nonlocal areas and has been used for enhancement of SAR images.Existing SAR ship detectors often independently handle small sub-images cropped from a large marine SAR image and do not exploit the nonlocal self-similarity therein.In this paper,we propose a new ship detector from the perspective of nonlocal self-similarity in SAR images to improve the ship detection performance,basically including three stages:prescreening,intra-cue calculation,and inter-cue calculation.In the prescreening stage,we design a new Histogram-based Density(HD)feature to rapidly select candidate sub-images potentially containing ship targets from a large SAR image.In the intra-cue calculation stage,target cues within a single candidate sub-image are extracted.In the inter-cue calculation stage,thanks to the nonlocal self-similarity among different candidate sub-images in terms of density features,we innovatively extract a weighted superpixel-HD map to obtain accumulated intracues across all the candidate sub-images.Finally,for each candidate sub-image,we fuse its inter-cue and intra-cue to obtain final detection results.Experimental results based on real SAR images show that our newly proposed method provides a better target-to-clutter contrast and ship detection performance than those of other state-of-the-art detection approaches.

Elastography can be used as a diagnostic method for quantitative characterization of tissue hardness information and thus,differential changes in pathophysiological states of tissues.In this study,we propose a new method for shear wave elastography(SWE)based on laser-excited shear wave,called photoacoustic shear wave elastography(PASWE),which combines photoacoustic(PA)technology with ultrafast ultrasound imaging.By using a focused laser to excite shear waves and ultrafast ultrasonic imaging for detection,high-frequency excitation of shear waves and noncontact elastic imaging can be realized.The laser can stimulate the tissue with the light absorption characteristic to produce the thermal expansion,thus producing the shear wave.The frequency of shear wave induced by laser is higher and the frequency band is wider.By tracking the propagation of shear wave,Young’s modulus of tissue is reconstructed in the whole shear wave propagation region to further evaluate the elastic information of tissue.The feasibility of the method is verified by experiments.Compared with the experimental results of supersonic shear imaging(SSI),it is proved that the method can be used for quantitative elastic imaging of the phantoms.In addition,compared with the SSI method,this method can realize the noncontact excitation of the shear wave,and the frequency of the shear wave excited by the laser is higher than that of the acoustic radiation force(ARF),so the spatial resolution is higher.Compared to the traditional PA elastic imaging method,this method can obtain a larger imaging depth under the premise of ensuring the imaging resolution,and it has potential application value in the clinical diagnosis of diseases requiring noncontact quantitative elasticity.

Zero trust architecture(ZTA)is a paradigm shift in how we protect data,stay connected and access resources.ZTA is non-perimeter-based defence,which has been emerging as a promising revolution in the cyber security field.It can be used to continuously maintain security by safeguarding against attacks both from inside and outside of the network system.However,ZTA automation and orchestration,towards seamless deployment on real-world networks,has been limited to be reviewed in the existing literature.In this paper,we first identify the bottlenecks,discuss the background of ZTA and compare it with traditional perimeter-based security architectures.More importantly,we provide an in-depth analysis of state-of-the-art AI techniques that have the potential in the automation and orchestration of ZTA.Overall,in this review paper,we develop a foundational view on the challenges and potential enablers for the automation and orchestration of ZTA.

Characterizing structure of cross-disciplinary impact of global disciplines:A perspective of the Hierarchy of Science

Purpose:Interdisciplinary fields have become the driving force of modern science and a significant source of scientific innovation.However,there is still a paucity of analysis about the essential characteristics of disciplines’cross-disciplinary impact.Design/methodology/approach:In this study,we define cross-disciplinary impact on one discipline as its impact to other disciplines,and refer to a three-dimensional framework of variety-balance-disparity to characterize the structure of cross-disciplinary impact.The variety of cross-disciplinary impact of the discipline was defined as the proportion of the high cross-disciplinary impact publications,and the balance and disparity of cross-disciplinary impact were measured as well.To demonstrate the cross-disciplinary impact of the disciplines in science,we chose Microsoft Academic Graph(MAG)as the data source,and investigated the relationship between disciplines’cross-disciplinary impact and their positions in the Hierarchy of Science(HOS).Findings:Analytical results show that there is a significant correlation between the ranking of cross-disciplinary impact and the HOS structure,and that the discipline exerts a greater cross-disciplinary impact on its neighboring disciplines.Several bibliometric features that measure the hardness of a discipline,including the number of references,the number of cited disciplines,the citation distribution,and the Price index have a significant positive effect on the variety of cross-disciplinary impact.The number of references,the number of cited disciplines,and the citation distribution have significant positive and negative effects on balance and disparity,respectively.It is concluded that the less hard the discipline,the greater the cross-disciplinary impact,the higher balance and the lower disparity of cross-disciplinary impact.Research limitations:In the empirical analysis of HOS,we only included five broad disciplines.This study also has some biases caused by the data source and applied regression models.Practical implications:

Improving the energy conversion efficiency in metallic fuel(e.g.,Al)combustion is always desirable but challenging,which often involves redox reactions of aluminum(Al)with various mixed oxidizing environments.For instance,Al-O reaction is the most common pathway to release limited energy while Al-F reaction has received much attentions to enhance Al combustion efficiency.However,microscopic understanding of the Al-O/Al-F reaction dynamics remains unsolved,which is fundamentally necessary to further improve Al combustion efficiency.In this work,for the first time,Al-O/Al-F reaction dynamic effects on the combustion of aluminum nanoparticles(n-Al)in oxygen/fluorine containing environments have been revealed via reactive molecular dynamics(RMD)simulations meshing together combustion experiments.Three RMD simulation systems of Al core/O_(2)/HF,n-Al/O_(2)/HF,and n-Al/O_(2)/CF4 with oxygen percentage ranging from 0%to 100%have been performed.The n-Al combustion in mixed O_(2)/CF_4 environments have been conducted by constant volume combustion experiments.RMD results show that Al-O reaction exhibits kinetic benefits while Al-F reaction owns thermodynamic benefits for n-Al combustion.In n-Al/O_(2)/HF,Al-O reaction gives faster energy release rate than Al-F reaction(1.1 times).The optimal energy release efficiency can be achieved with suitable oxygen percentage of 10%and 50%for n-Al/O_(2)/HF and n-Al/O_(2)/CF_4,respectively.In combustion experiments,90%of oxygen percentage can optimally enhance the peak pressure,pressurization rate and combustion heat.Importantly,Al-O reaction prefers to occur on the surface regions while Al-F reaction prefers to proceed in the interior regions of n-Al,confirming the kinetic/thermodynamic benefits of Al-O/Al-F reactions.The synergistic effect of Al-O/Al-F reaction for greatly enhancing n-Al combustion efficiency is demonstrated at atomicscale,which is beneficial for optimizing the combustion performance of metallic fuel.

Quantitative studies on charcoalification:Physical and chemical changes of charring wood

Charcoal is commonly preserved in both natural and artificial sediments,and is intensively used in paleontological,paleoenvironmental,and archaeological studies due to the abundant bio-information it contains.The biochemical properties of charcoal are also used for paleoclimatic reconstruction;however,the reliability of this approach has been challenged due to a lack of clarity on how physicochemical properties change during the charring process,as well as the temperatures required for charcoalification.To address this lack,in this study,Qinghai spruce and Chinese pine wood samples from the northeastern Tibetan Plateau were heated at different temperatures and for different lengths of time under restricted oxygen conditions.The reflectance;carbon,nitrogen,and oxygen content;and tracheid morphology were quantified before and after heating to assess changes related to the charring process.Archaeological charcoal remains were then evaluated to determine the charcoalification temperatures by comparing with the experimental results.The minimum temperature required for wood charcoalification was∼300℃,while temperatures recorded by archaeological charcoal were concentrated at 400-500℃.During the charring experiments,the tracheid cell walls gradually homogenized,and tracheid cell wall thickness and lumen area decreased by∼20%.On average,50%mass losses were observed;the carbon and oxygen content(%wt.)approximately changed from 47%to 60%and 48%to 35%respectively,while the nitrogen content(%wt.)fluctuated around 0.2%.The reflectance increased slightly from 0%to 0.5%.We propose that the charcoalification of wood tissue refers to charring(in restricted air)and carbonization(in the almost absence of air)when the wood is exposed to a heat source,which then finally transforms into a black,inert solid.This quantitative study provided valuable data and a thorough assessment of the process of wood charcoalification,as well as accurately estimated the feasibility of using charcoal physicochemical properties in paleoclimatic research

Fine-tuning of the electron-deficient unit in A-DA1D-A typed small-molecule acceptors (SMAs) plays a crucial role in developing efficient SMAs for organic solar cells (OSCs).Here,we developed a SMA based on benzo[4,5]thieno[2,3-b]quinoxaline,designated as QW1,as well as three SMAs based on 1-methylindoline-2,3-dione,identified as QW2,QW3,and QW4.Compared with QW2,QW1 displays slightly blue-shifted absorption spectra and a lower LUMO energy level due to the stronger electron-withdrawing capability of BTQx in contrast to MDO.On the other hand,the introduction of a bromine atom in QW3 and QW4 causes a blue shift in absorption and a reduction in the LUMO energy level compared to QW2.Density functional theory analysis reveals that QW1 exhibits the best molecular planarity,which endows QW1 with larger electron mobility and tighter molecular stacking.Consequently,PM6:QW1 device affords a better efficiency of 15.63% than those of the devices based on QW2 (14.25%),QW3 (13.21%) and QW4 (15.03%).Moreover,the QW4-based device yields the highest open-circuit voltage of 0.933 V,and the PM6:L8-BO:QW4 ternary device realizes a PCE of 19.03%.Overall,our work demonstrates that regulation of electron-deficient central units is an effective strategy to improve the photovoltaic performance of the resulting A-DA1D-A SMAs.
